Energy, Exergy and Environment Analysis of a Hydroelectric Power Plant
Conducting energy, exergy and sustainability analyses is a well-known method to increase the efficiency of renewable energy resources and detect losses. In this context, a Hydroelectric Power Plant (HEPP) in Bingol-Turkey was examined as a case study. It has been determined that exergy loss in HEPP occur in penstocks, Francis turbines, generators, transformers and switchyards. The efficiency of the HEPP is determined as 90%, the annual average exergy efficiency of the power plant is 91% and the exergy sustainability index of the power plant is 9.3%. The highest exergy loss in the equipment in the HEPP occurred in Francis turbines. It has been demonstrated that examined HEPP is an environmentally friendly, green and sustainable energy facility.

Gelir dağılımındaki adaletsizliğin görece belirgin hale gelmesi, gelir dağılımı konusunun önemini artırmıştır. Gelir eşitsizliğinin çözümüne yönelik olarak geliştirilen birçok politika vardır. Kurumsal kalite, gelir dağılımının adil şekilde dağıtılması noktasında üretilen politikalar içerisinde önemli bir yere sahiptir. Buradan hareketle gelir dağılımı ve kurumsal kalite ile ilgili çalışmaları bibliyometrik göstergeler bakımından analiz edilmesi bu çalışmanın amacını oluşturmaktadır. Web of Science veri tabanında yer alan makaleler, bu çalışmanın veri setini oluşturmaktadır. Yapılan filtrelemeler sonucunda 915 çalışma, R programının bir web arayüzü olan Biblioshiny’da analiz edilmiştir. Yapılan analiz sonucunda 5019 yazarın bu alana katkı sağladığı ve 783 makalenin kaleme alındığı tespit edilmiştir. Bunun yanında gelir dağılımı ve kurumsal kalite alanında %8,98’lik bir büyümenin olduğu sonucuna ulaşılmıştır. Gelir dağılımı ve kurumsal kalitenin yalnızca Web of Science (WoS)’ta yayınlanmış çalışmaların yer aldığı bir analize sahip olması çalışmanın kısıtlılıklarından birisini oluşturmaktadır. Çalışmaların birden çok veri tabanında, daha fazla dil seçeneğine sahip ve alana ait daha fazla anahtar kelimenin dahil edilerek yapılacağı gelecekteki çalışmaların alana sunacağı katkı öngörüsü, çalışmanın sonuç bölümünde sunulmaktadır.

To explore family belonging and communication patterns in individuals with substance use disorder (SUD), highlighting the essential role of psychiatric-mental health nurses (PMHNs) in family-centered care to support recovery. The study included 102 individuals receiving treatment for SUD. Data were collected using the Family Sense of Belonging Scale and Family Communication Patterns Scale. Descriptive and inferential analyses were applied. Participants had above average family belonging and communication, with self belonging higher than family belonging. Conversation orientation was positively linked to family belonging. Lower family belonging correlated with older age, unemployment, suicidal behavior, and violent family environments. Unemployment and suicide attempts were associated with lower conversation orientation. Strengthening family belonging and conversation communication can aid recovery in SUD. PMHNs are key in implementing family-focused interventions to address these factors.

Impaired wound healing in patients with diabetes mellitus is a major clinical problem resulting from chronic hyperglycemia, which disrupts immune function and prolongs inflammation. Cell-based therapies such as mesenchymal stem cells (MSCs) and stromal vascular fraction (SVF) have shown promise in promoting tissue repair, yet comparative data between SVF and adipose-derived stem cells (ADSCs) remain limited. In this experimental study, a diabetic rat model was used to compare the effects of allogeneic SVF and allogeneic ADSCs on wound healing. Wound areas were measured on days 5, 10, and 14. Histological analyses, including assessments of reepithelialization, fibrosis, and epithelial tongue length, as well as cytokine level measurements, were performed to evaluate tissue regeneration and inflammation. Both SVF and ADSC groups demonstrated significantly smaller wound areas compared with controls at all time points (P<0.05). On days 5, 10, and 14, the wound areas were 3.127±0.382, 0.833±0.360, and 0.552±0.185cm2 in the control group; 1.968±0.533, 0.543±0.165, and 0.347±0.144cm2 in the SVF group; and 2.242±0.565, 0.432±0.137, and 0.276±0.131cm2 in the ADSC group, respectively. Histological evaluation revealed enhanced reepithelialization and greater epithelial tongue length in both treatment groups. No significant differences were observed between SVF and ADSC groups. IL-12 levels on day 5 decreased in the SVF group (76.01±13.65ng/L) and increased in the ADSC group (96.50±19.66ng/L) compared with the control group (84.81±15.03ng/L) (P=0.047). Both allogeneic SVF and ADSC therapies significantly accelerated wound healing in diabetic rats. Given its simpler and faster isolation process, lower cost, and comparable efficacy to ADSCs, SVF may represent a practical and effective alternative for enhancing diabetic wound repair.

