Fetal Considerations in the Critically Ill Gravida
This chapter is dedicated to assessment of fetal health in the intensive care unit (ICU) setting with the goal of providing fundamental scientific information for the non-obstetric clinician, and important clinical fetal considerations encountered when caring for these clinically complicated pregnancies. It reviews current techniques for assessing fetal well-being, fetal assessment in the ICU, fetal considerations in selected maternal medical and surgical conditions, the contemporary management of the gravida who is brain-dead or in a persistent vegetative state, and the role of perimortem cesarean delivery in contemporary obstetrics. The chapter presents an overview of fetal heart rate (FHR) patterns pertinent to the critically ill gravida in labor with contractions. It focuses on periodic FHR changes that occur in response to uterine contractions, such as FHR accelerations and variable and late decelerations. Perimortem cesarean delivery has been described as an attempt to preserve life of the unborn child when the pregnant woman dies.

We define social media as an interactive online platform that allows users to communicate and exchange knowledge. Educational and medical profiles have slowly emerged on different social media platforms, helping to teach about and publicize diverse aspects of medicine. Radiology is one of the specialties that could potentially benefit the most from social media, as the radiologist tends to have little outside-the-hospital representation. Progressively, audiovisual content has been gaining ground on social networks: Facebook, Twitter, Instagram, Youtube, TikTok, etc. Instagram appears to be ideally suited for radiology given its image-based nature. In addition, Instagram can also be used as a tool to help radiologists share and discuss radiological images, improve communication with clinicians and patients, advertise themselves and their specialty, and humanize their profession. Nevertheless, legal matters and privacy issues should always be taken into account when using these tools. In this overview, we describe the development of social networks and communication tools in our own radiology department, focusing especially on our Instagram account, as it has had a wide impact on our hospital and radiology residents around the country. We will also provide a summary of the various social media platforms used for radiology education along with their pros and cons, including useful tips for safe and efficient use.
Read moreA decade of experience with the primary pull-through for hirschsprung disease in the newborn period: a multicenter analysis of outcomes.
To determine whether use of a primary pull-through would result in equivalent perioperative and long-term complications compared with the two-stage approach. During the past decade, the authors have advanced the use of a primary pull-through for Hirschsprung disease in the newborn, and preliminary results have suggested excellent outcomes. From May 1989 through September 1999, 78 infants underwent a primary endorectal pull-through (ERPT) procedure at four pediatric surgical sites. Data were collected from medical records and a parental telephone interview (if the child was older than 3 years) to assess stooling patterns. A similar group of patients treated in a two-stage fashion served as a historical control. Mean age at the time of ERPT was 17.8 days of life. Comparing primary ERPT with a two-stage approach showed a trend toward a higher incidence of enterocolitis in the primary ERPT group compared with those with a two-stage approach (42.0% vs. 22.0%). Other complications were either lower in the primary ERPT group or similar, including rate of soiling and development of a bowel obstruction. Median number of stools per day was two at a mean follow-up of 4.1 +/- 2.5 years, with 83% having three or fewer stools per day. Performance of a primary ERPT for Hirschsprung disease in the newborn is an excellent option. Results were comparable to those of the two-stage procedure. The greater incidence of enterocolitis appears to be due to a lower threshold in diagnosing enterocolitis in more recent years.

OSMOLALITIES OF ORAL AND PARENTERAL MEDICATIONS USED IN THE NEONATAL INTENSIVE CARE UNIT
Although the association between hypertonic medications and the development of Necrotizing Enterocolitis is unclear, hyperosmolar materials (more than 400 mOsm/kg H2O) can cause an osmotic diuresis, loose stools and probable mucosal injury.
