Définir les résultats essentiels en matière de prescription sociale à l’aide d’une approche Delphi : constatations et orientations futures
Introduction Bien que la prescription sociale constitue un courant de plus en plus important en faveur de la santé globale et de la société, aucune étude Delphi n’a déterminé quels résultats sont essentiels à évaluer. Notre objectif était de faire émerger un ensemble de résultats fondamentaux en fonction des commentaires de divers groupes d’utilisateurs susceptibles d’être concernés par la prescription sociale (aînés de 60 ans et plus) ou de personnes pouvant avoir une influence sur elle (comme les fournisseurs ou les chercheurs). Méthodologie Conformément aux lignes directrices normalisées pour les études Delphi, nous avons élaboré une enquête en ligne à deux cycles en ciblant les perspectives canadiennes. Nous avons demandé aux participants d’évaluer 21 résultats comme « cruciaux » (7 à 9 sur une échelle de 9 points), « importants mais non cruciaux » (4 à 6 points) ou « sans importance » (1 à 3 points). Nous présentons une description du sous-groupe de résultats obtenus du point de vue des aînés et de leurs familles et amis. Résultats Le premier cycle a été complété par 74 personnes relevant de 10 groupes d’utilisateurs et le second par 52 personnes relevant de 8 groupes d’utilisateurs (taux de rétention de 70 %). Les évaluations entre les cycles ont été généralement uniformes. Sept résultats ont atteint le seuil de résultat « crucial ». Aucun résultat n’a été exclu. Les résultats cruciaux portaient sur la santé mentale, le fonctionnement physique et social et le bien-être. Les participants ont formulé des commentaires sur les facteurs contextuels (ressources, prestation de soins, etc.) et les facteurs liés à l’équité Conclusion Cette étude a permis de faire émerger sept résultats cruciaux à prendre en compte dans les évaluations en matière de recherche et d’interventions portant sur la prescription sociale. Les recherches futures devraient porter sur la manière dont les facteurs contextuels et personnels ont une influence sur les résultats et elles devraient définir des outils précis (questionnaires, tests axés sur le rendement) pour évaluer chaque résultat. Définir quels résultats sont importants est un processus continu qui nécessite des mises à jour régulières dans la mesure où ces résultats sont susceptibles de changer en fonction de l’évolution de la prescription sociale et d’autres facteurs.

Using EASE to Identify Characteristics That Distinguish Traditional Versus Person Centered Living Areas
Abstract The Environmental Audit Screening Evaluation tool (EASE) is a unique assessment tool which was administered in 228 living areas across US and Canada. One-way analysis of variance was used to estimate eta-squared values as indicators of effect sizes between settings that were identified as being either traditional or homelike. Elimination of items with eta-square values 1.0 or less resulted in 39 items that maximally distinguished between different types of settings. These 39 items were subjected to parallel analysis which generated a 4-component solution that provided high loadings with few cross-loadings. Specifically, this subset of EASE items appeared to assess: 1) residential and familiar character (14 items) (e.g., kitchen design appears residential in style, and required institutional features (such as exhaust vents) are disguised) 2) orientation and functional supports (15 items) (e.g., doors to dangerous storage largely out of sight) 3) human scale and intuitive layout (8 items) (e.g., bedrooms close to social spaces) and 4) autonomy in daily living (8 items) (e.g., showers allow privacy and independence). The value and use of this subset of items as a tool in multi-site research will be discussed.

Abstract One goal of the EASE is to assist care providers who have older, traditional buildings determine how to best utilize scarce resources when considering renovations that will have the most positive impact on residents, families and staff. Analysis of differences between settings that clearly reflect traditional design elements (long, double loaded corridors and minimal space for meaningful social engagement) and settings that have adopted some person-centered care values identify smaller scale and the presence of a functional kitchen as key factors. Both of these involve significant operational as well as environmental changes and can be met with resistance. This session describes the process of working with a care provider that had seven highly traditional living areas assessed with the EASE which resulted in identification of specific recommendations to reduce the size of living areas and include functional kitchens. The evidence-based nature of the EASE supports constructive responses to perceived barriers to adoption of changes that require operational changes. The EASE also identifies numerous low-cost interventions that can be easily adopted allowing for a phased implementation process.
