Safety and reliability are the main issues for designing assistance wearable virtual environment of technical gesture in aerospace, or health application domains. That needs the integration in the same isomorphic engineering framework of human requirements, systems requirements and the rationale of their relation to the natural and artifactual environment. To explore coupling integration and design functional organization of support technical gesture systems, firstly ecological psychologyprovides usa heuristicconcept: the affordance. On the other hand mathematical theory of integrative physiology provides us scientific concepts: the stabilizing auto-association principle and functional interaction. After demonstrating the epistemological consistence of these concepts, we define an isomorphic framework to describe and model human systems integration dedicated to human in-the-loop system engineering. We present an experimental approach of safe design of assistance wearable virtual environment of gesture based in laboratory and parabolic flights. On the results, we discuss the relevance of our conceptual approach and the applications to future assistance of gesture wearable systems engineering.

BackgroundVisual information comprises one of the most salient sources of information used to control walking and the dependence on vision to maintain dynamic stability increases following a stroke. We hypothesize, therefore, that rehabilitation efforts incorporating visual cues may be effective in triggering recovery and adaptability of gait following stroke. This feasibility trial aims to estimate probable recruitment rate, effect size, treatment adherence and response to gait training with visual cues in contrast to conventional overground walking practice following stroke.Methods/designA 3-arm, parallel group, multi-centre, single blind, randomised control feasibility trial will compare overground visual cue training (O-VCT), treadmill visual cue training (T-VCT), and usual care (UC). Participants (n = 60) will be randomly assigned to one of three treatments by a central randomisation centre using computer generated tables to allocate treatment groups. The research assessor will remain blind to allocation. Treatment, delivered by physiotherapists, will be twice weekly for 8 weeks at participating outpatient hospital sites for the O-VCT or UC and in a University setting for T-VCT participants.Individuals with gait impairment due to stroke, with restricted community ambulation (gait speed <0.8m/s), residual lower limb paresis and who are able to take part in repetitive walking practice involving visual cues (i.e., no severe visual impairments, able to walk with minimal assistance and no comorbid medical contraindications for walking practice) will be included. The primary outcomes concerning participant enrolment, recruitment, retention, and health and social care resource use data will be recorded over a recruitment period of 18 months. Secondary outcome measures will be undertaken before randomisation (baseline), after the eight-week intervention (outcome), and at three months (follow-up). Outcome measures will include gait speed and step length symmetry; time and steps taken to complete a 180° turn; assessment of gait adaptability (success rate in target stepping); timed up and go; Fugl-Meyer lower limb motor assessment; Berg balance scale; falls efficacy scale; SF-12; and functional ambulation category.DiscussionParticipation and compliance measured by treatment logs, accrual rate, attrition, and response variation will determine sample sizes for an early phase randomised controlled trial and indicate whether a definitive late phase efficacy trial is justified.Trial registrationClinicaltrials.gov, NCT01600391.

Abstract The high decline rate observed in over pressured shale has attracted the attention of the industry, and better well management procedures for long term productivity improvement are still evolving. Operators are recognizing some benefit in controlled rate (controlled drawdown) production as one way of improving well performance for the wells in over pressured stress sensitive formations. During uncontrolled rate production because of high drawdown, the permeability in stress sensitive shales decays faster because of increased stress. Often high initial gas rate is accompanied by high decline rate as the permeability reduction takes effect. In addition, proppant could also be produced back, crushed or embedded in the formation. However, controlled rate production minimizes the rate decline, albeit at reduced initial gas rate. Modelers resort to using different stress permeability decay coefficients for these two production strategies. Higher values are assigned to uncontrolled rate production to produce lower EUR. This approach, although convenient, requires different permeability versus stress tables depending on the production strategy. Porosity and pore volume reduction in shales could be as high as 20 percent due to changes in net stress. The pore volume reduction provides in situ energy for gas recovery. The increased rate of permeability decay due to changing in situ stresses reduces the effectiveness of pressure support from pore volume reduction as fractures close under stress.. Controlled rate production strategy slows down permeability decay rate and this enables better use of pore volume energy. The pore volume consideration could provide additional gain to EUR for controlled rate. Our analytical simulation model couples geomechanics permeability and porosity stress coefficients and evaluates well performance under moderate and low net stress sensitivity. Haynesville and Marcellus shales were evaluated. The importance of pore volume stress effect from the stand point of well performance evaluation and reservoir characterization is assessed.

