Emphasizing on Reissner's Fiber: A Forgotten Pathway for Exploring Consciousness
One of the most remarkable ongoing stories in the history of neuroscience is the nearly universal neglect of what is arguably the most strategically located structure in the central nervous system (CNS), an enigmatic, thread-like aggregation of glycoproteins originating from the center of the brain called Reissner's fiber (RF). It is an anomaly, which, because it lies beyond the grasp of conventional theoretical and technological tools, has been rendered virtually invisible to neuroscientists. New tools, however, are poised to reveal the secrets of this unique, highly conserved structure which runs through the entire length of the fluid-filled cavities and passageways of the CNS. This paper proposes that an enigmatic, thread-like aggregation of glycoproteins originating from the center of the brain called Reissner's fiber (RF), explored with quantum theoretical tools and seen through the lens of its possible recent evolutionary degeneration, can provide a new intellectual and physical pathway toward a deeper understanding of consciousness.

Background:Pitching velocity is one of the most important metrics used to evaluate a baseball pitcher’s effectiveness. The relationship between age and pitching velocity after a lighter ball baseball training program has not been determined.Purpose/Hypothesis:The purpose of this study was to examine the relationship between age and pitching velocity after a lighter ball baseball training program. We hypothesized that pitching velocity would significantly increase in all adolescent age groups after a lighter baseball training program, without a significant difference in magnitude of increase based on age.Study Design:Cohort study; Level of evidence, 2.Methods:Baseball pitchers aged 10 to 17 years who completed a 15-week training program focused on pitching mechanics and velocity improvement were included in this study. Pitchers were split into 3 groups based on age (group 1, 10-12 years; group 2, 13-14 years; group 3, 15-17 years), and each group trained independently. Pitch velocity was assessed at 4 time points (sessions 3, 10, 17, and 25). Mean, maximum, and mean change in pitch velocity between sessions were compared by age group.Results:A total of 32 male baseball pitchers were included in the analysis. Mean/maximum velocity increased in all 3 age groups: 3.4/4.8 mph in group 1, 5.3/5.5 mph in group 2, and 5.3/5.2 mph in group 3. While mean percentage change in pitch velocity increased in all 3 age groups (group 1, 6.5%; group 2, 8.3%; group 3, 7.6%), the magnitude of change was not significantly different among age groups. Program session number had a significant effect on mean and maximum velocity, with higher mean and maximum velocity seen at later sessions in the training program (P = .018). There was no interaction between age and program session within either mean or maximum velocity (P = .316 and .572, respectively).Conclusion:Age had no significant effect on the magnitude of increase in maximum or mean baseball pitch velocity during a velocity and mechanics training program in adolescent males.
Read more2019 American Heart Association Focused Update on Neonatal Resuscitation: An Update to the American Heart Association Guidelines for Cardiopulmonary Resuscitation and Emergency Cardiovascular Care
This 2019 focused update to the American Heart Association neonatal resuscitation guidelines is based on 2 evidence reviews recently completed under the direction of the International Liaison Committee on Resuscitation Neonatal Life Support Task Force. The International Liaison Committee on Resuscitation Expert Systematic Reviewer and content experts performed comprehensive reviews of the scientific literature on the appropriate initial oxygen concentration for use during neonatal resuscitation in 2 groups: term and late-preterm newborns (≥35 weeks of gestation) and preterm newborns (<35 weeks of gestation). This article summarizes those evidence reviews and presents recommendations. The recommendations for neonatal resuscitation are as follows: In term and late-preterm newborns (≥35 weeks of gestation) receiving respiratory support at birth, the initial use of 21% oxygen is reasonable. One hundred percent oxygen should not be used to initiate resuscitation because it is associated with excess mortality. In preterm newborns (<35 weeks of gestation) receiving respiratory support at birth, it may be reasonable to begin with 21% to 30% oxygen and to base subsequent oxygen titration on oxygen saturation targets. These guidelines require no change in the Neonatal Resuscitation Algorithm-2015 Update.

In this chapter, we will consider only the last. This is an exciting topic with products being developed by an unusual consortium of academics, the JDRF, the NIH, the FDA, the Helmsley Foundation, and medical device companies. The first artificial pancreas was approved by the FDA in October of 2016 and was first marketed in June 2017 [1].

The therapy of diabetes was dramatically changed in the late twentieth century with the development of self-monitoring of blood glucose (SMBG). Prior to the advent of this technology, the only glucose tool available to patients was to measure urine glucose. This provided only a poor estimate of blood glucose, often representing the value of several hours earlier, and it was unpopular with patients. Today, in the United States, all people with Type 1 diabetes and most with Type 2 diabetes do some SMBG. In this chapter, we will discuss the history, technology, and clinical usage of SMBG and their integration with computers and mobile devices in the emerging field of tele-health.

