α-Lipoic Acid Activates the Nrf2/HO-1 Pathway to Ameliorate Arsenic-Induced Multiorgan Oxidative Damage.
α-Lipoic acid (α-LA) is a natural dietary antioxidant with metal-chelating properties. Inorganic arsenic, a prevalent environmental contaminant in agricultural products, induces systemic oxidative damage upon chronic exposure. However, whether α-LA mitigates arsenic-induced multiorgan toxicity and its underlying mechanisms remain unclear. This study integrated network analysis with in vivo and in vitro validation to investigate this issue. Our results showed that α-LA significantly enhanced systemic antioxidant defenses and alleviated arsenic-induced damage in multiple organs, particularly the liver and kidneys. Mechanistically, α-LA promoted nuclear translocation of nuclear factor erythroid 2-related factor 2 (Nrf2) and subsequent upregulation of heme oxygenase-1 (HO-1). Importantly, siRNA-mediated knockdown of Nrf2 significantly inhibited the protective effects of α-LA. These findings reveal that α-LA counteracts arsenic-induced multiorgan oxidative damage by activating the Nrf2/HO-1 signaling axis, highlighting its potential as an intervention for populations exposed to agricultural arsenic and warranting further clinical investigation.

يهدف البحث إلى تحليل دور منطقة التجارة الحرة القارية الأفريقية في تحقيق التكامل الاقتصادي والاندماج الأفريقي، من خلال دراسة الأطر النظرية والتاريخية للتكامل الاقتصادي الأفريقي وتحليل مراحل تطور منطقة التجارة الحرة القارية، وتقييم الأثار الاقتصادية المتوقعة والتحديات التشغيلية التي تواجهها. إن منطقة التجارة الحرة القارية تمثل نقلة نوعية في مسيرة التكامل الاقتصادي الأفريقي، حيث تسعى لإنشاء سوق قارية موحدة تخدم أكثر من (1.3) مليار نسمة، وزيادة التجارة البينية الأفريقية بنسبة (52.3%) بحلول عام 2035، مما يساهم برفع الناتج المحلي الإجمالي (GDP) للقارة بنسبة (1-3%) سنوياً. ويرتهن نجاح منطقة التجارة الحرة القارية بإمكانات معالجة التحديات الهيكلية المؤسسية، بجانب توافر إرادة سياسية حقيقية، تدفع صوب بناء شراكات فاعلة بين جميع الأطراف المعنية، ويقدم البحث مجموعة من التوصيات لتعزيز فعالية المنطقة، ولعل أبرزها تعزيز التكامل في سلاسل القيمة الإقليمية، وتطوير البنية التحتية الداعمة، وبناء القدرات المؤسسية، وتنفيذ برامج دعم للدول الأقل نمواً، وتطوير آليات تمويل مبتكرة للمشاريع الإقليمية.

يهدف البحث الحالي الى تحليل وتفسير العلاقة بين القيادة الاخلاقية وابعاد الانحراف في مكان العمل وإمكانية الاستفادة من الطرح المعرفي والسلوكي التي فسرتها محاور البحث النظرية والتطبيقية وفي ضوء ذلك يمكن تشخيص مشكلة البحث الأساسية المتمثلة هل هنالك علاقة ارتباط واثر ذو دلالة احصائية بين فاعلية القيادة الاخلاقية للقيادة في الحد ابعاد الانحراف في مكان العمل ومن خلال تحديد مشكلة البحث والوصول الى افضل الحلول للإشكاليات البحث تم بناء نموذج افتراضي يعكس علاقة الارتباط والاثر بين متغيرات البحث المتمثلة بالقيادة الاخلاقية وقدرات الانحراف في مكان العمل وبالتالي الوصول الى افضل وادق النتائج التي يمكن الاستفادة منها في إيجاد الحلول لمشكلة البحث ومن اجل في الحد ذلك تم اختيار البيئة الأكثر ملائمة لمتغيرات البحث والمتمثلة بشركة اللدائن العامة في بعقوبة حيث تم تحديد العينة القصدية من أصحاب القرار في الشركة المبحوثة والبالغة 61 ومن اجل تفسير محاور البحث النظري والتطبيقي تم اختيار المنهج الوصفي التحليلي لوصول الى نتائج سليمة ودقيقة حيث تم استخدام أداة البحث الرئيسية المتمثلة باستمارة البحث ومجموعة من الأساليب الإحصائية المتمثلة بالوسط الحسابي والانحراف المعياري والمعنوية وانحدار المسار وغيرها التي من شانها في الحد النتائج الأفضل التي يمكن الاستفادة منها في الحد اهداف البحث.

