PurposeTo evaluate the diagnostic performance of the current three-step cervical cancer screening strategy in Chinese women and explore the optimized value of integrating human papillomavirus (HPV) testing, ThinPrep cytology test (TCT), and colposcopic visual inspection with acetic acid/Lugol’s iodine (VIA/VILI) for improving cervical disease detection.Patients and MethodsA retrospective study was conducted on 11,405 outpatients of Shenzhen People’s Hospital (2018–2023). Inclusion criteria were a history of sexual activity plus at least one of the following: high-risk HPV (HR-HPV) positivity, abnormal TCT results, or highly suspicious symptoms. Exclusions included prior hysterectomy, cervical cancer (CC) treatment history, pregnancy, or incomplete medical records. The diagnostic performance of HR-HPV testing, TCT, and colposcopic VIA/VILI results was evaluated using pathologically confirmed high-grade squamous intraepithelial lesion (HSIL) and CC as the reference standard. The calculated metrics comprised sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and area under the curve (AUC).ResultsAmong the cohort, the positive rates were 71.56% for HR-HPV, 32.57% for abnormal cytology, and 9.34% for colposcopic VIA/VILI. For detecting HSIL/CC, the NPVs of HPV testing, TCT, and colposcopic VIA/VILI were 0.976, 0.955, and 0.965, respectively. Combined use of the three modalities achieved an NPV of 0.999 for HSIL/CC. Integrating colposcopic VIA/VILI into primary screening also reduced both false-positive and false-negative results compared with single-modality strategies.ConclusionIn this large real-world retrospective cohort, combining HPV testing, TCT, and colposcopic VIA/VILI improves the diagnostic accuracy of high-grade cervical disease (HSIL and CC), with practical feasibility for clinical application. However, this integrated approach needs prospective multi-center validation before widespread promotion in cervical cancer screening among community populations.

Methadone maintenance treatment (MMT) is effective for managing opioid use disorder, but adverse effects mean that optimal therapy occurs with the lowest dose that controls opioid craving. To assess the efficacy of acupuncture versus sham acupuncture on methadone dose reduction. Multicenter, 2-group, randomized, sham-controlled trial. (Chinese Clinical Trial Registry: ChiCTR2200058123). 6 MMT clinics in China. Adults aged 65 years or younger with opioid use disorder who attended clinic daily and had been using MMT for at least 6 weeks. Acupuncture or sham acupuncture 3 times a week for 8 weeks. The 2 primary outcomes were the proportion of participants who achieved a reduction in methadone dose of 20% or more compared with baseline and opioid craving, which was measured by the change from baseline on a 100-mm visual analogue scale (VAS). Of 118 eligible participants, 60 were randomly assigned to acupuncture and 58 were randomly assigned to sham acupuncture (2 did not receive acupuncture). At week 8, more patients reduced their methadone dose 20% or more with acupuncture than with sham acupuncture (37 [62%] vs. 16 [29%]; risk difference, 32% [97.5% CI, 13% to 52%]; P < 0.001). In addition, acupuncture was more effective in decreasing opioid craving than sham acupuncture with a mean difference of -11.7 mm VAS (CI, -18.7 to -4.8 mm; P < 0.001). No serious adverse events occurred. There were no notable differences between study groups when participants were asked which type of acupuncture they received. Fixed acupuncture protocol limited personalization and only 12 weeks of follow-up after stopping acupuncture. Eight weeks of acupuncture were superior to sham acupuncture in reducing methadone dose and decreasing opioid craving. National Natural Science Foundation of China.

