Objective: Tampa Bay Area is an estuary and thus has varying levels of salinity, so it has become imperative to determine whether or not the leading combat strategy (clay flocculation) to HABs is effective under these differing conditions. Method: an experiment was conducted that measured the settling rates of Kaolin clay at varying levels of salinity (0, 17.5, 30, and 45 Parts Per Thousand) within the range of Karenia Brevis. Results: There is no statistically significant difference between the settling rates of Kaolin clay within the range of tolerance of K. Brevis, and therefore can be used to combat K. Brevis blooms in Tampa Bay regardless of salinity.

Billing and insurance-related costs are a significant source of wasteful health care spending in Organization for Economic Cooperation and Development nations, but these administrative burdens vary across national systems. We executed a microlevel accounting of these costs in different national settings at six provider locations in five nations (Australia, Canada, Germany, the Netherlands, and Singapore) that supplements our prior study measuring the costs in the US. We found that billing and insurance-related costs for inpatient bills range from a low of $6 in Canada to a high of $215 in the US for an inpatient surgical bill (purchasing power parity adjusted). We created a taxonomy of billing and insurance-related activities (eligibility, coding, submission, and rework) that was applied to data from the six sites and allows cross-national comparisons. Higher costs in the US and Australia are attributed to high coding costs. Much of the savings achieved in some nations is attributable to assigning tasks to people in lower-skill job categories, although most of the savings are due to more efficient billing and insurance-related processes. Some nations also reduce these costs by offering financial counseling to patients before treatment. Our microlevel approach can identify specific cost drivers and reveal national billing features that reduce coding costs. It illustrates a valuable pathway for future research in understanding and mitigating administrative costs in health care.

The purpose of this study was to evaluate and compare the densities of four different brands of root canal sealers using three different intraoral imaging receptors. Four different root canal sealers used were – AH Plus (Dentsply Germany, Resin based), Epiphany (Pentron USA, Resin based), U/P (Sultan Healthcare, Zinc oxide eugenol based) and Apexit (Ivoclar / Vivadent, Calcium hydroxide based). The sealers were mixed according to manufacturer instructions and six specimens of each material were fabricated. All the specimens were imaged using three different intraoral #2 sized imaging receptors-D and E speed film and storage phosphor plates. D and E speed films were digitized and stored in JPEG format. All the images were exported into the Image J software (rsb.info.nih.gov/ij/) and the mean grey values and integrated density of each material was calculated from four random areas of each image and averaged. Mixed model ANOVA was performed. Pair wise comparison of mean grey values between the three imaging receptors and integral densities recorded by the receptors showed high statistical significance for all the four different root canal sealers. Results showed that the four different types of root canal sealers showed different optical densities on all the three receptors. The purpose of this study is to evaluate and compare the optical densities of four different brands of root canal sealers using three different intraoral imaging receptors.

Employment opportunities in India are not proportionate to the vast population. Young Indians are spending more time and money accumulating several educational degrees in different streams to find the best fit in the job market. Finding a job that matches educational qualification and a pay scale that is compatible with ability, is still a distant dream for many. For people with disabilities (PWDs), job opportunities in India are minimal due to scarce employment support. PWDs often have lower educational achievement as they may suffer from cognitive impairments, poor health, or both. Additionally, perceived low productivity is a factor that may pose a barrier in hiring PWDs. Such societal discrimination has put the PWDs on the margins by depriving them of job opportunities, leading them to poverty, among other psychosocial and emotional issues. This article highlights the employment opportunities, challenges, and stress of PWDs at the work-places in India.

