Machine Learning Applied to Cholesterol-Lowering Pharmacotherapy: Proof-of-Concept in High-Risk Patients Treated in Primary Care.
Objectives: Machine learning has potential to improve the management of lipid disorders. We explored the utility of machine learning in high-risk patients in primary care receiving cholesterol-lowering medications. Methods: Machine learning algorithms were created based on lipid management guidelines for England [National Institute for Health and Care Excellence (NICE) CG181] to reproduce the guidance with >95% accuracy. Natural language processing and therapy identification algorithms were applied to anonymized electronic records from six South London primary care general practices to extract medication information from free text fields. Results: Among a total of 48,226 adult patients, a subset of 5630 (mean ± standard deviation, age = 67 ± 13 years; male:female = 55:45) with a history of lipid-lowering therapy were identified. Additional major cardiometabolic comorbidities included type 2 diabetes in 13% (n = 724) and hypertension in 32% (n = 1791); all three risk factors were present in a further 28% (n = 1552). Of the 5630 patients, 4290 (76%) and 1349 (24%) were in primary and secondary cardiovascular disease prevention cohorts, respectively. Statin monotherapy was the most common current medication (82%, n = 4632). For patients receiving statin monotherapy, 71% (n = 3269) were on high-intensity therapy aligned with NICE guidance with rates being similar for the primary and secondary prevention cohorts. In the combined cohort, only 46% of patients who had been prescribed lipid-lowering therapy in the previous 12 months achieved the NICE treatment goal of >40% reduction in non-high-density lipoprotein cholesterol from baseline pretreatment levels. Based on the most recent data entry for patients not at goal the neural network recommended either increasing the dose of statin, adding complementary cholesterol-lowering medication, or obtaining an expert lipid opinion. Conclusions: Machine learning can be of value in (a) quantifying suboptimal lipid-lowering prescribing patterns, (b) identifying high-risk patients who could benefit from more intensive therapy, and (c) suggesting evidence-based therapeutic options.

Background: The aim of the present study was to assess the role of 3 Tesla (3T) magnetic resonance imaging (MRI) in the assessment of infiltrative cardiomyopathy (ICM).Methods: Cardiac MRI was performed on a 3T MRI machine for 15 patients who had clinical or echocardiographic signs of infiltrative cardiomyopathy. Each scan was assessed on a set of anatomical and functional parameters. The patterns of left ventricular (LV) late gadolinium enhancement (LGE) were also analyzed.Results: Bi-atrial dilatation was noted in 14 patients, consistent with a restrictive phenotype. All 15 patients had diastolic dysfunction with reduced LV diastolic ventricular filling and prolonged peak filling times. Eleven patients had a decreased peak filling rate. Twelve patients had systolic dysfunction with reduced ejection fraction (EF). Ten patients had contractile dysfunction in the form of global LV hypokinesia. On delayed contrast imaging, four patients showed no abnormal LGE. Two patients showed diffuse subendocardial enhancement. Two patients showed patchy subendocardial enhancement. Six patients showed patchy mid-myocardial enhancement. One patient showed diffuse mid-myocardial enhancement. Three patients showed patchy subepicardial enhancement. Two patients showed patchy transmural enhancement. Three patients showed reversed myocardial nulling. All 15 patients received a provisional diagnosis of infiltrative cardiomyopathy on the basis of cardiac MRI findings. Sarcoidosis was given as a probable cause in four patients, amyloidosis in three patients, an infectious cause in two patients, and drug-induced cardiomyopathy in one patient. In five patients, no obvious cause could be identified.Conclusion: Infiltrative cardiomyopathies, although relatively uncommon, pose significant challenges in diagnosis and treatment. Cardiac MRI has become the gold standard for non-invasive diagnosis of all infiltrative cardiomyopathies.

En s’appuyant sur l’exemple de Doodle m’a tuer , l’article s’emploie à montrer dans quelle mesure la fiction peut venir élargir le répertoire des gestes documentaires et renseigner des problématiques sociétales avec une acuité renouvelée. En mobilisant la sociologie interactionniste autant que l’histoire de la poésie, les sciences de l’information et surtout la pratique radiophonique, il s’agira de montrer pourquoi la fiction ne doit pas être tenue en disjonction avec le régime testimonial. Alors que le développement du fact-checking risque de nous couper de ces enjeux, les témoignages les plus authentiques, relevés par une poétique plus ou moins explicite, restent d’une sincérité toujours discutable.

»Wir haben kein ›Racial Profiling‹-Problem«, behauptet Max Hofmann vom Verband Schweizerischer Polizeibeamter in einem Interview mit dem offentlich-rechtlichen Schweizer Radiosender SRF. Denn die Polizei kontrolliere keinesfalls einzelne Personen oder Gruppen aufgrund deren Hautfarbe. Solche und ahnliche Behauptungen ausern zahlreiche Vertreter* innen der Polizei, aber auch der Politik - nicht nur in der Schweiz.1 Problematisch daran ist nicht nur die Behauptung selbst, sondern auch, dass solche Aussagen noch immer die offentliche Auseinandersetzung um Racial Profiling dominieren und damit sowohl eine langst uberfallige Debatte als auch notwendige Sanktionen gegen rassistische Polizeipraktiken weitgehend blockieren.2 Diese Form der Bagatellisierung aber hat System: Mittels Abwehr und Verdrangung wird nicht nur jegliche Verantwortung zuruckgewiesen, sondern zudem die Benennung diskriminierender Praktiken verunmoglicht. Rassismus und Polizei werden als undenkbare Verbindung begriffen und damit wird verhindert, Praktiken der Polizeikorps als strukturell rassistisch identifizieren zu konnen. Eine zentrale Einsicht gesellschaftskritischer Ansatze der Rassismusforschung besteht aber darin, dass sich Rassismus sowohl durch intentional oder unbewusst agierende Individuen artikuliert, als auch in vielfaltiger Weise durch institutionelle Prozesse, in gesellschaftlichen Strukturen und in allgemeinen Werten und Normen wirkt. Solche als institutionell, strukturell oder auch systemisch bezeichnete Rassismen finden alltaglich statt, werden aber oft nicht direkt sichtbar, weil sie nicht als hate speech, Beleidigungen oder Ausschlusse daherkommen, sondern sich in den als normal geltenden Entscheidungen und Handlungen gesellschaftlicher Institutionen verstecken.3 In gewisser Weise liegt dem Rassismus eine »Banalitat« inne, schreibt Mark Terkessidis4 in Anlehnung an Hannah Arendt, um die Verwobenheit von Rassismus in die alltaglichen Funktionsmodi von Gesellschaft zu bezeichnen. Racial Profiling stellt in diesem Sinne eine Praxis dar, die sowohl bei der Polizei als auch in der Mehrheitsgesellschaft uberwiegend als »normale« Polizeikontrolle angesehen wird, wahrend sie bei den immer wieder Kontrollierten immens negative Wirkungen hervorruft.

Sans parler des menaces pesant sur les administrations, formulées par les armées d’avocats des firmes pharmaceutiques, ou de l’invention pure et simple de nouvelles maladies par les services de marketing.

MIC Image-Reject and -Enhancement Mixers for Radio-Link Applications
A class of broadband image-reject and enhancement mixers for radio-link applications has been developed. The mixers are constructed in micro-wave-integrated-circuit (MIC) form. Their design concept is explained and details of the actual implementation are presented. Typical performances for 3.8 to 4.2 GHz are 4.5 dB noise figure, 25 dB image suppression and ? 25 dB LO-RF isolation. The mixers can be used as the first element in a receiver or combined with an RF preamplifier, but also as a single-side-band upconverter.
