Effects of a cystic artery-first Calot's triangle laparoscopic approach versus conventional laparoscopic cholecystectomy on therapeutic efficacy and complications in acute cholecystitis.
Acute cholecystitis (AC) is a common biliary disorder, most often caused by gallstones obstructing the cystic duct and leading to gallbladder inflammation. This study aimed to compare the therapeutic efficacy and complication rates of laparoscopic cholecystectomy (LC) performed using the Calot's triangle approach vs traditional LC techniques in the treatment of AC. A retrospective analysis was conducted on 120 patients diagnosed with AC, with 60 patients undergoing LC using the Calot's triangle approach (study group) and 60 patients treated with traditional LC techniques (control group). Surgical parameters, including operation time, intraoperative hemorrhage, postoperative recovery times, and 30-day postoperative complications were recorded. Intraoperative adhesion formation was evaluated through direct visualization and graded based on severity. Postoperative pain was assessed using the visual analogue scale (VAS). There was no statistically significant difference in the baseline characteristics between the 2 groups, confirming their comparability. The study group (Calot's triangle approach) demonstrated significantly shorter average operation time, postoperative exhaust time, and diet recovery time compared to the control group. Additionally, patients in the study group had significantly lower intraoperative bleeding, lower VAS pain scores at 24 h and 72 h postoperatively, and a lower overall complication rate compared to the control group (p < 0.05). The LC Calot's triangle approach demonstrated shorter operation times and lower rates of certain complications compared with traditional LC techniques. However, the absence of statistically significant differences in some key outcomes highlights the need for further research to fully evaluate its clinical advantages and long-term benefits.

Nonlinear associations between the aggregate index of systemic inflammation and cardiovascular disease in adults: evidence from NHANES 2011-2020.
Although the connection between inflammation and cardiovascular disease (CVD) is well-documented, the association involving the aggregate index of systemic inflammation (AISI)-a distinctive indicator derived from this formula (neutrophil count × monocyte count × platelet count)/lymphocyte count-has not been extensively explored. Addressing this research gap, our study leveraged the National Health and Nutrition Examination Survey (NHANES) database for investigating a correlation among AISI and CVD among adults in U.S. This investigation provides new insights into the inflammatory mechanisms underlying cardiovascular conditions and may have implications for preventive and therapeutic strategies in cardiovascular health. Our study included 23,472 adults from the 2011-2020 NHANES database. The relationship between AISI and CVD was examined using weighted generalized linear models, subgroup analyses, and threshold effects to estimate effect sizes and saturation effects. Nonlinear relationships between the AISI and CVD were further elucidated through smoothed curve analyses. The prevalence of CVD increases with AISI(P < 0.001). Within weighted generalized linear models, individuals within Quartile 4 [OR = 1.27, 95%CI (1.00, 1.61)] exhibited a favorable correlation with CVD prevalence, in contrast to the figures observed during Quartile 1. Notably, this correlation was consistent across various subgroups, indicating its independence from factors such as age, ethnicity, smoking, hyperlipidemia, hypertension, and diabetes mellitus (P for interaction > 0.05). Additionally, using threshold effect analysis, we identified a nonlinear relationship across AISI and CVD, with a critical inflection point observed at an index value of 467. Above this threshold, every increment of 100 units within AISI was linked to a 3% elevated CVD prevalence [OR = 1.03, 95%CI (1.00, 1.07)]. Our work substantiates robust evidence for a important positive correlation between AISI and CVD. However, it also highlights the importance of conducting additional prospective research to comprehensively explore the function of AISI in etiology and advancement of CVD.

Malignant transformation complicates diagnosing and treating solitary fibrous tumors (SFTs) hard due to its complexity and risk of severe spreading. In this study, we report a 51-year-old woman with a history of brain tumor treatment who experienced recurrence after 9 years, treated by surgery and radiotherapy. Four years later, she had recurrent diarrhea and liver and lung metastases, which were confirmed by histologically, immunohistochemically (positive for STAT6, CD34, and Ki-67), and by CT (liver nodules 111 × 102 mm and lung nodules 55 × 33 mm). The patient suffered from transcatheter arterial embolization, hepatic artery infusion chemotherapy (oxaliplatin + cyclophosphamide), intravenous antiangiogenic therapy (bevacizumab), and intravenous immunotherapy (sindilizumab). A re-examination on January 3, 2023, showed liver lesions and a solid nodule (112 × 82 mm) in the right lobe, reduced from previously. Multiple nodules and masses in both lungs were also noted, with a larger nodule in the left lung (55 × 34 mm). Follow-up exams in May and August 2023 showed improvement. This rare case highlights the potential of integrated therapy for inoperable, brain-originated metastatic SFT.

