Abstract

Transjugular intrahepatic portasystemic shunts (TIPS) provide a low-resistance pathway between the hepatic veins and the intrahepatic portion of the portal vein for blood to return to the heart in patients with portal hypertension thereby bypassing and decompressing the hepatic sinusoids. The ability to achieve portal venous decompression like a surgical portacaval shunt by TIPS without the attendant risks of general anesthesia and major surgery have led to a resurgence in interest in mechanical portal decompression for the treatment of portal hypertension. While several large controlled studies have examined the role of TIPS in the management of sinusoidal portal hypertension (1, 2, 3), its use for postsinusoidal and especially presinusoidal portal hypertension remain to be fully evaluated. In this issue, Ganger et al.(4) extend the body of anecdotes and small series existing in literature (5, 6, 7, 8, 9) by documenting the efficacy and safety of TIPS for postsinusoidal (Budd-Chiari syndrome) or presinusoidal (portal vein thrombosis) portal hypertension.
