Siete años después de su publicación en Francia, en 1987 se publicó en Brasil la versión en portugués del libro de Christophe Dejours, intitulado “A loucura do trabalho : estudo de psicopatologia do trabalho” (“El desgaste mental en el trabajo : ensayo de psicopatología del trabajo”). Brasil atravesaba entonces un momento de regreso a la democratización, con la valorización de los derechos sociales, incluidos los valores sociales del trabajo. Este escenario hizo con que el libro encontrara un terreno fértil para su acogida. El libro tuvo un gran impacto al proponer una forma de entender la normalidad y la enfermedad en el trabajo desde una perspectiva que no se limitaba a modelos causales simplistas. Asimismo, el papel defendido en el texto sobre los colectivos de trabajo para el desarrollo de la salud y la realización del propio trabajo, alimentó reflexiones que superaron los enfoques solipsistas del problema, sirviendo de soporte a diversas investigaciones e intervenciones en la realidad brasileña.

This chapter investigates the library decoration in Italian Dominican, Franciscan and Augustinian reformed male communities, and shows that, by and large, the decorative schemes used are significantly different from earlier ones, often being more lavish in nature (which in itself is surprising against the background of Observant ideas about simplicity and poverty), with an emphasis on the depiction of friars who exemplified the scholastic achievements of their order. The latter, in turn, seems to be related to a renewed interest, within Observant circles, in order history for the purpose of religious identity formation.

The proposed thesis maintains that the bills or legislative acts that are processed in the Congress of the Republic of Colombia have a greater chance of being approved, in a way that is quite significant, depending on the status position of the parliamentarian who presents the legislative initiative or work as speaker of the same This implies that the members of parliament, senators and representatives to the chamber, are not equal to each other, since there would be congressmen with greater power and prestige than others, according to their status. The content of a project is important, but in many cases it can lose relevance, if it does not have the support represented in the authority that emanates from the power and prestige of the parliamentarian who postulates it. The research identifies which status systems have decisive influence and seeks to understand their origin and the reasons behind their influence. The investigation was carried out with the help of observing parliamentary activity and conducting in-depth interviews with congressmen and personnel from the UTL Legislative Work Units.

Abstract Background The efficiency of the management of an outpatient clinic largely depends on the administration of patient flows and waiting times increase costs and affect clinical quality. In this study, we verify if the visit acceptance times are influenced by demographic or geographical factors in a large cohort of patients referred to a city and suburban private outpatient multidisciplinary clinic. Methods We included all scheduled visits of patients aged from 18 to 75 years who arrived in 2021, 2022 and 2023 in our private outpatient clinics, consisting of 34 medical clinics scattered in Milan metropolitan city and hinterland. The variables collected were age, visit time, check-in time, address of the medical clinic and its distance from the closest underground station, patient typology (new business vs. follow-up patient), and the medical branch of the visit. Outcome is ’punctuality’, defined as visit time minus check-in time (in minutes). Results We considered a sample of 410.808 visits from January 2021 to April 2023. The majority of patients check-in early (84.4%) and we found that the percentage of punctual patients increases linearly with age. Earlier hours in the morning show the worst punctuality pattern as well as blood draws in the analysis of different medical branches. We also observed that patients who already had some activity recorded in our systems show the worst pattern of punctuality. No particular differences emerged considering the geographical location of the clinics. Conclusions Younger patients have worse punctuality than older patients. Moreover, earlier hour slots are the most disadvantaged and the medical specialty has an influence on the arrival habits. This data should be considered for better clinical quality and efficiency.

El objetivo de este artículo de revisión es hacer un breve recorrido sobre los inicios de la enseñanza del idioma inglés y la relevancia que se le ha dado a lo largo de este proceso, a su vez este escrito hace un análisis sobre la dificultades y obstáculos que los docentes deben enfrentar en las instituciones de primaria en zonas rurales de Colombia en cuanto a la enseñanza de esta lengua extranjera, se pretende hacer una reflexión y brindar algunos parámetros que los docentes y el gobierno deberían tener presente para brindar una educación que sea pertinente y significativa a los estudiantes de estas zonas rurales que día a día se enfrentan a condiciones poco favorables para una educación con calidad.

The text begins with the analysis of becoming-animal, a process defined by Deleuze and Guattari in A Thousand Plateaus.The authors challenge the relation that Deleuze and Guattari establish between animality -defined as violence that frees bodies' energetic potential -and politics as that which tames bodies and reduces energies.The authors propose a twofold critique of this opposition.First, they elaborate on Elaine Scarry's research on the metaphorization of war suffering to demonstrate the consequences of violence aestheticization toward which Deleuze and Guattari incline.Second, following Silvia Federici, Frantz Fanon, Carl Schmitt, or Charles Tilly, the authors study the continuity between violence and institutional processes that defines modernity.