يُعدُّ موضوع الاقلية اليهـود فـي تركيا وموقف الحكومات مـنهم خلال الحقبة (1923-1980) مـن المواضيع المهمة، كون اليهـود يشكلون احدى الاقليات التي لعبت دورا كبيرا خلال العهد العثماني والذي بدأ بتزايد هجرتهم من اوربا للدولة العثمانية حتى وصل الامر الى العمل على انشاء وطن قومي لهم في ارض فلسطين على الرغم من فشل محاولات هرتزل مع السلطان عبد الحميد واستمر عمل اللوبي الصهيوني بمجيئ الوعد المشؤوم وعد بلفور الذي زاد من الهجرة اليهودية الى ارض فلسطين، اتسم موقف الاتراك من اليهود بالتذبذب بين الرغبة بالتتريك والأجرءات القانونية والأقتصادية وعليه نجد إتساع نشاطهم خلال قيام الجمهورية التركية فـي النشاط الصناعي والتجاري بل كان الهيود من اكثر الموثرين في النشاطات الاقتصادية اذ تطور بشكل كبير مما اسهم في زيادة الاقتصاد التركي، فضلاً عن دورهم الخطير فـي السياسة الداخلية والخارجية، ومشاركتهم في مناصب عليا في الدولة، لكن ذلك لم يوقف الهجرة الى ارض فلسطين، وبعد قيام دولتهم المزعومة في فلسطين اعترفت جمهورية تركيا بإسرائيل رغبةً منها للتقرب من المعسكر الغربي.

Anomalous origin of the left coronary artery from the pulmonary artery is a rare congenital coronary anomaly. This case report describes an adult male with anomalous origin of the left coronary artery from the pulmonary artery, in whom transthoracic echocardiography revealed mitral valve prolapse without apparent anomalies of the coronary ostia. The definitive anatomical diagnosis was established using invasive coronary angiography and coronary computed tomography angiography. This case underscores the indispensable value of multimodality imaging in the diagnosis of anomalous origin of the left coronary artery from the pulmonary artery.

Rising detection of unruptured intracranial aneurysms (UIAs) in China, driven by an aging population and increased neuroimaging utilization, coincides with significant regional disparities in neurointerventional resources. This study characterizes contemporary management practices nationwide. A WeChat-based survey distributed to cerebrovascular specialists in 2024 assessed clinical decision-making, resource availability, and case-based preferences. Resource disparities were pronounced: while computed tomography angiography (CTA) was widely accessible (82.4%), dual-arm digital subtraction angiography (DSA=32.4%) and hybrid operating rooms (36.4%) remained limited. Domestic flow diverters surpassed international devices (47.1% vs. 31.4%). Clinician experience significantly influenced management: physicians with >10 years' experience intervened at smaller UIA sizes (>5 mm; 55.5% vs. 40.1% in <2-year cohort, p=0.03) and preferred flow diverters for incidental aneurysms (80.7% vs. 38.3%, p<0.001). Coiling dominated acute aneurysmal subarachnoid hemorrhage (SAH) management, but senior neurosurgeons favored clipping with hematoma evacuation (66.5% vs. 35.3%, p<0.001). Significant practice variations persist in China, driven by experience gaps and regional resource inequities. Standardized training and equitable resource allocation are urgently needed to optimize aneurysm care.

