Experiences of home-care workers with the 'Stay Active at Home' programme targeting reablement of community-living older adults: An exploratory study.
To face the challenges of an ageing population, many Western countries nowadays stimulate an ageing in place policy to empower older adults to grow old in their own homes with the highest degree of self‐reliance. However, many community‐living older adults experience limitations in (instrumental) activities of daily living ((I)ADLs), which may result in a need for home‐care services. Unfortunately, home‐care workers often provide support by taking over tasks, as they are used to doing things for older adults rather than with them, which undermines their possibilities to maintain their self‐care capabilities. In contrast, reablement focuses on capabilities and opportunities of older adults, rather than on disease and dependency. Consequently, older adults are stimulated to be as active as possible during daily and physical activities. The 'Stay Active at Home' programme was designed to train home‐care workers to apply reablement in practice. To explore the experiences of home‐care workers with this programme an exploratory study was conducting in the Netherlands, between April and July, 2017. In total, 20 participants were interviewed: nine nurses (including a district nurse), 10 domestic support workers and the manager of the domestic support workers. The semi‐structured interviews focused on the experienced improvements with regard to knowledge, skills, self‐efficacy and social support. Furthermore, the most and least appreciated programme components were identified. The study has shown that home‐care workers perceived the programme as useful to apply reablement. However, they also need more support with mastering particular skills and dealing with challenging situations. Future implementation of the 'Stay Active at Home' programme can potentially benefit from small adaptions. Furthermore, future research is needed to examine whether the programme leads to more (cost‐) effective home care.

Objetivo. Establecer el impacto y la eficacia de la promoción de servicios de salud en un sector rural del Ecuador mediante el uso de la red social Facebook.
 Método. Se utilizó la herramienta pautaje de publicidad en la red social Facebook para realizar una campaña de promoción en salud dirigida a mujeres entre 20 y 40 años en periodo de gestación. Las mujeres interesadas en participar en el estudio acudieron a la unidad de salud y llenaron una encuesta previa a la consulta con el médico ginecólogo, en la cual se indagó como obtuvieron conocimiento para acceder a la campaña. Los resultados del pautaje fueron compilados por la herramienta anuncios y promociones de la red social y los datos obtenidos de la encuesta se procesaron en hoja de cálculo Excel.
 Resultados. La pauta publicitaria realizada alcanzó en 10 días a 13.504 usuarias. Del total alcanzado 463 usuarias demostraron interés en la publicación, 367 revisaron los términos y condiciones de la promoción y la información específica del estudio y su procedimiento. La campaña alcanzó al 4,36% del publico definido y la taza de efectividad de promoción de producto (clics en el enlace 367) se situó en el 2,72%. El total de mujeres que acudieron a la consulta médica fue de 41, de las cuales solo el 13,8% fueron contactadas mediante la campaña publicitaria. El 86,2% de mujeres declararon acudir por información indirecta de mujeres que vieron la campaña.
 Este primer resultado muestra que la inversión económica y el presupuesto destinado alcanzó al 4,36% del publico definido y la taza de efectividad de promoción de producto (clics en el enlace 367) se situó en el 2,72%.
 Conclusiones. El uso de las redes sociales como Facebook para la promoción de salud tiene un costo mucho más bajo que utilizar los medios convencionales a través de promotores o técnicos sanitarios. El limitante encontrado, fue la poca efectividad y alcance de las redes sociales en zonas donde la oralidad como sistema de comunicación comunitaria sigue siendo preponderante.

Short-term effects of an educational intervention on physical restraint use: a cluster randomized trial
BackgroundPhysical restraints are still frequently used in nursing home residents despite growing evidence for the ineffectiveness and negative consequences of these methods. Therefore, reduction in the use of physical restraints in psycho-geriatric nursing home residents is very important. The aim of this study was to investigate the short-term effects of an educational intervention on the use of physical restraints in psycho-geriatric nursing home residents.MethodsA cluster randomized trial was applied to 5 psycho-geriatric nursing home wards (n = 167 residents with dementia). The wards were assigned at random to either educational intervention (3 wards) or control status (2 wards). The restraint status was observed and residents' characteristics, such as cognitive status, were determined by using the Minimum Data Set (MDS) at baseline and 1 month after intervention.ResultsRestraint use did not change significantly over time in the experimental group (55%–56%), compared to a significant increased use (P < 0.05) in the control group (56%–70%). The mean restraint intensity and mean multiple restraint use in residents increased in the control group but no changes were shown in the experimental group. Logistic regression analysis showed that residents in the control group were more likely to experience increased restraint use than residents in the experimental group.ConclusionAn educational programme for nurses combined with consultation with a nurse specialist did not decrease the use of physical restraints in psycho-geriatric nursing home residents in the short term. However, the residents in the control group experienced more restraint use during the study period compared to the residents in the experimental group. Whether the intervention will reduce restraint use in the long term could not be inferred from these results. Further research is necessary to gain insight into the long-term effects of this educational intervention.
