Characterization of Carbapenem-Resistant Klebsiella pneumoniae in a Tertiary Hospital in the Eastern Coastal Region of Zhejiang, China: Detection of a Novel ST6853 Clone
PurposeCarbapenem-resistant Klebsiella pneumoniae (CRKP) is linked to hospital-acquired infections and associated with significant mortality rates, presenting a serious risk to public health. This research aimed to examine the in-hospital epidemiology and antimicrobial resistance of CRKP isolates, offering insights for the management and control of CRKP infections.Patients and MethodsIn total, 76 carbapenem-resistant Klebsiella pneumoniae isolates were collected from inpatients at the First Affiliated Hospital of Ningbo University from May 2022 and February 2024. The isolates were mainly acquired from sputum, urine, secretions, and blood. Antimicrobial susceptibility testing was performed using the VITEK 2 Compact system or the Kirby-Bauer disk-diffusion method. In addition to susceptibility testing, phylogenetic analysis, and serotype analysis, whole genome sequencing was conducted to identify genes associated with antibiotic resistance.ResultsAntimicrobial susceptibility testing showed that all 76 CRKP isolates were resistant to most common antibiotics. The CRKP isolates mainly carried Klebsiella pneumoniae carbapenemase, and the predominant gene was blaKPC-2. Homology analysis found that the major sequence type (ST) was ST11, as a single isolate was identified as an ST11 single-locus variant (ST6853), followed by ST15 and ST48. Serotype analysis found that the major serotype was K64, followed by K24 and K62.ConclusionThe CRKP isolates in our study exhibited resistance to the majority of commonly used antibiotics. The blaKPC-2 gene was identified as the predominant carbapenemase. The major ST of CRKP found in our hospital was ST11, with K64 and O2a being the prevalent predicted serotypes. These findings should prove useful for the control and management of CRKP infections, and provided guidance for clinical antibiotic therapy.

Research on dry-peeling of peaches using thermal flame heating
This study proposes and evaluates thermal flame heating dry-peeling (TFHDP) as an environmentally favorable alternative to conventional lye peeling (LP) for peach processing. TFHDP demonstrated comparable peeling efficiency to LP while eliminating the use of chemical agents. The effects of heating duration and heating distance on temperature distribution, peeling performance, and quality attributes were investigated in both frozen and non-frozen peaches. Results showed that TFHDP achieved complete dry peeling, with peeling efficacy enhanced by prolonged heating time and reduced heating distance. Frozen peaches exhibited superior peeling performance relative to non-frozen samples. Compared with LP-treated peaches, TFHDP-treated peaches displayed reduced browning, increased firmness, and better retention of the sugar-to-acid ratio and bioactive compounds in the outer flesh, thereby enhancing antioxidant capacity. Notably, TFHDP effectively preserved the external flesh quality of frozen peaches, with no significant difference observed between external and internal tissues. Thus, optimal processing parameters were identified as 95 s for non-frozen and 45 s for frozen peaches at a heating distance of 5 cm. This study provides key insights into an environmentally favorable and broadly applicable peach peeling method, offering a practical model for fruit and vegetable processing that can reduce energy use and operational costs in the food industry. • Thermal flame heating effectively utilized for dry-peeling of peach. • Flame energy transfers from the peach’s external to its internal during heating. • The heating time and distance affect the peeling performance and quality of peach. • TFH dry-peeling presents a viable, sustainable alternative to lye peeling. • Frozen peaches have better peeling performance and quality than non-frozen samples.

The present study aims to evaluate metformin-associated lactic acidosis (MALA) using real-world data from the Food and Drug Administration Adverse Event Reporting System (FAERS), providing reference for rational metformin use in clinical practice. Relevant adverse event data from January 2018 to March 2024 was extracted from the FAERS database. Disproportionality analysis, logistic regression, and time-to-event analysis were performed to assess risk factors, temporal patterns, and potential drug interactions associated with MALA. Among 40,883 metformin-related adverse event reports, 10,370 were MALA cases. Diabetes (adjusted odds ratio [OR]: 2.35, 95% confidence interval [CI]: 2.03–2.74), heart disease (adjusted OR: 1.75, 95% CI: 1.44–2.14), and hypertension (adjusted OR: 1.40, 95% CI: 1.21–1.62) were identified as significant risk factors. Contrast media showed the highest signal strength for drug interactions (reporting odds ratio = 323.7, 95% CI: 226.8–461.99), followed by furosemide and bisoprolol. The median time to lactic acidosis onset was 106 days, significantly longer than the 20 days for other adverse events (P < .0001), with 44.0% occurring within 30 days and 40.4% after 360 days of therapy initiation. Taken together, our comprehensive analysis of the FAERS database enhances understanding of MALA risk factors and temporal patterns, contributing to informed decision-making in metformin’s clinical application and facilitating timely management of this potentially life-threatening complication, particularly in patients with cardiovascular comorbidities.

