Abstract

The incidence of the abnormally invasive placenta (AIP) is increasing due to rising rates of Caesarean sections (CS). Anticipation is the key in the early antenatal diagnosis of AIP. Hence our centre structured a protocol to evaluate placenta previa and low lying placenta, in patients of previous CS or uterine surgery, irrespective of indication for the scan. Accordingly, we evaluate the placental morphology, thinning of the retroplacental myometrium, abnormal retroplacental blood flow, disruption of echogenic serosal line and focal irregularity of bladder wall. We also evaluate previous Caesarean scar. We present our first trimester data. Out of 1193 patients referred for NT scan, between January 2017 to December 2018, 298 were with previous CS. Following the protocols, we could suspect 4 cases with AIP. These cases were followed at anomaly scan and at growth scan and reassessed for the extent of placental invasion. The diagnosis was confirmed on CS, in all cases. Thus Protocol-based evaluation in a patient with previous uterine surgery can help in the accurate prediction of AIP in the first trimester.