Inclusive education means educating students with special needs in an age-appropriate general education setting where students receive high-quality, standard-based instructions, interventions, and support that enable them to experience success in the general education classroom. Effective pedagogical practices involve the collaboration of both special education teacher and general education teacher to identify and implement effective teaching practices which enhance student engagement and promote higher-order thinking skills. Special education teacher supports the general education teacher in adapting curriculum and teaching methods, modifying assessments, and providing accommodations to students with disabilities in inclusive classrooms. Effective pedagogical practices are affected by teacher's belief; students' disabilities; and their roles and responsibilities towards the students with disabilities. This chapter examines previous researches and studies that investigate effective pedagogical practices in inclusive classrooms for students with disabilities.

Introduction: Although the role of successful reperfusion in improved outcome after endovascular treatment (EVT) for stroke is known, there has been limited evaluation of the effects of poor reperfusion. We compared results in the HERMES collaboration between standard care and those in the EVT group with poor reperfusion. Methods: Patient-level data were pooled in this meta-analysis of seven randomized trials comparing endovascular thrombectomy with standard care in anterior ischemic stroke. Functional outcome was assessed by ordinal analysis of the modified Rankin scale (mRS) at 90 days, comparing standard care versus EVT subjects achieving mTICI of 0 or 1 post-procedure. Analyses were adjusted for baseline prognostic variables to correct for potential imbalances. Results: The meta-analysis included 877 subjects in the standard care group and 78 in the EVT group with mTICI 0-1 per the HERMES central imaging core laboratory. At baseline, the EVT cohort had higher NIHSS (median 19 vs 17, p=0.011), but was not significantly different from standard care in other characteristics including age, sex, ASPECTS, time to randomization, site of occlusion and alteplase administration. Subjects with poor reperfusion in the EVT group had worse mRS at 90 days than standard care, unadjusted (p=0.003) and after adjustment for baseline characteristics (common odds ratio 0.59 (0.38-0.91), p=0.016). Fewer subjects in the EVT poor-reperfusion cohort achieved mRS 0, mRS 0-1 and all other dichotomized mRS cutpoints (Figure 1). Symptomatic intracranial hemorrhage was not different between groups (3.9% vs 3.5%, p=0.75). Conclusion: In recent endovascular trials, poor reperfusion after EVT was associated with worse outcomes than standard care in recent endovascular trials. This suggests that additional efforts to achieve reperfusion by EVT should be encouraged if deemed safe to do so.

Introduction: The benefit that endovascular thrombectomy (EVT) offers to stroke patients with large vessel occlusions depends strongly on reperfusion grade as defined by the mTICI (modified Thrombolysis in Cerebral Infarction) scale. Our aim was to estimate the public health potential of improved reperfusion. Methods: A Markov model estimated lifetime quality-adjusted life years (QALY) of EVT-treated patients and associated costs based on mTICI grades. The analysis was performed from a United States healthcare perspective. Input parameters were based on best available evidence, including patient data from the HERMES collaboration. The lead analysis was conducted for stroke onset at 65 years. Overall lifetime costs and the net monetary benefit (NMB), which combines weighted QALYs and costs into one composite outcome, were analyzed. A willingness-to-pay threshold of $100,000 per QALY was used for NMB calculations. Results: Lifetime QALYs and the NMB per patient increased for every grade of improved mTICI reperfusion (Fig 1). The final mTICI 2C/3 rate across all trials in the HERMES collaboration was 31%, yielding on average 5.09 QALYs at lifetime costs of $230,799, resulting in a positive NMB of $278,336. Every 10% increase in the final mTICI 2C/3 rate would yield 0.16 incremental QALYs and $16,878 incremental NMB for the average patient. For a national 10% improvement of the mTICI 2C/3 rate of all EVT-treated patients in the United States, we estimated 3,645 additional QALYs and an incremental NMB of $385 million per year. Conclusions: The public health and economic impact of the grade of reperfusion is significant, warranting further improvement of devices and procedural techniques.

