BHUTANESE COUNSELORS' EXPERIENCES WITH WESTERN COUNSELING: A QUALITATIVE STUDY Susan V. Lester Old Dominion University, 2015 Director: Dr. Radha J. Horton-Parker Following centuries of independence from the rest of the world, the remote and fastdeveloping Kingdom of Bhutan has recently opened its doors to outside influence. Bhutan has invited the profession of counseling to aid in responding to growing mental health problems, social and family issues, and school and career guidance needs. This study employed the methods of phenomenology to investigate the experiences of Bhutanese counselors with Western counseling in order to understand the intersection of the culture of Bhutan with the culture of counseling. Eleven participants were interviewed and four themes were identified: Conceptualization of counseling, Bhutanese culture, Bhutanese counseling, and the relationship between Western counseling and Bhutan. The worldview differences between collectivism and individualism are highlighted as well as the compatibility and integration of Buddhist principles with counseling. Elements of a potential model of Bhutanese counseling are suggested as well as recommendations for counselor training, ongoing Western contributions, and the further development of the profession.

This study examined counselor-client concurrence and variation on working alliance and session impact in cybercounseling and face-to-face counseling conditions. Counselors were three second-year graduate students in a counseling practicum who provided 50-minute sessions to clients through cybercounseling and face-to-face counseling. After each session, counselors and clients independently completed the Working Alliance Inventory (WAI) and Session Impact Questionnaire (SEQ) to assess the strength of working alliance and impact of counseling session. Transcripts from both cybercounseling and face-to-face counseling sessions were analyzed using the helping skill system developed by Hill and O’Brien (1999). Results indicated the following: In cybercounseling condition, counselors used open-ended and close-ended questions the most, and challenging the least. In face-to-face counseling condition, counselors used close-ended questions and restatement the most. Working alliance has an increasing trend both cypbercounseling and face-to-face counseling conditions, while session impact (coherence, depth, & positivity) varied according to the number of sessions.

Background: Home-based HIV counselling and testing (HBHCT) is highly acceptable\nand may be an effective strategy for HIV prevention and population-based test-and-treat\nprogrammes in sub-Saharan Africa (SSA). However, few data are available on linkage\nto care or on the effectiveness of strategies to increase linkage to care among HIVpositive\npersons identified through HBHCT in SSA. The aims of this PhD were to (i)\nsystematically review the literature on linkage to care among HIV-positive adults\ndiagnosed through HBHCT in SSA; and (ii) to conduct a cluster-randomised controlled\ntrial to measure the effectiveness of a counselling intervention after HIV diagnosis\nthrough HBHCT in increasing linkage to care in rural Masaka district, Uganda.\nMethods: (i) Five databases (Medline, Embase, Global Health, Web of Science, and\nAfrica-Wide information) were systematically searched for studies published between\n1st January 2000 and 19th August 2016. Authors of studies for which some required\ninformation was missing were requested to provide additional data. (ii) For the trial, 28\nrural communities were randomly allocated (1:1) to the intervention (HBHCT, referral,\nand brief home-based counselling sessions one and two months after HBHCT) or\ncontrol group (HBHCT and referral only). HIV-positive adults (≥18 years) not yet in\ncare were enrolled. Primary outcomes were linkage (registration with an HIV clinic) at\n6 months after HBHCT, and time to linkage. Analyses were by intention-to-treat using\nrandom effects logistic regression and Cox regression with shared frailty.\nResults: (i) 19 eligible studies were identified; one had all the required data. Additional\ndata were obtained for 13 studies; thus, 14 studies were included in the review. Linkage\nto care was generally lower (<33%) if HBHCT was followed by referral only, and\nhigher (>80%) if some strategy was used to facilitate uptake of referral. Only one study\nwas a randomised trial and most were susceptible to outcome ascertainment bias. (ii) In\nthe trial, 551 individuals tested HIV-positive; 205 (37.2%) were already in care and thus\nineligible. 302 (87.3% of those eligible) were enrolled (intervention, n=149). Retention\nwas similar across trial arms (92% overall). Overall linkage to care was 42.1%.\nCounselling was associated with a 2.18-fold [95% confidence interval (CI)=1.26-3.78]\nincrease in the odds of linkage. There was no evidence of a difference between arms in\nthe rate of linkage in the first two months, but subsequently the rate of linkage was\nhigher in the intervention arm (hazard ratio=4.87, 95% CI=1.79-13.27).\nConclusion: Counselling substantially increases linkage to care among HIV-positive\nadults identified through HBHCT and can increase antiretroviral therapy coverage in\nSSA.