Abstract Aims Sacubitril/valsartan have changed the treatment of heart failure with reduced ejection fraction (HFrEF), due to the positive effects morbidity and mortality partly mediated by left ventricular reverse remodelling (LVRR). The aim of this multicentre study was to identify echocardiographic predictors of LVRR after sacubitril/valsartan administration. Methods and results Patients with HFrEF requiring therapy with sacubitril/valsartan from 13 Italian centres were included. Echocardiographic indexes including speckle tracking echocardiography (STE) indexes were used to predict LVRR [defined as LV end-systolic volume reduction and ejection fraction (LVEF) improvement > 10% at follow-up] at 6 months follow-up as the primary endpoint. Changes in symptoms (NYHA class) and neurohormonal activations [N-terminal-pro-brain natriuretic peptide (NTproBNP)] were also evaluated as secondary endpoints. The final population (excluding patients with poor acoustic windows and missing data) consists of 341 patients [mean age: 65 ± 10 years; 18% female, median LVEF 30% (interquartile range: 25–34)]. At 6 months follow-up, 82 (24%) patients showed early complete response (LVRR and LVEF ≥35%), 55 (16%) early incomplete response (LVRR and LVEF <35%), 204 (60%) no response (no LVRR and LVEF <35%). Non-ischaemic etiology, a lower left atrial volume index and a higher global longitudinal strain were all independent predictors of LVRR at multivariable logistic analysis (all P < 0.01). LA strain was the best predictor of positive changes in NYHA class and NT-proBNP (all P < 0.05) (Figure 1). Conclusions STE parameters at baseline could be useful to predict LVRR and clinical response to sacubitril-valsartan, and thus could be used as a guide for treatment in patients with HFrEF.

Abstract Aims This sub-study deriving from a multicentre Italian register (DISCOVER-ARNI) investigated whether sacubitril/valsartan in adjunction of optimal medical therapy (OMT) could reduce the rate of implantable cardioverter-defibrillator(ICD) indications for primary prevention in heart failure with reduced ejection fraction (HFrEF) according to European guidelines indications, and its potential predictors. Methods and results In this observational study, consecutive patients with HFrEF eligible for sacubitril/valsartan from 13 Italian centres were included. Lack of follow-up or speckle tracking data represented exclusion criteria. Demographic, clinical, biochemical and echocardiographic data were collected at baseline and after 6 months of therapy. Of 351 patients, 225 (64%) were ICD carriers and 126 (36%) were not ICD carriers (of whom 13 had not indication) at baseline. After 6 months of sacubitril/valsartan, among 113 non-ICD carriers despite having baseline left ventricular (LV)EF ≤ 35% and New York Heart Association (NYHA) class = II–III, 69(60%) did not show ICD indications; 44(40%) still fulfilled ICD criteria (Figure 1). Age, atrial fibrillation, mitral regurgitation>moderate, left atrial volume index (LAVi), and LV global longitudinal strain (GLS) significantly varied between the groups. With ROC curves, age ≥ 75 years, LAVi ≥ 42 ml/m2 and LV GLS ≥ −8.3% were associated with ICD indications persistence (AUC = 0.65, 0.68, and 0.68, respectively). With univariate and multivariate analysis, age and LV GLS emerged as the only significant predictors of ICD indications at follow-up. Conclusions Sacubitril/valsartan provided early improvement of NYHA class and LVEF, reducing the possible number of implanted ICD for primary prevention in HFrEF. Baseline advanced age and reduced LV GLS were markers of ICD indication despite OMT. Early therapy with sacubitril/valsartan may save infective/haemorrhagic risks and unnecessary costs deriving from ICDs.

Type 2 diabetes mellitus (T2DM) represents a major health issue worldwide, as patients with T2DM show an excess risk of death for cardiovascular causes, twice as high as the general population. Among the many complications of T2DM, heart failure (HF) deserves special consideration as one of the leading causes of morbidity and reduced life expectancy. T2DM has been associated with different phenotypes of HF, including HF with reduced and preserved ejection fraction. Cardiopulmonary exercise testing (CPET) can evaluate the metabolic and ventilatory alterations related to myocardial dysfunction and/or peripheral impairment, representing a unique tool for the clinician to study the whole HF spectrum. While CPET allows for a thorough evaluation of functional capacity, it cannot directly differentiate central and peripheral determinants of effort intolerance. Combining CPET with imaging techniques could provide even higher accuracy and further insights into the progression of the disease since signs of left ventricular systolic and diastolic dysfunction can be detected during exercise, even in asymptomatic diabetic individuals. This review aims to dissect the alterations in cardiopulmonary function characterising patients with T2DM and HF to improve patient risk stratification.

O objetivo deste trabalho consistiu no exame da relacao entre variacoes cambiais e os precos de exportacao brasileira do cafe arabica, relacao definida como o pass-through da taxa de câmbio, tendo como referencia o periodo de janeiro de 2000 a marco de 2019. Para tanto, estimaram-se dois modelos: em dolar e em euro. Os dados foram coletados nos sites do Conselho dos Exportadores de Cafe do Brasil – CECAFE, no Instituto de Economia Aplicada – IPEA e na Intercontinental Exchange – ICE. Neste sentido, fez-se uso dos instrumentais de series temporais, especialmente do Modelo Vetor de Correcao de Erros. Os resultados encontrados forneceram indicacoes de que o grau de pass-through da taxa de câmbio para os precos de exportacao do cafe arabica ocorreu de forma incompleta, com os coeficientes para dolar e euro de, respectivamente, -0,69 e -0,91, representando que depreciacoes da taxa de câmbio nao se traduzem em ganhos significativos de competitividade, uma vez que nao reduzem expressivamente os precos de exportacao.