Objective: The programmed death receptor 1/programmed death ligand 1 (PD-1/PD-L1) pathway can negatively regulate the immune response of the body, and serum soluble PD-L1 (sPD-L1) can reflect the expression level of PD-L1. This study aims to compare the expressional differences of sPD-L1 in serum between patients with chronic hepatitis B (CHB) and C (CHC) and further explore the factors influencing the clinical cure of CHB. Methods: 60 cases with CHB, 40 cases with CHC, and 60 healthy controls were selected. Serum levels of sPD-L1 were detected using an ELISA kit. The relationship between sPD-L1 levels and viral load, liver injury indicators, and others was analyzed in CHB and CHC patients. According to the distribution type of the data, a one-way ANOVA or Kruskal Wallis test as well as Pearson's correlation or Spearman's rank correlation analysis were performed. A difference of P<0.05 was considered statistically significant. Results: The serum sPD-L1 levels were significantly higher in CHB patients (414.6 ± 214.9) pg/ml than those in CHC patients (58.9 ± 122.1) pg/ml and the healthy control group (66.27 ± 24.43) pg/ml, and there was no statistically significant difference in serum sPD-L1 between CHC patients and the healthy control group. Further grouping and correlation analysis showed that the level of serum sPD-L1 was positively correlated with the content of HBsAg in CHB patients but not with HBV DNA, alanine transaminase, albumin, and other liver injury indicators. Additionally, there was no correlation between serum sPD-L1 levels, HCV RNA, and liver injury indicators in CHC patients. Conclusion: The serum sPD-L1 levels are significantly higher in CHB patients than those in the healthy control group and the CHC group, and there is a positive correlation between sPD-L1 levels and HBsAg. The persistent presence of HBsAg is an important mechanism for the activity of the PD-1/PD-L1 pathway, indicating that the activity of the PD-1/PD-L1 pathway may be an important factor that cannot be clinically cured in CHB as in CHC.

Abstract Background: A lot of recent researches have focused on the duration of the nature immunity elicited by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. An improved understanding of the immunity offered by the antibodies developed against SARS-CoV-2 in recovered patients is critical for the development of diagnostic tests and vaccines. Methods: We enrolled 114 donors, which providing blood samples after discharge for half a year and one year. Neutralizing antibodies (NAbs) were tested using a micro-neutralization assay. Results: In two tests, 82 of 114 recovered patients completed the first test half a year after discharge and NAbs remained detectable in the vast majority of patients (75/82, 91.46%). In the comparison of the two intervals, 50% (27/54) of individuals had increased NAbs titers. when 31.48% (17/54) of patients remained unchanged. Conclusion: Our results suggest that immune ability is acquired in most individuals infected with SARS-CoV-2 and is sustained in a majority of patients for up to a year after recovery.

Abstract BackgroundEffective therapies are urgently needed for the SARS-CoV-2 pandemic. Chloroquine has been proved to have antiviral effect against coronavirus in vitro. In this study, we aimed to assess the efficacy and safety of chloroquine with different doses in COVID-19.MethodIn this multicenter prospective observational study, we enrolled patients older than 18 years old with confirmed SARS-CoV-2 infection excluding critical cases from 12 hospitals in Guangdong and Hubei Provinces. Eligible patients received chloroquine phosphate 500mg, orally, once (half dose) or twice (full dose) daily. Patients treated with non-chloroquine therapy were included as historical controls. The primary endpoint is the time to undetectable viral RNA. Secondary outcomes include the proportion of patients with undetectable viral RNA by day 10 and 14, hospitalization time, duration of fever, and adverse events.ResultsA total of 197 patients completed chloroquine treatment, and 176 patients were included as historical controls. The median time to achieve an undetectable viral RNA was shorter in chloroquine than in non-chloroquine (absolute difference in medians −6.0 days; 95% CI −6.0 to −4.0). The duration of fever is shorter in chloroquine (geometric mean ratio 0.6; 95% CI 0.5 to 0.8). No serious adverse events were observed in the chloroquine group. Patients treated with half dose experienced lower rate of adverse events than with full dose.ConclusionsAlthough randomised trials are needed for further evaluation, this study provides evidence for safety and efficacy of chloroquine in COVID-19 and suggests that chloroquine can be a cost-effective therapy for combating 102 the COVID-19 pandemic.

Objective To explore the clinical efficacy of treating bacterial vaginosis with ornidazole and Dingjunsheng.Methods Retrospectively analyzed the 84 outpatients from our hospital from May,2008 to March,2011,who were diagnosed with bacterial vaginosis and treated at our hospital.Randomly divided them into two groups:treatment group with 42 cases,during the first 6 days,orally took ornidazole,then were treated with Dingjunsheng (lactic acid bacteria viable capsules) for 10 days; and control group with the remaining 42 patients,only took ornidazole for 6 days.Evaluate their clinical efficacies.Results After the treatment,the total effective rate of treatment group was 92.85％,which was significantly higher than that of control group(83.33％); the differences was significantly obvious (P＜ 0.05).During the follow-up,treatment group' s recurrence rate was 0.0％,which was significantly lower than control group' s(11.9％), the difference was significantly obvious (P＜ 0.05).Conclusion Using both omidazole and Dingjunsheng to treat bacterial vaginosis is more effective than using only one of them,which is worth to be generalized in clinical practice.
