Comparison of Neuromuscular Blocking Effects and Reversibility of Cisatracurium versus Atracurium During Thyroid Surgery
Objective: To measure the neuromuscular blocking effects and reversibility of Cisatracurium (0.15mg/kg) with Atracurium (0.5mg/kg) during thyroid surgery. Study Design: Quasi-experimental study. Place and Duration of Study: Department of Anesthesia, Combined Military Hospital, Peshawar, Pakistan from Sep 2022 to Apr 2023. Methodology: The present study was conducted on sixty patients undergoing thyroid surgery of age ranges from 20-65 years of both gender belonging to American Society of Anesthesiologists (ASA) I and II were included. All the patients were randomly divided into two equal Groups. Patients in Group-A received Atracurium (0.5 mg/kg) while in Group-B, received Cisatracurium (0.15 mg/kg). Patients were intubated three minutes after administration of Neuromuscular Blocking Agents (NMBAs). The outcomes in terms of time for onset of action, intubation conditions and reversibility were recorded. Results: The mean time for onset of action was 4.67±0.88 minutes in Group-A while 2.37±0.76 minutes in Group-B with p-value of 0.001. In Group-A, the mean reversibility time was 41.80±1.69 minutes while 45.33±1.27 minutes in Group-B with p-value of 0.001. Intubating conditions assessed clinically were excellent in 10(33.33%), good in 09(30.0%) and poor in 11(36.67%) in Group-A whereas, in Group-B, excellent in 13(43.33%), good in 10(33.33%) and poor in 7(23.33%). Conclusion: This study concluded that Cisatracurium (0.15mg/kg) is more effective Neuromuscular Blocking Agent (NMBA) than Atracurium (0.5mg/kg) in patients under general anesthesia.

In the focus of this research are the concept of construction and the architecture of the ergatical interface of a prospective aviation complex using integrated modular avionics, which are based on the methodology of rational coordination of the aggregate properties of human operator interaction, technology and the image forming module of the management goal and application in the interests of increasing the efficiency of crew activity and flight safety. Information display and control systems, which are based on digital electronic devices and integrated information management, with the separation of the time of operations for monitoring and displaying data and providing the most convenient form of information representation, are examined in this article. In such systems, only the information that the pilot needs for a given flight mode is reproduced, or called upon them on demand. Wide opportunities in varying the volume and type of presentation of individual parameters depending on the flight modes require engineering and psychological justification for the principles of rational information support for the pilot in the process of solving professional problems. The results of comparative studies of the principles of mapping the spatial position of aircraft for three types of indication are presented: "view from aircraft to ground" (type 1), "view from ground to plane" (type 2) and "view of aircraft and "leader" from another aircraft located behind at the same altitude and ground sp eed" (type 3), which showed the advantages of the third type of indication — information command-leader display. This type of indication provides the pilot with the geocentric image of the flight relative to the earth’s normal coordinate system, the directions of which are the vector of the ground speed, the gravitational vertical and the horizon line, and the image of the control objective and app lication in the form of reflection of change in attitude and change in the configuration position in accordance with the "do as I do" management principle. The possibilities of using IMA-technology and mnemonic frames with information command-leader indication, which allow to increase the efficiency of the crew of multifunctional aviation complexes, are presented.

Here we reported an interesting case of an 84-year-old woman with acute onset of paresis of left arm and paresthesia of left face and arm. The symptoms resolved within two hours. She also had a similar prior episode two weeks ago with only left arm paresthesia. Her MRI revealed different stages of lacunar ischemic lesions. Interestingly, the SWAN sequences showed lateralized rather than global multiple microhemorrhages over the right MCA and PCA territory, and the sulcal hyperintensity on FLAIR was also seen with no associated susceptibility effect and minimal enhancement, indicating probable cerebral amyloid angiopathy (CAA) based on Boston Criteria. It has been acknowledged that the CAA could manifest with certain localization preference. Cerebral microinfarct and white matter disease in CAA have been more often observed in the posterior circulation territory, however the restricted lateralization reported in our case has not been seen. Since CAA is often diagnosed when the characteristic MRI findings are picked up incidentally, recognizing this as a potential “TIA mimic” will be important for guiding treatment due to its higher risk of bleeding. In summary, this case highlights that the CAA could present as restricted lateralized lesions and occur as transient neurologic deficits, which to our knowledge has not be reported before. Recognition of it as a potential manifestation of CAA will be valuable in the clinical diagnosis process.
