Introduction: Erectile dysfunction (ED) is a common and multifactorial complication after pelvic fracture urethral injury (PFUI), yet objective characterization of ED severity and penile Doppler findings remains limited. We aimed to describe ED severity categories and penile Doppler findings in men with PFUI undergoing anastomotic urethroplasty. Methods: We performed a retrospective analysis of a prospectively maintained database. Among 110 men who underwent anastomotic urethroplasty for PFUI between September 2022 and September 2023, 97 with complete preoperative penile Doppler ultrasound and standardized Sexual Health Inventory for Men assessment were included. Pelvic fractures were classified using the Young–Burgess system. All patients received tadalafil 5 mg daily for 6 months. Postoperative Doppler was not performed. Results: The median age was 29.5 years with a median follow-up of 11 months. Urethroplasty success rate was 85%(83/97). Preoperatively, 56 out of 97 patients had ED (42 mild/moderate; 14 severe). Penile Doppler abnormalities were nearly universal: 93 had cavernosal, and 83 had dorsal artery insufficiency. Postoperatively, 13 out of 41 men developed de novo ED, while 13 out of 56 improved by at least one severity category. Among 76 patients with complete fracture data, greater fracture severity was associated with worse postoperative ED, and worsening erectile function was more frequent among patients with urethroplasty failure. Conclusions: ED and penile Doppler abnormalities are highly prevalent after PFUI. ED severity correlated with pelvic injury severity, supporting the concept that ED reflects global trauma burden rather than isolated vascular pathology. Given the retrospective design and routine postoperative use of PDE5 inhibitors, these findings should be considered descriptive and hypothesis-generating.

Introduction: Pelvic fractures, often from high-energy injuries, can cause pelvic fracture urethral injury (PFUI), especially in males. PFUI can lead to complications like urethral stenosis, erectile dysfunction, and urinary incontinence. These injuries usually occur with signicant trauma, such as vehicular accidents, and are indicated by blood at the meatus, inability to urinate, and a full bladder. Initial management involves suprapubic catheter placement, with delayed urethroplasty after 3-6 months being the gold standard treatment. This approach allows for local healing and successful tension-free anastomosis, reducing the risks of morbidity and complications. Methodology: The study included 75 patients aged 18-61 with pelvic fracture urethral injuries. Excluded were those with inammatory, iatrogenic, congenital, recurrent, or post-radiation strictures. Patients underwent delayed urethroplasty with full bulbar urethra mobilization, end-to-end anastomosis, or additional procedures for tension. Postoperatively, patients had corrugated drains, and catheter removal was based on RUG ndings. Success was dened by effective voiding and no further interventions, while failure included persistent or recurrent strictures. Statistical analysis used chi-square tests. Result: The studyl included 75 patients with pelvic fracture urethral injuries treated with progressive perineal urethroplasty. Procedures performed were urethral mobilization alone (52%), with corporal body separation (32%), and with inferior pubectomy (16%). The overall success rate was 88%, with urethral mobilization alone being the most successful (92.3%). Complications included erectile dysfunction (8%), re-stricture formation (16%), and wound infections (24%). Auxiliary procedures like Optical Internal Urethrotomy were effective for re-stricture management, highlighting the importance of tailored surgical approaches. Conclusion: Road trafc accidents were the primary cause of pelvic fracture urethral injuries. Perineal urethroplasty, the gold standard, shows excellent results. Key factors for success include maintaining urethral vascularity, meticulous scar tissue excision, and tension-free anastomosis. Achieving this may require distal urethral mobilization, crural separation, and occasionally inferior pubectomy. Recent approaches minimize steps, avoiding supracrural urethral rerouting. High-quality research is needed to improve diagnosis, management, and follow-up of urethral strictures.

V13-12 BREAKING BOUNDARIES IN URETHRAL STRICTURE MANAGEMENT: A PIONEERING SIMULTANEOUS APPROACH TO ANTERIOR AND POSTERIOR

Ischemic necrosis of the bulbar urethra in a patient with pelvic fracture urethral injury without a prior history of surgical intervention is extremely rare and results in long-segment obliterative strictures that are difficult to manage. Instead of the more traditional approach of vascular reconstruction followed by transpubic end-to-end urethroplasty, these patients are better managed by up-front urethroplasty with a tubed flap or as a staged procedure with grafting and tubularization. Herein, we report a case of primary bulbar urethral ischemic necrosis due to pelvic fracture managed with tubularized preputial flap (McAninch flap) urethroplasty.

Outcomes of Endoscopic Realignment of Pelvic Fracture Associated Urethral Injuries at a Level 1 Trauma Center

MP78-12 OUR EXPERIENCE IN PENILE IMPLANT AFTER ANASTOMOTIC URETHROPLASTY FOR PELVIC FRACTURE URETHRAL INJURY

ObjectiveUrethral stenosis caused by pelvic fracture urethral injury (PFUI) is a complex urological disease, especially for the redo cased. However, to find the proximal end of the posterior urethra, and to avoid injury to the rectum and to forecast to remove the inferior pubic margin are two key points for a successful surgery. These steps can be challenging for even the most experienced urologists. This study is to describe a new technique for understanding the three-dimensional (3D) anatomy of the urethra, which will also aid in surgical planning and simplify urethroplasty.Materials and methodsThree patients underwent routine urethroscopy, X ray urethrography and contrast CT urethrography. The 3D images were then reconstructed, and the data were transmitted to a 3D printer. 3D models were printed with polyacrylic acid to simulate the anatomical structure and relationship of urethral stenosis with pubic symphysis and rectum. Various diagnosis methods were compared with the condition in surgery. The patients and trainee questionnaires were performed.ResultsThree models of urethral CT were obtained. These models were presented to patients and trainee doctors along with routine urethroscopy, urethrography, and urethral CT. The scores of patients and trainee question forms demonstrated that the 3D printed urethral stenosis model of pelvic fracture has obvious advantages in urethral adjacency and ease of understanding. The 3D printed urethras were easy to show the pubic symphysis and simulate its excision and exposure of urethra. The model could show the precise distance from urethra to rectum to prevent the rectum injury in surgery.Conclusions3D printing technology can be applied to the preoperative evaluation of urethral stenosis caused by PFUI. It can be auxiliary to understand the anatomical structure of the posterior urethra, the direction of urethral displacement, protecting the rectum and the forecasting for pubectomy. It is especially helpful for the accurate preoperative planning of some complex urethral stenosis and redo cases.

