Transcranial Magnetic Stimulation (TMS) is a technique that is frequently utilized in neuroscience for both therapeutic and research purposes. TMS offers critical medical services like treating major depression and is vital in almost every research facility. Because TMS relies on scalp placement, hair is thought to affect efficacy because it varies the distance to the target site. Further, it is presumed that the hair textures and length that are predominantly seen in minoritized persons might pose significant challenges to collecting high-quality data. Here, we present preliminary data demonstrating that TMS may be influenced by hair, particularly in historically underrepresented minoritized groups. The Sol braiding approach is introduced here as an easy-to-learn, quick-to-implement technique that reduces variability in TMS. Compared across nine participants, it was found that the Sol method significantly increased motor evoked potential (MEP) strength and consistency (p < 0.05). By removing the physical hair barrier that impedes direct coil-to-scalp contact, the Sol approach enhances TMS delivery. The MEP peak amplitude and the MEP area under the curve (AUC) were shown to increase as a result. While preliminary, these data are an important step in addressing diversity in neuroscience. These procedures are explained for non-braiding experts.

This paper uses the embodied, Jewish identities of its three authors, and the experimental methodology of kibbitzing as a form of collective inquiry and self-reflexive praxis in order to demonstrate the limitations of chatbots to produce humorous narratives from an explicitly Jewish epistemology. By contrasting the affordances of large language models (LLMs) and their associated chatbots with the context-based logics of Jewish joke craft and storytelling, this article goes on to demonstrate the risk of cultural erasure that is posed by the positivist, denotative meanings associated with ChatGPT’s attempts at producing jokes for, or about, Jews.

Online-food-delivery (OFD) services among consumers have not only revolutionised the food industry but have also impacted the lifestyle of individuals. The purpose of this study is to empirically explore the factors that affect the use of OFDs, and the influence of continued use of OFDs on consumer's food consumption pattern and health concerns. A mixed-method approach was considered, comprising of qualitative interviews of 12 OFD users and later a quantitative cross-sectional survey of 292 Gen Y OFD users from India. The findings suggest a significant association between emotional values, conditional values, monetary values and health consciousness on intention. Additionally, there is also a notable positive influence of continuance intention on changing-food-consumption-pattern and health issues. A multi-group analysis was also conducted to assess the varying impact of the variables when compared among different groups (place-of-stay, gender, and occupation). This study concludes with the research implications, limitations and future scope.

Excellence in congenital cardiac care requires team resilience, complex decision-making, analysis of errors to adjust practice, and continuous targeted improvement. Medicine has learned from other high reliability organizations, but effective team communication and awareness of improvement opportunities remain challenging. We describe how our Heart Center Performance Rounds (HCPR) meeting addresses these gaps, and provides attention to quality improvement (QI) work, which in turn leads to increased participation and improved clinical outcomes. Our HCPR is a weekly review of one week’s worth of surgical and catheterization cases. Each patient’s course is reviewed against both internal and external benchmarks, such as length of stay (LOS) and markers of clinical progression. Eligibility for multidisciplinary improvement projects is reviewed for each patient. A committee tracks trends in discussion, assessing areas for improvement work and team learning. This committee then organizes methods for follow up and arranges presentation back to the Heart Center (HC) audience. Over the course of 12 months, we tracked LOS data and participation for two multidisciplinary QI projects to assess project participation and outcomes. Projects featured in HCPR included reduction of chest tube duration and awareness of median clinical targets. Patients undergoing specific operations were eligible, but could be excluded at the request of the clinical team. We noted a decrease in exclusion over 12 months, as teams became more aware of eligibility criteria and early results. As participation in these projects improved, both variability and overall length of stay decreased for 2 of 4 studied operations. Updates on outcomes were periodically presented to HC audience. Survey responses and interviews demonstrated increased awareness of projects and their potential for improved care metrics. Our HCPR has promoted knowledge of benchmarks and increased participation in quality improvement projects, leading to improved clinical outcomes. Transparency in data and communication of ongoing work is essential for success. Challenges include dedicated time and resources for spreading and sustaining improvement.

Background: Packed red blood cells (PRBC) are often ordered for pediatric cardiac catheterizations (PCC) with no current appropriate use guidelines, resulting in increased utilization. A quality improvement (QI) project was created to focus on optimizing ordering of PRBC for PCC. The primary aim was to increase adherence to new guidelines to > 97%, with a global aim to decrease patient cost. Methods: Retrospective chart review was performed to collect baseline data. For PDSA cycle 1, we updated our own PRBC ordering guidelines by risk stratifying procedures. The PCC report was updated to include documentation of PRBC ordering or transfusion. PDSA cycle 2 included further refinement of the guidelines, and improved awareness amongst those placing the pre-PCC orders. Process measures included ensuring the need for PRBC stated during every pre-PCC time-out, and consistent documentation in the PCC report. Balancing measures included monitoring the rate of PRBC transfusion within 24 hours post-PCC, and incidence of emergency release of PRBCs. Results: For 3 months of baseline data, 112/403 of CCPs (27.8%) had PRBC ordered, 87.5% (98/112) of which followed the prior protocol, with 13/403 (3.2%) receiving PRBC transfusion during CCP. With our revised guidelines, there could be > 20% reduction in PRBC ordering, with estimated > $20,000 cost savings based on 1 crossmatch ($173) and 1 unit PRBC ($730) prepared. For PDSA cycle 1, 27/81 (33%) of PCC had PRBC ordered, with 92% adherence to the updated guidelines, and 100% documentation compliance. For PDSA cycle 2, 13/47 (27.6%) had PRBC ordered with 100% adherence to ordering guidelines. During both cycles, 3 patients had PRBC transfusions within 24 hours of their procedure for other medical reasons not related to bleeding. No cases required emergency release of PRBC. There was a statistically significant process change from a median of 86% to 100% adherence (p=0.01) since implementation of the updated guidelines. Conclusion: With revised guidelines and improved adherence, hospital resources and charges were decreased in low-risk cardiac catheterization procedures without an increase in adverse events. This model can be utilized in other systems to similarly reduce resource utilization and costs.

In March of 2020, the world came to a screeching halt due to the COVID-19 pandemic. Without any warning, businesses big and small were closed, elementary schools and high schools ordered to shut down, and colleges and universities urged to move all in-person classes to online. This chapter explores the curriculum design for digital delivery and readiness for an emergency pivot.