Objective:British clinical guidelines recommend statins as first-line lipid-modifying treatment (LMT) for patients at high risk of cardiovascular disease (CVD). We undertook an observational study to assess total cholesterol (TC) and low-density lipoprotein cholesterol (LDL-C) levels in high-risk patients who were treated with atorvastatin monotherapy by UK general practitioners.Methods:This retrospective database study included patients with a prescription for atorvastatin monotherapy between November 30, 2008, and November 30, 2011, with the index date defined as the first atorvastatin prescription during this period. Eligible high-risk patients with evidence of coronary heart disease (CHD), atherosclerotic vascular disease (AVD), diabetes mellitus (DM), or familial hypercholesterolemia (FH) were required to have ≥1 TC and LDL-C measurement between 3 and 12 months after the index date, and continuous enrollment 1 year before and 1 year after the index date. Cholesterol levels were assessed using the National Institute for Health and Care Excellence (NICE) guidelines: TC <4.0 mmol/L or LDL-C <2.0 mmol/L.Results:Of 2999 high-risk patients (60.2% men; mean [SD] age = 67.9 [10.6] years) meeting selection criteria, 23.9% 28.2%, 36.2%, and 11.6% received prescriptions for atorvastatin 10, 20, 40, and 80 mg, respectively (percentages do not sum to 100 because of rounding). Across all doses, the mean (SD) follow-up TC was 4.08 (0.80) mmol/L and LDL-C 2.08 (0.65) mmol/L. A large proportion of patients (88.8%) had TC < 5.0 mmol/L. However, only 45.8% had TC < 4.0 mmol/L, and 46.5% had LDL-C < 2.0 mmol/L. Although a larger proportion of patients with CHD/AVD + DM reached guideline-recommended lipid levels, only 63.7% of such patients had TC < 4.0 or LDL-C < 2.0 mmol/L, which are the current targets for this subgroup as recommended by NICE.Conclusions:Less than half of UK high-CVD-risk patients receiving atorvastatin monotherapy achieved guideline-recommended treatment targets for TC, and less than two-thirds of patients with CHD/AVD + DM had values below TC (4.0 mmol/L) or LDL-C (2.0 mmol/L) targets. More effective lipid-lowering strategies may be warranted to optimize cholesterol lowering and target attainment in high-risk patients. Limitations of this study include its retrospective, observational nature.

El mundo organizacional siempre tendra objetivos que alcanzar y se encontrara en el camino obstaculos que dificultan su consecucion. El primer paso para resolver problemas y establecer objetivos es identificarlos y representarlos, lo cual casi siempre se hace por medio del lenguaje natural, evidenciando la carencia de metodos formales para el planteamiento de sus enunciados. Por ello, en este articulo se proponen estructuras para expresar objetivos y problemas de manera que se facilite su analisis y se puedan establecer relaciones semanticas y de consistencia entre ellos, empleando logica de predicados, ya que este mecanismo permite una traduccion directa desde lenguaje natural hacia una representacion formal de frases para constatar la veracidad o falsedad del enunciado. Abstract: Organizational world will always have goals to reach and obstacles to fight, in order to reach those goals. Identifying and representing goals and problems are previous stages in solving problems and establishing goals. These tasks are commonly made in natural language, and no formal methods are used on them. For this reason, in this paper we propose first-order-predicate-logic structures for expressing goals and problems, so we can easily analyze them and establish semantic relationships and consistency among them, making a direct translation from natural language to a phrasal formal representation. This representation leads us to assess true-or-false value of statements.

Phosphodiesterase type 5 (PDE5) inhibitors have proved to be efficacious, safe, and well tolerated, in clinical trials and practice, for men with erectile dysfunction (ED). However, many patients are not satisfied with treatment and discontinue it prematurely. This review discusses evidence-based strategies that nurse practitioners (NPs) can use to improve diagnosis of ED, optimize patient outcomes, and identify opportunities to detect other potentially serious comorbid conditions. This article was based on a previously published review, which involved a PubMed-MEDLINE search of the clinical literature from January 1, 1998 (year of sildenafil's approval in many markets), through August 30, 2008 (date of search). Strategies to optimize responses to PDE5 therapy are summarized by the mnemonic "EPOCH": Evaluating and educating to ensure realistic expectations of therapy; Prescribing a treatment individualized to the couple's needs and preferences; Optimizing drug dose/regimen and revisiting key educational messages at follow-up visits; Controlling comorbidities via lifestyle counseling, medications, and/or referrals; and Helping patients and their partners to seek other forms of therapy if they have decided not to use a PDE5 inhibitor. The "EPOCH" mnemonic may remind NPs of steps to optimize treatment outcomes with PDE5 inhibitors.
