Abstract

Diseases, like environment and organic life, change fundamentally. Infectious agent mutations and host alterations create clinical variations which may make descriptions of "classical" clinical findings obsolete a few decades after publication. Rheumatic fever represents a disease in this category; recent studies by Begg and associates1indicate that in many respects acute rheumatic fever is less severe now than it was in the past. These investigators studied 139 adolescent and adult patients with acute rheumatic fever from 1954 to 1961. There were no fatalities, and only 2 patients experienced a severe form of the illness. These observations are in conformity with recent suggestions made in the United States and in Britain that acute rheumatic fever is becoming a milder disease. Bland emphasized this change by noting that "the clinical features of severe rheumatic fever of three and four decades ago, characterized so frequently by nodules, pericarditis, pleuritis, pneumonitis, and congestive heart