Genetic association between human skin microbiota with vaginitis: atwo-sample Mendelian randomization study.
The human skin microbiome is closely associated with various diseases. We aimed to find the causal association of the human skin microbiome with vaginitis. A two-way two-sample Mendelian randomization study used summary statistics of the human skin microbiota from the largest genome-wide association study meta-analysis. Pooled statistics for vaginitis disease were obtained from the FinnGen consortium R10 release. Inverse variance weighted, MR-Egger, weighted median, weighted model, MR-PRESSO, and STEIGER methods were used to test for causal associations between different parts of the human skin microbiota and vaginitis disease in either dry or moist environments. IVW estimates suggest that phylococcus hominischaracterizes a protective effect against vaginitis lesions at the Antecubitalfossa wet site. Staphylococcus in the dry state at the Dorsalforearm site also demonstrated a protective effect against vaginitis lesions. Micrococcus showed a trend of up-regulation of vaginitis risk under IVW method estimation. In contrast, Anaerococcus and Veillonella were associated with a low risk of vaginitis as estimated by the IVW method: ASV061 and ASV065. In the Class, IVW estimation showed that Bacilli at Antecubitalfossa site had an increased risk effect on vaginitis in the moist skin condition. Similarly in Genus, Staphylococcus at Antecubitalfossa sites had an increased risk effect against vaginitis in the moist skin condition. Based on the results of the inverse MR analysis, no significant causal effect of vaginitis on the human skin microbiota was found. This two-sample bidirectional Mendelian randomization study found a causal relationship between seven human skin microbiota and vaginitis, further enriching our understanding of the value of skin flora in reproductive diseases such as vaginitis.

Abstract MicroRNAs (miRNAs) have been recognized as significant mediators in non-small cell lung cancer (NSCLC). However, their particular effects on NSCLC remain largely unknown. In addition, fibroblasts are essential components of the tumor microenvironment (TME) and play significant roles in tumor development. It has been also reported that exosomes can mediate the communication between tumor cells and fibroblasts in TME. However, the role of extracellular miR-21 in the intercellular communication of tumor cells and fibroblasts remains elusive. The differentially expressed miRNAs between NSCLC and normal tissues were identified by screening the Gene Expression Omnibus (dataset, GSE63805) and The Cancer Genome Atlas databases using R language software. The bioinformatics analysis results showed that compared with normal tissues, miR-21-5p was upregulated in lung cancer tissues, while the increased expression levels of miR-21-5p were associated with worse survival rate in patients with NSCLC. Additionally, exosomes could deliver miR-21-5p from NSCLC cells to lung fibroblasts, thus enhancing the levels of miR-21-5p in fibroblasts. Furthermore, the results demonstrated that tumor necrosis factor α induced protein 3 (TNFAIP3) was a direct target of miR-21-5p in NSCLC cells. These results suggested that exosomal miR-21-5p derived from NSCLC cells could activate lung fibroblasts via targeting TNFAIP3-mediated nuclear factor κB (NF-κB) signaling. Most importantly, exosomal miR-21-5p could facilitate fibroblast activation, which in turn could enhance the migration and epithelial-mesenchymal transition of NSCLC cells. Overall, the findings of the present study revealed that NSCLC cell-derived exosomal miR-21-5p could mediate lung fibroblast activation via the TNFAIP3/NF-kB signaling pathway to promote NSCLC cell migration and invasion. Therefore, targeting exosomal miR-21-5p could provide a potential strategy for the prevention and treatment of NSCLC.

To investigate the emotional response of hospital staff to the Depression Anxiety Stress Scale-21 (DASS-21) over the COVID-19 pandemic and after the relaxation of prevention and control measures in Zhejiang Province, China, and identify influencing factors. Multicenter online cross-sectional design. From January 10, 2023, to January 20, 2023, 1054hospital employees in Zhejiang, China, were recruited using WeChat. Data was gathered via online, self-administered surveys. T-tests and one-way analyses of variance, Pearson's correlation analysis, and multiple linear stepwise regression analyses were used to examine the data. More than 90% of hospital employees were infected with COVID-19. Through regression analysis, the following variables were found to be independent predictors of emotional response to DASS-21: resilience (-13.346, P < 0.0001), sleep (14.689, P < 0.0001), friend support (-4.278, P < 0.0001), education level (2.699, P = 0.007), and marriage (-2.214, P = 0.027). The emotional responses were not as severe as predicted, but they were still well above the Chinese norm. A longitudinal comparison of DASS-21 emotional responses with similar studies showed a parabolic downward trend over time. Our results identified education level, marriage, friend support, resilience, and sleep as independent predictors of emotional responses to the DASS-21 among hospital workers in this outbreak. Improving the mental resilience and sleep status of staff is a key target. Unmarried medical personnel with higher education should be given greater attention and support by management. In addition, there is still room for further improvement in the government and societal responses to similar outbreaks. The study also found a parabolic downward trend in DASS-21 emotional responses among hospital workers during the COVID-19 outbreak over time.

Abstract The aim of this study to explore the use of new-media health information and its influence on their health and health management. Methods: A total of 208 elderly people in Qiqihar City were selected as the survey objects, and a questionnaire was used to investigate their access to health information through new-media platforms. We analyze the factors ( e.g., education、pre-retirement occupation,etc) in the use of health information on new-media platforms. Results: Monthly income, educational level, pre-retirement occupation and attention of the new-media of the elderly had significant effects on health information acquisition (p<0.05). Conclusion: The use of new-media health information among the elderly was diversified and had an impact on their health management. We should standardize the management of new-media health information dissemination platforms and publicize the use of new-media health information in a targeted way, so as to improve the self-health management of the elderly and reduce their burden on the national medical system.