Circular RNAs in endometriosis analyzed through RNA sequencing and bioinformatics for expression profile.
Endometriosis severely affects women's physical and mental health; it is particularly important to find targets for the treatment and diagnosis of endometriosis. This research aimed to investigate the circRNA expression pattern in endometriosis, a type of non-coding RNA that can modulate parental gene expression by acting as miRNA sponges. Through high-throughput sequencing, we analyzed the circRNA expression profile in endometriosis patients in comparison to individuals without the condition. We detected 371 circular RNAs (circRNAs) showing increased expression and 308 circRNAs displaying decreased expression levels. To validate these findings, we employed quantitative real-time PCR (qRT-PCR) to confirm the expression of the top three differential expressed circRNAs listed in circBase. We inferred potential roles of these differentially expressed circRNAs in endometriosis development by Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis. Moreover, by examining the circRNA-microRNA-target gene network, we uncovered a plausible mechanism. Specifically, interactions involving the markedly upregulated hsa_circ_000005 and significantly downregulated hsa_circ_000011 with miR-5787 may influence downstream targets, potentially contributing to the pathogenesis of endometriosis. Our study offers a foundational and crucial circRNA expression profile within the framework of endometriosis.

BackgroundImpaired inhibition of inappropriate responses in the emotional context is a core feature in patients with bipolar disorder. However, there has been little research exploring the underlying mechanism of impaired response inhibition for emotional stimuli in adolescents with bipolar depression. To explore this issue, we employed event-related potentials (ERPs) to investigate the underlying neuroelectrophysiological mechanisms of inhibition of inappropriate emotional stimuli in adolescents with bipolar depression.MethodsTwenty-five adolescents with bipolar depression and nineteen healthy controls completed an emotional Go/No-Go task during electroencephalography recording. Reaction time (RT), reaction time variability (RTV), discriminability, and response bias were measured as behavioral performance indicators. ERP components, theta-band oscillation and inter-trial coherence (ITC) were compared between the two groups.ResultsBehavioral performance analysis found that adolescents with bipolar depression showed smaller d’ values, and larger RT and RTV, than healthy controls. Nogo-P3 amplitude was decreased in adolescents with bipolar depression in comparison with healthy controls. Theta-band oscillation and ITC for emotional stimuli were also reduced in adolescents with bipolar depression. Pearson correlation analysis showed there was a negative correlation between the Nogo-P3 amplitude induced by negative trials and RTV in adolescents with bipolar depression.ConclusionOur findings suggest that adolescents with bipolar depression exhibit abnormal response inhibition in the emotional context. Impaired attentional function and discrimination of emotional information are related to the failure of behavioral inhibition in negative emotional contexts, and attenuated P3 amplitude and theta-band oscillation could be an electrophysiological indicator for this impairment.
Read moreApplication of Intravenous Chloroprocaine in Gastrointestinal Endoscopy: A Randomized Controlled Trial.
While propofol is a commonly utilized medication for sedation during gastrointestinal endoscopy, it is associated with adverse effects such as hypotension and injection pain. This trial was conducted to test the hypothesis that chloroprocaine can reduce the requirement for propofol and alleviate injection pain during gastrointestinal endoscopy. Sixty patients undergoing gastrointestinal endoscopy were enrolled and randomly divided into study group (Group CP) and control group (Group C). Patients in Group CP received intravenous chloroprocaine 2 mg/kg, followed by continuous infusion at 6 mg·kg-1·h-1 until the end of examination. Patients in Group C received the same volume of saline. Subsequently, all patients were intravenously administered sufentanil at a dose of 0.05μg/kg. Thirty seconds later, propofol was uniformly infused intravenously at a rate of 60 mL/min using an infusion pump. The primary outcome was the consumption of propofol. Secondary outcomes included the incidence of hypoxemia, hypotension, bradycardia, injection pain, and coughing/body movement during examination. The recovery time, PACU stay time, postoperative pain score, and endoscopists' satisfaction score were also recorded. Group CP demonstrated a significantly lower total requirement for propofol compared to Group C, with means of (119±14) mg and (148±18) mg respectively, P<0.001. This trend was also observed for both the first and supplemental doses. There were no significant differences between the two groups regarding intraoperative adverse events. The incidence of injection pain in Group CP was lower than that in Group C (P=0.007). The recovery time [(4.7±1.4) vs (6.6±1.3), P<0.001], PACU stay time [(13.0±2.9) vs (16.7±3.0), P<0.001] and postoperative pain score [(1.9±0.7) vs (2.5±0.7), P=0.002] in Group CP were lower than those in Group C. Intravenous chloroprocaine reduces the requirement for propofol, alleviates propofol injection pain, and improves recovery in patients undergoing gastrointestinal endoscopy.
