IntroductionLifestyle change is a Class I indication for the management of chronic diseases such as atherosclerosis and Cardiovascular diseases(CVD). The Syndemic of diabesity indicates that a ‘lose weight and exercise more’ strategy has abjectly failed. We have devised an Artificial Intelligence mediated digital real-time personalised lifestyle intervention for patients with CVD risk factors (obesity, Type II Diabetes; Hypertension; dyslipidaemia; metabolic syndrome). This has been used in corporate, primary care settings and for a cohort with Type II diabetes mellitus. Since 2015 we have performed multiple cohort studies in at risk groups in various setting achieving similar favourable results.MethodsSubjects were from primary care, corporate and diabetic clinic settings. Subjects enrol via an App in the Redicare Program (controldtx.com). Baseline parameters are obtained (height, weight, BMI, systolic & diastolic BP, HbA1c, lipid profile). Exercise (steps), diet, caloric intake, are monitored via Artificial intelligence in real-time. Subjects receive advice via text and dietary education via nudge psychology videos and have 1:1 access to a health coach. Dietary advice is strictly evidence based. Lifestyle courses depending on intensity can last 4–32 weeks (figure 1). Laboratory parameters are monitored by the patient’s primary care physician and medication changes made as needed. Pre and post-intervention parameters were analysed using the student t-test.Results the composite results of these cohorts are shown in table 1. The average weight and BMI loss is ~6.5% (n=320: p<0.01); HbA1c reduction is ~10.9% (n=80: p<0.01); blood pressure reductions were systolic ~7.1% and diastolic ~5.9% (n=90; p<0.01); triglycerides were reduced ~30% (n=83; p<0.01). Patients noted improved quality of life and the system incorporates seamlessly into the primary care practice via Healthlink. Reductions in medications are noted, especially in patients with or at risk of diabetes mellitus. 61 Figure 1Course options 61 Table 1Composite resultsConclusionAn Artificial Intelligence mediated digital Lifestyle change programme is effective at significantly reducing multiple metabolic syndrome parameters in patients. It provides real-time monitoring of patients which is linked to their primary care physician and embeds seamlessly into GP practice. This system is personalised and scalable; and provides a template for lifestyle change for cardiovascular diseases delivered at a fraction of the cost of drug therapies.