Case Report: Management of cervical intramedullary spinal cord metastasis from NSCLC with a literature review
BackgroundIntramedullary spinal cord metastases (ISCM) from non-small cell lung cancer (NSCLC) are rare and carry a grave prognosis. Cervical segment involvement is exceptionally uncommon, and its distinct clinicopathological profile is not well characterized.MethodsWe present the case of a 72-year-old male with a history of NSCLC who developed acute quadriparesis and sphincter dysfunction. Cervical magnetic resonance imaging (MRI) revealed a C7 intramedullary mass. The patient underwent C6-T1 laminectomy with microsurgical gross-total resection. Histopathology confirmed metastatic lung adenocarcinoma. We supplemented this case with a systematic literature review of NSCLC-derived ISCM cases to summarize demographic, clinical, and therapeutic outcomes.ResultsHistopathology confirmed metastatic lung adenocarcinoma. Postoperatively, the patient's neurological function improved. Although local recurrence was detected at 11 months and treated with salvage radiotherapy, the patient nevertheless maintained ambulatory function and was alive at the 18-month follow-up. Our literature review of 68 cases with complete data identified a male predominance (4.2:1 ratio) and a mean age of 58.1 years. The cervical spine was the most commonly involved segment (47.1%). Analysis of treatment modalities revealed that multimodal therapy, particularly the combination of surgery and chemotherapy (potentially incorporating modern agents such as immune checkpoint inhibitors), was associated with improved survival, with a mean overall survival of 15.0 months in this subgroup. This paradigm, centered around maximal safe resection, successfully achieved long-term functional preservation and survival.ConclusionCervical ISCM from NSCLC represents one of the most challenging complications in spinal oncology. This case, supported by our literature review, provides a surgical-led, multimodal management template for spine surgeons, demonstrating that aggressive yet strategic intervention can achieve favorable long-term neurological and survival outcomes.

An increasing amount of clinical evidence indicates that one-stage exchange arthroplasty provides outcomes comparable to those of two-stage exchange arthroplasty in the management of chronic periprosthetic joint infection (PJI) following total knee arthroplasty (TKA). However, for cases complicated by a sinus tract, most specialists still advocate a two-stage exchange arthroplasty strategy. This study aims to evaluate the effectiveness of one-stage exchange arthroplasty in treating chronic PJI following TKA with sinus tract formation and to determine the feasibility of this approach in achieving infection control and restoring joint function. A retrospective analysis was conducted on 23 patients with PJI following TKA accompanied by a sinus tract, who were admitted between January 1, 2019 and December 31, 2023. All patients completed a minimum follow-up of 12 months. During this period, microbiological findings, individualized antimicrobial regimens, and incidences of infection recurrence were meticulously documented. To objectively evaluate infection control, serial measurements of serum inflammatory markers—including the erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), interleukin-6 (IL-6), and procalcitonin (PCT)—were obtained preoperatively and on postoperative days 3, 7, 10, and 14, allowing for assessment of their dynamic changes. Furthermore, to assess functional recovery, the visual analog scale (VAS) score, knee range of motion (ROM), and Knee Society Score (KSS) were compared preoperatively and at postoperative day 14, 3 months, and the final follow-up. The mean follow-up duration was 14.0 ± 0.9 months. At the final follow-up, the infection-free survival rate was 95.65% (22/23) as defined by the International Consensus Meeting (ICM) criteria, with only one case of recurrence at two months postoperatively. All evaluated clinical outcomes—including the VAS score, ROM, and KSS—demonstrated statistically significant improvements compared to preoperative baseline values (P < 0.001). By the final follow-up, patients had achieved a nearly pain-free state. The mean knee ROM significantly increased from 64.8° ± 5.0° preoperatively to 120.0° ± 4.5°, while the mean KSS improved from 33.1 ± 5.3 points to 87.7 ± 5.2 points. For chronic PJI following TKA with a sinus tract, one-stage exchange arthroplasty, when performed in strict adherence to a standardized protocol, demonstrates favorable potential in infection control and significant functional restoration, with satisfactory medium-term outcomes. Therefore, under strictly standardized surgical protocols, one-stage exchange arthroplasty may be considered a viable treatment option for carefully selected cases of periprosthetic joint infection complicated by a sinus tract following thorough and prudent evaluation.
