Abstract Aim: This research will be performed with the aim of determining the effect of training and telephone counseling given to hypertension patients on drug medication adherence and rational drug use. Methods: The study protocol will be created using the SPIRIT guide and reporting of the research will be constructed according to the CONSORT checklist. The research will be performed from October 2021 to June 2022 with hypertension patients attending in a state hospital in Turkey. The research sample will comprise 92 hypertension patients, including 46 subjects and 46 controls. In the research, patients will be randomly assigned to subject and control groups with the block randomization method. This assignment process will be performed by a researcher not included in the implementation and group assignments will be announced in order during randomization by using opaque envelopes. The posttest in the research will be implemented by a nurse with undergraduate degree who is not one of the researchers. Research data will be collected with the ‘Patient Description Form’, ‘Drug Medication Adherence Self-Efficacy Scale Short Form’ and ‘Rational Drug Use Scale’. Hypertension patients in the experiment group will undergo ‘Drug Use and Rational Drug Use Training for Hypertension Patients’. Additionally, patients in the experiment group will be given telephone counseling lasting mean 10-15 minutes a total of 4 times; 2 times during the 1st month (2nd and 4th week), 1 time during the 2nd month (8th week) and 1 time during the 3rd month (12th week). Control group patients will receive routine hospital care. Results: The research is planned as a single-blind, randomized, controlled experimental study. Conclusion: This protocol will assess the effect of training and telephone counseling given to hypertension patients on their adherence to drug medication and rational drug use.

Purpose: The aim of this study was to assess whether orthopedic surgeons’ graduation centers affect their academic career.Materials and Methods: In this observational study, we have reached the education and working information of 1767 orthopedic surgeons currently working in Turkey. Orthopedic surgeons’ graduation center and city, current affiliation, current academic status and duration of practice were investigated through the web. According to their graduation center, orthopedic surgeons were divided into two groups as a university hospital and training and research hospital. Results: Orthopaedic surgeons’ current affiliations were significantly associated with their graduation centres and cities. Besides, orthopaedic surgeons graduated from university hospitals were more likely to have an academic career than those graduated from training and research hospitals; however, no significant association was found between the city of graduation and academic career. According to regression analysis, orthopedic surgeons graduated from university hospitals were more likely to affiliate in university hospitals and private hospitals. Conclusion: Orthopedic surgeons graduated from university hospitals were significantly more likely to affiliate in university hospitals and to have an academic career in Turkey.

The purpose of this study was to determine the factors affecting survival in patients who underwent pancreaticoduodenectomy for periampullary cancers and to discuss the outcomes of our findings. This retrospective study included 79 patients who underwent pancreaticoduodenectomy for periampullary cancers between September 1987 and October 2011 in the Department of General Surgery at Ondokuz Mayis University School of Medicine. The factors of age, tumor localization, tumor size, lymphovascular invasion, status of lymph node metastasis, tumor differentiation, preoperative CA 19-9 levels, preoperative total bilirubin levels, preoperative albumin levels, and preoperative biliary drainage were investigated to determine their influence on survival. The survival periods were calculated using the Kaplan-Meier method. The log-rank test was used for comparison of the prognostic factors. The independent prognostic factors affecting survival were determined by Cox hazard regression analysis and hazard ratios (HR) and 95% confidence intervals (CI) were calculated. P<0.05 was considered as significant. The following were identified as independent prognostic factors adversely affecting survival: tumor size ≥2 cm (HR: 2.0, 95% CI: 0.27-0.90), lymphovascular invasion (HR: 2.9, 95% CI: 0.18-0.60), CA 19-9 levels ≥100 U/mL (HR: 2.0, 95% CI: 0.26-0.90), and albumin levels <2.5 mg/dL (HR: 2.7, 95% CI: 1.14-6.66). The independent prognostic factors identified in this study can be used for selection of patients for whom pancreaticoduodenectomy should be applied for periampullary cancers. These factors could help us to estimate survival rates.

To avoid performing axillary lymph node dissection (ALND) for non-sentinel lymph node (SLN)-negative patients with-SLN positive axilla, nomograms for predicting the status have been developed in many centers. We created a new nomogram predicting non-SLN metastasis in SLN-positive patients with invasive breast cancer and evaluated 14 existing breast cancer models in our patient group. Two hundred and thirty seven invasive breast cancer patients with SLN metastases who underwent ALND were included in the study. Based on independent predictive factors for non-SLN metastasis identified by logistic regression analysis, we developed a new nomogram. Receiver operating characteristics (ROC) curves for the models were created and the areas under the curves (AUC) were computed. In a multivariate analysis, tumor size, presence of lymphovascular invasion, extranodal extension of SLN, large size of metastatic SLN, the number of negative SLNs, and multifocality were found to be independent predictive factors for non-SLN metastasis. The AUC was found to be 0.87, and calibration was good for the present Ondokuz Mayis nomogram. Among the 14 validated models, the MSKCC, Stanford, Turkish, MD Anderson, MOU (Masaryk), Ljubljana, and DEU models yielded excellent AUC values of > 0.80. We present a new model to predict the likelihood of non-SLN metastasis. Each clinic should determine and use the most suitable nomogram or should create their own nomograms for the prediction of non- SLN metastasis.

The purpose of this study is to study lipid metabolism in oocytes and embryos that is a neglected parameter in human IVF. We have tested the total carnitine content (TC) in the follicular fluid of 278 patients (217 non pregnant, 61 pregnant) undergoing IVF. The follicular fluid TC is neither correlated with the circulating estradiol content in serum nor with the outcome the IVF attempt. Carnitine, through the carnitine shuttle, is a major partner in lipid beta oxidation, metabolic pathway involved in the acquisition of oocyte competence. The expression of carnitine synthesis enzymes and lipid beta oxidation was studied in cumulus cells collected at the time of ovum retrieval and in oocyte. Surprisingly the expression for carnitine synthesis is not detectable in oocytes whereas the enzymes involved in lipid beta oxidation are rather strongly expressed. The addition of carnitine in oocyte maturation and embryo culture media should not be overlooked.
