Migräne und Hormonbehandlung
Eine Hormontherapie erhöht bekanntermaßen das Thrombembolierisiko. Migräne mit Aura erhöht bei Frauen unter 45 Jahren das Risiko für einen Schlaganfall um ca. das Doppelte. Zwei Fakten, die deutlich machen, dass bei Frauen mit Migräne, bei denen eine Hormontherapie indiziert ist, einiges zu beachten ist. Dieser Beitrag informiert u. a. über Vor- und Nachteile einer Hormongabe bei Migränepatientinnen in verschiedenen Lebensphasen.

The author investigates bodily aspects of the defense organization in the treatment of a soldier suffering from a war traumatization. The patient reports two situations—a bomb attack and the subsequent confrontation with wounded comrades—that had a traumatizing impact. In the treatment process, a phase of stagnation is described before the shared attention is focused on the bodily perception of the patient. His petrified body feeling (“my body feels like concrete”) was systematically examined in the therapeutic process then slowly transformed through shared perception, leading to a process of vitalizing reorganization. This method is called somatic narration. It reverses the defense processes of the encapsulated body engram. This capsule results from the threatening impact of a traumatic event, disorganizing the patient’s body-self. This disorganization then is encapsulated through a process of petrification and avoidance of awareness. The therapeutic process is described in detail. The structure of the bodily unconscious is revealed. The process of reorganization of perception and memory is outlined.

Unrepresented states are considered important obstacles to the psychoanalytic process. They describe elements that are beyond the reach of the symbolic network with which psychoanalysis is used to working. The emergence of unrepresented states has often been described as the failure of the caregiver to symbolize the child's emotions and thereby enable the child to connect his or her bodily states to the psychic representation. Psychoanalysis, however, has been reluctant to name the locus of these inscriptions beyond the symbolic network as the body-self. The author proposes to do so and discusses two concepts for describing the dynamics of the bodily unconscious and the therapeutic method for calibrating our technique to unrepresented states. The concept of the encapsulated body engram is used to describe the dynamic structure of the bodily unconscious. Processes of disorganization, petrification, perceptual defense, and secondary self-stimulation form the dynamics of the bodily unconscious. The method of somatic narration systematically examines body sensations of the analysand, reverses the defense processes of the engram, and leads to a reorganization of the body self, which can now find connection to symbolic structures again. This requires a more active analytic stance that responds to the defensive processes with which the subject fends off the threat of annihilation he or she was exposed to in the traumatic engram. A clinical vignette illustrates the mode of operation.

Die Migränepersönlichkeit
ZUSAMMENFASSUNGDie Suche nach einer Migränepersönlichkeit ist in der Medizingeschichte nicht neu. Es gab verschiedene Ansätze, alle Menschen mit Migräne einem bestimmten Persönlichkeitstypus zuzuordnen. Dies ist aus heutiger Sicht gescheitert, obwohl es Verhaltensmerkmale und kognitive Besonderheiten gibt, die bei der Mehrzahl der Menschen mit Migräne auftreten. Dazu gehören ein unspezifisch erhöhter Neurotizismus-Score (am ehesten durch die Schmerzen bedingt) und eine mangelnde Habituation auf kognitive Reize zwischen den Migräneattacken. Ein weiterer Aspekt der „Migränepersönlichkeit“ ist die Komorbidität mit einigen psychiatrischen Erkrankungen wie Depression und Angsterkrankungen. Hierbei handelt es sich möglicherweise um gemeinsame pathophysiologische Grundlagen, die aber nicht in ein konsistentes psychiatrisches Krankheitsprofil bei Menschen mit Migräne münden. Weiterhin muss bei den Auswirkungen der Migräne auf die Betroffenen auch die deutlich beeinträchtigte Lebensqualität berücksichtigt werden, die inzwischen mit krankheitsspezifischen Instrumenten erfasst wird. Abschließend gibt dieser Artikel auch Hinweise, wie die o. g. spezifischen Aspekte der Migränepatienten im ärztlichen Gespräch berücksichtigt werden können.

A cryptic invader of the genus Persicaria has recently increased in the damper, northern parts of La Palma and Gran Canaria in the Canary Islands (Spain) and locally behaves as an invasive species. Examination of historical herbarium specimens showed this species to be present in Gran Canaria since the 1960s and the same probably applies to La Palma. Up to now, this species had been assigned to the Old World weed P. maculosa. However, morphologically, these plants clearly correspond with P. hydropiperoides, a common and widespread weed native to the New World, and indeed morphologically similar to P. maculosa. Diagnostic features for these two species, as well as for another similar species (P. decipiens, originally described from the Canary Islands), are compared, thoroughly discussed, and copiously illustrated. The current distribution, ecology, and naturalization status of P. hydropiperoides in the Canary Islands are also assessed. The variability of P. hydropiperoides is discussed, more precisely the taxonomic position of a southern ‘race’ of it that is sometimes referred to as a distinct species, P. persicarioides, and to which the Canarian plants belong. The taxonomic value of the latter appears to be clear, although at a lower level. A new combination, at varietal rank, is proposed and validated. In addition to our morphology-based study, a molecular phylogenetic analysis has been conducted on the nuclear ITS region and the plastid DNA region trnL-F.

Daran anknüpfend werden offene Untersuchungsfelder beschrieben, die sich aus dem zunehmenden Einsatz von robotergestützten Systemen, die klangliche Umgebungen nutzen, ergeben.
