In patients with mild persistent asthma, do inhaled corticosteroids increase the risk of pneumonia?

EVIDENCE-BASED ANSWER Use of inhaled corticosteroids (ICS), even high-dose ICS, does not increase the risk of pneumonia in patients with asthma, and may provide some protection (SOR: A, metaanalysis of RCTs and retrospective analysis of RCTs). Additionally, in patients hospitalized with communityacquired pneumonia, previous ICS use is not associated with any differences in inflammatory markers or time to resolution of pneumonia (SOR: C, small retrospective cohort study).