The samples were 1,114 grade 1 - 3 Thai adolescent students selected for the autonomy component study. Subsequently, eight students as an experimental group and eight students as a control group were randomly selected from 24 Sansai Wittayakom School students with total autonomy scores at 25 th percentile and lower. A Confirmatory Factor Analysis verified that the autonomy functioning model was consistent with the empirical data and characterized into seven high loading factors (p < .05); namely, self-efficacy, self-regulation, self-reliance, self-confidence, self-responsibility, self-assertiveness and self-evaluation. Each factor covered four areas: academic performance, personal care, peer relationship and parental/guardian connectedness. A three- stage group counseling, the initial stage, the transition and working stage, and the final stage, was constructed by integrating counseling theories and techniques to enhance the students’ autonomy. Statistically significant differences (p < .05) in the total and each autonomy component of the experimental group existed before, after counseling and after the follow up period. Moreover, statistically significant differences (p < .05) in the total and each autonomy component between the experimental group and the control group existed before, after counseling and after the follow up period.

This research project aims to explore the perceived experience of planning and chance in the career experience of secondary students as they graduate from school and move in to post school pathways and the utility of chance related intervention in high school career education. To acknowledge a relationship between chance and planning in career paths will involve a shift from traditional career counselling, which for much of the past hundred years had been fairly linear, to embracing nonlinear models. One of the cornerstones that career counselling has been built on is that career development is linear and rational (Guindon and Hanna, 2002). Various authors and studies have suggested that we live in a complex world resulting in careers that are not always linear (Baumgardner 1976; Salomone and Slaney 1981; Scott and Hatalla 1990; Betsworth and Hansen 1996; Pryor and Bright 2003) and that in the interest of better serving our clients we need to explore ways to incorporate this into career counselling. Krumboltz (1998) proposes that we need to reconsider our career counselling approach to include the significance of unexpected change. The author does not claim to discredit traditional approaches to career counselling but to investigate supplementing these with an approach that includes both planning and chance. This thesis will investigate existing literature on the role of unplanned events and careers as well as carry out research with secondary to post-secondary students to establish firstly, the perceived experience of unplanned events in their career paths and secondly, the utility of chance related career counselling intervention. The research will take place with students from a comprehensive high school of approximately 750 students in New South Wales from a “realist constructivist” epistemology (Robson 2002). Two groups of studies are proposed. The first studies will involve a phone interview eighteen months after participants have completed high school which will compare their current career pathway with the one planned in school. The second group of studies will explore the utility of chance related career lessons compared to more traditional Trait and factor matching in high school career education settings.

The development of smart devices and the products of ICT are becoming a medium for verbal abuse, causing serious problems in bank counseling. Research on verbal abuse in the bank counseling process started at call centers, and as the damage caused by verbal abuse is known, a lot of research were rapidly conducted. These include the understanding of verbal violence and the closest group in which verbal violence can occur over a long period of time and the horizontal and vertical forms of verbal violence. There are also multilateral studies to identify the causes of verbal violence and to reduce verbal violence. The scope of verbal violence also varies with definition, phenomenon, type, cause, effect, and factor. In the early days of the study of verbal violence, definitions and types were central. This study focuses on analyzing the relationship of the impact on professional self-concept through psychological exhaustion of verbal violence experiences during bank counseling. Verbal violence is categorized as violence against people who are not related to them and those who are related to them. Verbal violence against irrelevant people is an impromptu violence situation between the perpetrator and the victim during various counseling processes. It refers to violence committed against others during counseling through cyberspace and various media. Personal violence in the counseling process tends to be neglected because the substance is not clear, but the damage is continuing and increasing. Although various psychological studies have been conducted on real violence, there are no studies related to psychological exhaustion and professional self-concept. The factors that determine the verbal violence experience are the violence experience, the violence victim experience, and the violent attitude. The collected data were analyzed by Smart PLS 2.0 using structural equations. The summary of the study is as follows. First, the experience of violence had a significant impact on psychological exhaustion. Second, the experience of violence damage had a significant impact on psychological exhaustion. Third, violent personality had a significant effect on psychological exhaustion. Fourth, psychological exhaustion had a significant impact on professional self-concepts. To solve the problem of overcoming professional self-concepts, differentiated efforts in the experience of violence, violence victimization, and violent attitude are needed in consideration of verbal violence experiences. Related organizations seeking to address verbal violence need to study various opinions on the causes of verbal violence behavior.