Pelvic fracture causes urethral injury in about 10% of patients. The etiology of injury is different in developing and developed world. While high-velocity automobile accidents are common in developed countries, where patients are in the car and there is usually a side impact, in the developing world, significant number of injuries are caused by two-wheeler accidents, pedestrians, farming accidents, fall from height, fall from tractor, fall from tree, and other causes like earthquake. We share our experience which is the largest in the reported literature. In our tertiary referral center, we get referrals from all across the globe. Since 1995 till 2018 we have performed 1307 surgeries for Pelvic fracture urethral injury. Our referrals are for complex urethroplasty. Our data from 1995-2018 was analyzed. Data after June 2018 was not included so as to have a minimum follow up of 6 months. 1296 patients were males and 11 were females. In the group of 1296 males, mean age was 32.4years (range 1-79years). The minimum follow-up was 6months, and the median follow-up was 56.7months (range 6.2-233.7). The overall success rate was 88.79% for primary cases while re-do urethroplasty patients had a success rate of 83.70%. The majority of our patients (more than 61.40%) needed inferior pubectomy: Of the total 1307 cases of urethroplasty for pelvic fracture urethral injury data was available for 1042 patients. Data were available for 1042 patients. The data from 2012 onwards were prospectively analyzed while the previous data were retrospectively analyzed. PFUI are common in the developing world. They tend to be more complex and have longer gaps as compared to developed world. This could be related to the anthropometric differences between races as well as nature of injury. They are best managed with delayed transperineal repair with excellent outcomes. Ancillary maneuvers are more frequently required. Re-do urethroplasty can achieve good results.

To review systematically the literature on female urethral injuries associated with pelvic fracture and to determine the optimum management of this rare injury. Using Meta-analysis of Observational Studies in Epidemiology criteria, we searched the Cochrane, Pubmed and OVID databases for all articles available before 30 June 2016 using the terms 'female pelvic fracture urethroplasty', 'female urethral distraction', 'female pelvic fracture urethral injury' and 'female pelvic fracture urethra girls.' Two authors of this paper independently reviewed the titles, abstracts, and articles in duplicate. We identified 162 individual articles from the databases. Fifty-one articles met our criteria for full review, including 158 female patients with urethral trauma. Of these injuries, 83 (53%) were managed with immediate repair; 17/83 (20%) via primary alignment and 66/83 (80%) via anastomotic repair. The remaining 75/158 (47%) were managed with delayed repair. Rates of urethral stenosis and fistula were highest after primary alignment. Urethral integrity appears to be similar after both primary anastomosis and delayed repair; however, patients experienced significantly more incontinence and vaginal stenosis after delayed repair. Patients who underwent delayed urethral repair were more likely to undergo more extensive reconstructive surgery than those who underwent primary repair. The optimum management of female urethral distraction defects is based on very-low-quality literature. Based on our review of the available literature, primary anastomotic repair of a female urethral distraction defect via a vaginal approach as soon as the patient is haemodynamically stable appears to be optimal.

PurposeTo report the clinical features of pelvic fracture urethral injury (PFUI) and assess the real effect of factors that are believed to have adverse effects on delayed urethroplasty.Methods An observational descriptive study in a single urological center examined 376 male patients diagnosed with PFUI who underwent open urethroplasty from 2009 to 2013. Analyzed factors included patient age at the time of injury, etiology of PFUI, type of emergency treatment, concomitant injuries, length and position of stricture, type of urethroplasty and the outcome of surgery. Univariate and multivariate logistic regression analyses were applied, together with analytical statistic methods such as t test and Chi-square test.ResultsThe overall success rate of delayed urethroplasty was 80.6 %. Early realignment was associated with reduced stricture length and had beneficial effect on delayed surgery. Concomitant rectum rupture, strictures longer than 1.6 cm and strictures closer than 3 cm to the bladder neck were indicators of poor outcome. Age, type of injury, urethral fistula and bladder rupture were not significant predicators of surgery outcome. Failed direct vision internal urethrotomy and urethroplasty had no significant influence on salvage operation.ConclusionsThe outcome of posterior urethroplasty is affected by multiple factors. Early realignment has beneficial effect; while the length and position of stricture and its distance to bladder neck plays the key role, rectum rupture at the time of injury is also an indicator of poor outcome. The effect of other factors seems insignificant.

Bulbar urethral ischemic necrosis (BUIN) is an iatrogenic entity resulting from repeated attempts at performing anastomotic urethroplasty for pelvic fracture urethral injuries. Etiologically speaking, BUIN is related to a compromised blood supply of the bulbar urethra, which normally relies on anterograde supply from bulbar arteries and retrograde supply from recurrent branches of dorsal penile arteries, through the glans. At each transection of the bulbar urethra, both the anterograde and retrograde supplies are compromised, increasing the risk of BUIN. Even though this term is widely used among reconstructive urologists, BUIN is orphan of an accepted scientific definition. We aim to report our personal perspective on BUIN, to identify factors associated with its occurrence, and to describe the management options in these patients.