İntroduction: This study aims to investigate the impact of neoadjuvant chemotherapy (NAC) on the number of lymph nodes dissected during axillary lymph node dissection (ALND) in breast cancer patients, a topic of significant debate among surgeons and pathologists. Material and Methods: We conducted a retrospective analysis of 440 breast cancer patients who underwent surgery between January 1, 2018, and December 31, 2020. Patients were categorized into two groups based on NAC administration. We collected demographic, clinical, and pathological data, focusing on the total and malignant lymph node counts. Results: Among the 440 patients, 124 received NAC, while 316 did not. The mean number of lymph nodes removed was 18.1 in total, with 17.2 in the NAC group and 18.5 in the non-NAC group (P = 0.17). No significant differences were observed in the number of malignant lymph nodes between the two groups (P = 0.73). However, multivariate analysis indicated that NAC significantly influenced the total number of lymph nodes dissected (P = 0.05). Additionally, lymphovascular invasion (LVI) was found to significantly impact the number of malignant lymph nodes. Conclusion: Our findings suggest that NAC is associated with a reduced number of lymph nodes dissected during ALND in breast cancer patients. These results indicate that the standard guideline of removing at least 10 lymph nodes may require reconsideration for patients undergoing NAC. Further studies are needed to clarify the implications of these findings for staging and treatment protocols.

Abstract Aim: Although the irinotecan–bevacizumab (IB) combination is a frequently used treatment option in recurrent brain tumors, clinical benefit shows heterogeneity among patient subgroups. This study aimed to evaluate clinical and prognostic factors associated with early-stage mortality and overall survival (OS) in patients receiving IB treatment. Material and Methods: Seventy-eight brain tumor patients who received IB treatment between 2015 and 2023 were retrospectively evaluated. Patients with complete clinical and radiological data were included in the study. The demographic, clinical, and pathological characteristics of the patients were recorded. The timing of the initiation of IB therapy was categorized according to whether it was started at the first recurrence or at the second and subsequent recurrences. Factors associated with early-stage mortality (≤6 months) were determined using log-linear regression analysis, while independent prognostic factors affecting GSK were determined using the Cox proportional hazards regression model. Results: The mean age of patients was 53.01±12.78 years, and the mean follow-up period was 9.47 months. The mortality rate within the first 6 months was determined to be 44.9%. Early-stage mortality was found to be significantly associated with multilobular tumor localization (p=0.037) and initiation of IB therapy at ≥2nd stage (p=0.036). In the log-linear regression analysis, the probability of survival beyond 6 months was 5.05 times lower (RR=0.198, 95% CI: 0.054-0.735) in multilobular localization, while in those who started IB at ≥2nd node, it was 4.40 times lower (RR=0.227, 95% CI: 0.069-0.748). In the multivariate Cox regression analysis, starting IB at ≥2nd line (HR: 2.35, 95% CI: 1.09-5.07, p=0.029), the presence of IDH mutation (HR: 1.98, 95% CI: 1.09-3.58, p=0.025), and age (HR: 1.03, 95% CI: 1.01-1.05, p=0.015) were identified as independent prognostic factors for GSK. Conclusion: This retrospective cohort study based on real-world data demonstrates that the clinical benefit of the IB combination shows significant heterogeneity across patient subgroups. Multilobar tumor location and initiation of IB therapy in the late stages of recurrence are associated with early mortality and poorer survival. Our findings emphasize that IB therapy in recurrent brain tumors should be administered in a personalized manner, considering patient selection and treatment sequence, rather than as a routine approach.
