AimTo determine the proportion of out-of-hospital cardiac arrest (OHCA) patients, who fulfilled eligibility criteria for extracorporeal cardiopulmonary resuscitation (eCPR) on scene and were managed by a national helicopter emergency medical service (HEMS). Additionally, we compared estimated time intervals from collapse until hypothetical eCPR for the scenarios of on scene and in-hospital eCPR.MethodsThis multicentre, retrospective study included 6687 non-traumatic OHCA patients (01/2010–12/2019). Time intervals from collapse until hypothetical eCPR were calculated using measured, published or estimated data.ResultsECPR eligibility criteria were fulfilled in 890 [13.3 % (95 %CI, 12.5–14.2 %)] of 6687 patients. In these patients, the estimated time from collapse to hypothetical eCPR was shorter in the on scene than the in-hospital eCPR scenario (42.2 ± 5.2 vs. 82.4 ± 6.1 min, p < 0.001). More patients underwent hypothetical eCPR within 35, 40, 45, 50, 55, 60, 65, 70 and 75 min after collapse in the on scene compared to the in-hospital eCPR scenario (p < 0.001 for all comparisons). The proportions of patients, who fulfilled eCPR eligibility criteria but could not undergo hypothetical eCPR within 60 or 75 min, were lower in the on scene compared to the in-hospital eCPR scenario [60 min: 0.9 % (95 %CI, 0.4–1.8 %) vs. 100 % (95 %CI, 99.6–100 %), p < 0.001; 75 min: 0.1 % (95 %CI, 0–0.6 %) vs. 87.4 % (95 %CI, 85.1–89.5 %), p < 0.001].ConclusionIn Austrian HEMS, one in seven OHCA patients fulfilled eCPR eligibility criteria on scene. Our estimates strongly suggest that initiation of eCPR on scene could significantly shorten low-flow times and increase the proportion of eligible OHCA patients, who receive this potentially life-saving intervention.

Objective: Left ventricular dysfunction commonly occurs as a complication after acute myocardial infarction. This study sought to explore the occurrence of acute left ventricular dysfunction among patients who have experienced an acute myocardial infarction (AMI) Methods: A prospective observational study was conducted at a tertiary care hospital over 12 months from February 2022 to January 2023. Non-probability convenience sampling technique was used to recruit the participants following informed consent. All patients aged 18 or more admitted with acute myocardial infarction confirmed by clinical, ECG findings and cardiac biomarkers were included in the study and patients with pre-existing valvular heart disease, previous history of myocardial infarction and history of cardiac surgery were excluded from the study. Socio-demographic details and risk factors like hypertension and diabetes were recorded, alongside presenting symptoms and vital signs. Echocardiography was performed to assess left ventricular function, including ejection fraction and other parameters. Data was analyzed utilizing SPSS version 28. The ejection fraction data were stratified into three distinct categories: 1) between 25% and 40%, 2) between 41% and 55%, and 3) between 55% and 70%. The Chi-square test was employed to assess significant differences in the proportion of patients with different demographic characteristics, co-morbidities and risk factors falling into each category. The level of statistical significance was p <0.05. Results: 100 patients with acute myocardial infarction were recruited. 69% were male and the mean age was 61 years. Most patients (55%) had an ejection fraction (EF) between 25% and 40% and the majority of patients (63%) had acute anterior MI. The coexistence of hypertension with diabetes and smoking was prevalent (74% and 48% respectively), with significant associations (p = 0.000 and 0.02). A substantial difference in EF distribution was observed between anterior and inferior wall MI patients (p = <0.0001), with 71% of anterior MI patients having EF below 40%, compared to 27% of inferior MI patients. Conclusion: This study provides valuable insights into the decrease in ejection fraction observed in patients with acute myocardial infarction, particularly those presenting with anterior wall MI.

QDAの主成分分析図からGroup 1 (G, I: 磯の臭い, 酸味), Group 2 (F, K, H, L: カラメル香, 酸味, 醤油香, みりん様の甘い香り), Group 3 (A, B, C, D, E, J: 燻製様香, コク味, 塩味, 甘味, たくあん臭, 魚臭さ, 焦げ臭, 油焼け臭) に特徴がみられ, この中より6検体 (G, I, H, L, C, D) を消費者試験試料に選択した.

eDrilling is a new and innovative system for real time drilling simulation, 3D visualization and control from a remote drilling expert centre. The concept uses all available real time drilling data (surface and downhole) in combination with real time modelling to monitor and optimize the drilling process. This information is used to visualize the wellbore in 3D in real time. The system is composed of the following elements, some of which are unique and ground-breaking: An advanced and fast Integrated Drilling Simulator which is capable to model the different drilling sub-processes dynamically, and also the interaction between these sub-processes in real time. The Integrated Drilling Simulator is used for automatic forward-looking during drilling, and can be used for what-if evaluations as well.Automatic quality check and corrections of drilling data; making them suitable for processing by computer models.Real time supervision methodology for the drilling process using time based drilling data as well as drilling models / the integrated drilling simulator.Methodology for diagnosis of the drilling state and conditions. This is obtained from comparing model predictions with measured data.Advisory technology for more optimal drilling.A Virtual Wellbore, with advanced visualization of the downhole process. A new generation visualization system designed to integrate all participants involved, will enable enhanced collaboration of all drilling and well activities in a global environment.Data flow and computer infrastructure. The combination of the various elements will make e-Drilling very attractive to the new people coming into the industry; the