Accessible summary People with learning disabilities may not know about sexual relationships. People with learning disabilities may need education about sexual relationships. People with learning disabilities may need education about contraception. SummaryThe introduction of the Mental Capacity Act 2005 has brought to the fore issues regarding capacity in various clinical settings. One important area where capacity assessment is often vital is regarding vulnerable people being able to consent to sexual relations. Case law from courts has evolved with regard to this sometimes complex and emotive issue both before and after the implementation of the Mental Capacity Act 2005. This article describes and analyses the evolution of such case law which predominantly involves people with learning disabilities. In doing so, it explains key principles and tests both for consenting to sexual relations and for the use of contraception which can be applied and used in clinical practice. The article also discusses the associated human rights issues for vulnerable adults in trying to balance the right to sexual relations as part of an individual's private life whilst managing potential abuse issues.

SummaryIssues relating to capacity are increasingly topical and relevant, particularly following the implementation of the Mental Capacity Act 2005. Powers of attorney are one such issue that requires the assessment of capacity for their execution. This article focuses on lasting powers of attorney (LPAs), which were introduced under the Mental Capacity Act in 2007. We describe how LPAs evolved and how they have been used since the inception of the Act. We review salient case law. In doing so, we elucidate pertinent issues for clinicians, particularly in the case of health and welfare LPAs: for the first time in England and Wales, the law allows donors to nominate an attorney to make decisions about their personal health and welfare once they become incapacitated.

There is good evidence for effective transfer of learning from virtual to real-world environments, and this holds true even for complex spatial tasks such as route learning. However, there is little research into the simple equivalence of an individual's performance across real and virtual environments, an important topic which could support the use of virtual reality as an assessment and research tool. This pilot study compared route-learning performance in a desktop virtual town with performance around a real-world route. Participants were "driven" around a route through a virtual town and around a different (but equally complex) route through a real-world suburb, then asked to direct the driver back around each of the routes from memory. They completed strategy checklists after learning each route. Results indicated good equivalence between the real and virtual environments, with comparable error rates and no differences in strategy preferences. This demonstrates that simple desktop virtual environments may be a useful tool for assessment of and research into route learning.

The day was mine. Not going to work meant I could miss the battle of the bathrooms. With time I can manage without help, I told my wife. So with a quiet house I laid out my clothes and meandered down to the shower. Firstly, the shampoo. The only bottle I can squeeze had disappeared, probably into my son's overnight bag. I took my wife's expensive stuff into the living room, lay on the couch, bit the bottle, and managed to get some of the gloop on to my head, the sofa, and under my arms. A sticky stride to the shower ensued and a good elbow turned on a scalding stream. The …

Adrenal corticosteroids in Chinese herbal remediesSir, Edwards et al. 1 provide a useful reminder that traditional Chinese remedies are sometimes adulterated by the addition of substances more familiar in conventional medicine. It is disappointing that this topic is not by now more common knowledge. We described 2 in 1979 a patient treated by us for rheumatoid arthritis the previous year.During the first five months of observation and treatment, she had been getting worse and then suddenly got better. The degree of improvement and timing were unexpected and coincided with her taking a Chinese herbal remedy ('Chuei-Fong-Tou-Geu-Wan') on the advice of friends in Holland. She was entirely open about this, and stopped our treatment, although she continued to attend for assessments. After 3 months our suspicions, based on the abrupt clinical improvement and steep falls in acute protein levels, were confirmed when she began to become Cushingoid.Plasma cortisol concentration was low and it seemed certain that the tablets contained a corticosteroid.Within months we heard that public interest in Holland had led to analysis of the tablets by the Dutch Government Institute of Medicines. The
