Purpose: Male hypogonadism, characterized by decreased functionality of the testicles and evidenced by low testosterone levels, requires treatment strategies tailored to the specific type of hypogonadism. Primary hypogonadism, resulting from direct testicular impairment, is typically addressed with testosterone replacement therapy (TRT). In contrast, secondary hypogonadism, stemming from dysfunction in the hypothalamic-pituitary axis, often necessitates alternative treatments aimed at preserving fertility markers. These include Selective Estrogen Receptor Modulators (SERMs), Aromatase Inhibitors, and human chorionic gonadotropin (hCG). Although all treatment options can be effective, TRT uniquely offers dose-dependent benefits, which are not as achievable with alternatives due to their inability to bypass the body's natural testosterone production controls. However, the choice of TRT, particularly for individuals with secondary hypogonadism, involves a trade-off between maximizing treatment efficacy and preserving fertility markers. Our study addresses this dilemma by exploring a novel therapeutic approach, dubbed 'Oral TRT Plus,' which combines two oral medications: oral native testosterone and enclomiphene citrate. This approach aims to provide the dose-dependent benefits of exogenous testosterone while also preserving fertility markers.Materials and Methods: 79 male patients, encompassing both hypogonadal and eugonadal men, participated in a sequence of four distinct experiments. These experiments were designed to determine the ideal dosing, assess the necessity of fat intake for enhanced drug absorption, and identify the most effective timing for blood tests to capture peak testosterone levels. Measurements of various blood markers, including total testosterone, free testosterone, luteinizing hormone (LH), follicle-stimulating hormone (FSH), estradiol (E2), and sex hormone-binding globulin (SHBG), were obtained using both at-home lab testing kits and in-person venipuncture. In addition, participants completed subjective questionnaires—the quantitative Androgen Deficiency in the Aging Male (qADAM) and the Patient Health Questionnaire-4 (PHQ-4)—to evaluate subjective improvements in hypogonadal symptoms during the treatment. Results: With optimized dosing, total testosterone levels in both eugonadal and hypogonadal men increased fourfold and free testosterone fivefold (both p < 0.025), surpassing the 99th percentile for both measurements in all participants. Meanwhile, LH and FSH levels remained within normal ranges across all participants (p < 0.025), demonstrating the treatment's ability to preserve fertility markers irrespective of the increased testosterone dosing. Estrogen levels significantly decreased but stayed within normal limits. Subjective health measures improved notably; over 70% of participants reported enhanced energy, strength/endurance, improved work performance, and quality of life according to qADAM scores, with significant reductions in anxiety and depression symptoms evidenced by 97% of participants reporting normal function (both p<0.001). Conclusions: Both eugonadal and hypogonadal men utilizing the “Oral TRT Plus Protocol” demonstrated a fivefold increase in free testosterone levels on average, while simultaneously mitigating endogenous suppression and preserving fertility markers. This protocol elicited significant improvements across various subjective health metrics, including quality of life, energy, strength, mood, libido, and work performance, alongside a marked decrease in anxiety and depression symptoms. Furthermore, the protocol effectively regulated estrogen levels within normal physiological ranges, eliminating the necessity for adjunctive medications. The “Oral TRT Plus Protocol” aligns with the evolving landscape of telemedicine by demonstrating how advanced hormonal treatment regimens can be effectively administered and monitored remotely, offering an innovative, non-invasive strategy for enhancing men's mental, physical, and hormonal health while preserving fertility markers.

The wildland firefighter exposure and health effect (WFFEHE) study was a 2-year repeated-measures study to investigate occupational exposures and acute and subacute health effects among wildland firefighters. This manuscript describes the study rationale, design, methods, limitations, challenges, and lessons learned. The WFFEHE cohort included fire personnel ages 18-57 from six federal wildland firefighting crews in Colorado and Idaho during the 2018 and 2019 fire seasons. All wildland firefighters employed by the recruited crews were invited to participate in the study at preseason and postseason study intervals. In 2019, one of the crews also participated in a 3-day midseason study interval where workplace exposures and pre/postshift measurements were collected while at a wildland fire incident. Study components assessed cardiovascular health, pulmonary function and inflammation, kidney function, workplace exposures, and noise-induced hearing loss. Measurements included self-reported risk factors and symptoms collected through questionnaires; serum and urine biomarkers of exposure, effect, and inflammation; pulmonary function; platelet function and arterial stiffness; and audiometric testing. Throughout the study, 154 wildland firefighters participated in at least one study interval, while 144 participated in two or more study interval. This study was completed by the Centers for Disease Control and Prevention's National Institute for Occupational Safety and Health through a collaborative effort with the U.S. Department of Agriculture Forest Service, Department of the Interior National Park Service, and Skidmore College. Conducting research in the wildfire environment came with many challenges including collecting study data with study participants with changing work schedules and conducting study protocols safely and operating laboratory equipment in remote field locations. Forthcoming WFFEHE study results will contribute to the scientific evidence regarding occupational risk factors and exposures that can impact wildland firefighter health over a season and across two wildland fire seasons. This research is anticipated to lead to the development of preventive measures and policies aimed at reducing risk for wildland firefighters and aid in identifying future research needs for the wildland fire community.

Over a three-year period, the National Oceanic and Atmospheric Administration organized and implemented a Pacific-wide field campaign entitled CAPSTONE: Campaign to Address Pacific monument Science, Technology, and Ocean NEeds. Under the auspices of CAPSTONE, NOAA mapped 597,230 km 2 of the Pacific seafloor (with ~ 61% of mapped area located within US waters), including 323 seamounts, conducted 187 ROV dives totaling 189 hours of ROV benthic imaging time, and documented more than 347,000 individual organisms. This comprehensive effort yielded dramatic insight into differences in biodiversity across depths, regions, and features, at multiple taxonomic scales. For all deep sea taxonomic groups large enough to be visualized with the ROV, we found that fewer than 20% of the species were able to be identified. The most abundant and highest diversity taxa across the dataset were from one class and two phyla (Anthozoa, Porifera and Echinodermata). We further examined these phyla for taxonomic assemblage patterns by depth, geographic region, and geologic feature. Within each taxon, there were multiple genera with specific distribution and abundance by depth, region, and feature. Additionally, we observed multiple genera with broad abundance and distribution, which may focus future ecological research efforts. Novel taxa, records, and behaviors were observed, which are suggestive of new species interactions, drivers of community composition, and overall diversity patterns. To date, only 13.8% of the Pacific has been mapped using modern methods. Despite the many new contributions and insights to the Pacific deep-sea, CAPSTONE is far from the culminating experience the name suggests. Rather, it marks the beginning of a new era for exploration that will offer extensive opportunities via mapping, technology, analysis, and insights.

Seismic companies will remain competitive only if they reduce costs and at the same time increase the technological capabilities of their products. Overcapacity and lower prices plague geophysical films. Several key improvements in offshore seismic data collection were made during 1985, e.g., wide-tow seismic source arrays that improve the signal-to-noise ratio of the data by reducing various unwanted noise compounds have become standard and seismic source levels have also been increased by a wider variety of air and water gun offering. These improvement in data collection are discussed.

On occasion, reliability analysts have test data gathered both at the component level of a multicomponent system and at the system level. When the system test data provide no information on component performance, classical statistical techniques in all but trivial cases do not allow using both sets of data. We present a Bayesian procedure that does allow using both sets. The procedure for attribute data makes use of a lemma that relates the moments of the prior and posterior distributions of reliability to the test data. The procedure for variables data assumes the time to failure distribution of each component is exponential.