Background: Pregnancy is associated with increased stroke risk in young women. Idiopathic moyamoya syndrome (IMMS) is a rare condition characterized by progressive narrowing of large cerebral arteries resulting in flimsy collaterals prone to rupture or thrombosis. Data are limited on pregnancy outcomes in women with IMMS. We hypothesized that IMMS would be associated with increased pregnancy morbidity, including stroke. Methods: Using the New York State Department of Health Statewide Planning and Research Cooperative System data from 2000-2014 we identified all women aged 18 and older with diagnoses of IMMS (ICD-9 437.5) who had hospitalizations for delivery at any time either prior, concomitant or subsequent to IMMS diagnosis. We considered a pregnancy to be IMMS-exposed if it occurred ≤ 3 years prior to, or any time after, IMMS diagnosis. We defined pregnancy morbidity as admission within 1 year of delivery for any of the Center for Disease Control and Prevention’s severe maternal morbidity indicators, including stroke. We compared the morbidity of IMMS-exposed pregnancies to unexposed pregnancies occurring in the same cohort &gt;3 years prior to IMMS diagnosis. We used conditional logistic regression to calculate odds ratio (OR) and 95% confidence intervals (95%CI), adjusting for women with both exposed and unexposed pregnancies. Results: We identified 134 patients with IMMS, with 117 exposed pregnancies and 147 unexposed pregnancies. Severe maternal morbidity was higher for exposed pregnancies (33.3% vs 19.0%, OR: 2.1, 95%CI: 1.2-3.7). After adjusting for maternal morbidity, exposed pregnancies had more caesarean sections (44.4% vs 23.0%, OR: 4.2, 95%CI: 2.3-7.6). Complications occurring during and after exposed pregnancies included stroke, acute myocardial infarction, cardiac arrest, cardiac arrhythmia requiring conversion, eclampsia, pulmonary edema, and adult respiratory distress syndrome. Conclusion: Pregnancies within 3 years prior or any time after IMMS diagnosis had increased maternal morbidity within 1 year of delivery. Prospective studies are needed to better characterize increased maternal risks for women with moyamoya syndrome and develop preventive strategies.

Importance: Large vessel disease (LVD) stroke subtype carries the highest risk of early recurrent stroke, reaching up to 30% in the first few days. Predictors of early recurrence have been previously described, but less is known regarding infarct expansion and other causes of neurological worsening. We aim to determine the association between impaired perfusion and neurological decline in patients with LVD subtype. Methods: This is a single center retrospective cohort study of all consecutive patients 18 years or older with LVD admitted with a diagnosis of ischemic stroke within 24 hours from symptom onset (12/1/2016 to 3/31/2017). Patients with 1) evidence of ≥ 50% stenosis of a large intra- or extracranial artery on computerized tomography angiography (CTA); 2) symptoms referable to the territory of the affected artery and NIHSS &lt; 15 and 3) perfusion imaging data using the RAPID processing software were included. The primary predictor was unfavorable mismatch volume ≥15 mL, defined as perfusion deficit of Tmax &gt; 6sec volume minus infarct volume similar to neuro-interventional trials. The outcome was recurrent cerebrovascular events (RCVE) at 90 days (adjudicated independently by two vascular neurologists) defined as a decline in neurologic function in the absence of a medical cause, or new infarct or infarct extension in the territory of the affected artery. We estimated the hazard ratio (HR) and 95% confidence interval (CI) for unfavorable perfusion imaging as predictor of RCVE using univariable and multivariable Cox proportional hazards models. Results: Sixty-eight patients met our inclusion criteria (mean age 64.7 years; 61.8% male; 58.8% intracranial LVD). When compared to patients without RCVE, patients with RCVE were more likely to have unfavorable mismatch volume [71.4% vs. 14.8%, p&lt;0.001]. This association persisted after adjusting for sex, dual antiplatelet therapy, initial stroke severity, and intracranial location of LVD (adjusted HR 15.6, 95% CI 3.7-66.7, p&lt;0.001). Conclusion: Perfusion mismatch is associated with RCVE in patients with ischemic stroke due to LVD. Pursuit of more aggressive treatment and management strategies may be warranted in this population.
