Future Needs for Long-Term Monitoring Design

BackgroundProviding timely, adequate and appropriately-resourced care to people experiencing their first episode of psychosis needs to be informed by evidence-based models of future need in the population. We sought to develop a validated prediction model of need for provision of early intervention in psychosis [EIP] services at the small area level in England up to 2025, based on current epidemiological evidence and demographic projections of the at-risk population.MethodsWe developed a Bayesian population-level prediction tool. First, we obtained small area incidence data on first episode psychoses, aged 16–64 years, from three major empirical studies of psychosis risk (ÆSOP, ELFEP and SEPEA). Second, we identified suitable prior information from the published literature on variation in psychosis risk by age, sex, ethnicity, deprivation and cannabis use. Third, we combined this empirical data with prior beliefs in six Bayesian Poisson regression models to obtain a full characterisation of the underlying uncertainty in the form of suitable posterior distributions for the relative risks for different permutations of covariate data. Fourth, model coefficients were applied to population projections for 2017 to predict the expected incidence of psychotic disorders, aggregated to Commissioning Group [CCG] and national levels. Fifth, we compared these predictions to observed national FEP data from the NHS Mental Health Services Data Set in 2017 to establish the most valid model. Sixth, we used the best-fitting model to predict three nested indicators of need for psychosis care: (i) total annual referrals to early intervention in psychosis [EIP] for “suspected” FEP (ii) total annual cases accepted onto EIP service caseloads, and (iii) total annual new cases of probable FEP in England up until 2025, using small area population projections.ResultsA model with an age-sex interaction, ethnicity, small area-level deprivation, social fragmentation and regional cannabis use provided best internal and apparent validity, predicting 8112 (95% Credible Interval 7623 to 8597) individuals with FEP in England in 2017, compared with 8038 observed cases (difference: n=74; 0.94%). Apparent validity was acceptable at CCG level, and by sex and ethnicity, although we observed greater-than-expected need before 35 years old. Predicted new referrals, caseloads and probable incidences of FEP rose over the forecast period by 6.2% to 25,782, 23,187 and 9,541 new cases in 2025, respectively.DiscussionOur translational epidemiological tool provides an accurate, validated method to inform planners, commissioners and providers about future population need for psychosis care at different stages of the referral pathway, based on individual and small area level determinants of need. Such tools can be used to underpin evidence-based decision-making in public mental health and resource allocation in mental health systems.

Assessment of the current computer literacy and future computer needs of emergency medicine residents and faculty

The present status and future needs of four coral reef sites (Taba, Nuweiba , Dahab and Sharm El-Sheikh) along the Gulf of Aqaba, Egypt were evaluated. A total of 60 quadrats, distributed at different depths or different patches were surveyed at each site during the year 2007. Three mooring buoys were reported only in Ras Umm Seid but nothing was found in other sites, however excessive boating and diving pressure were recorded only in Ras Umm Seid. Highest amount of garbage was recorded in Nuweiba, patrolling efficiency was best in Taba but no patrolling activities were re- corded in other sites. Ras Umm Seid (Sharm El-Sheikh) had the highest amount of dead corals (13%) due to increased boating and diving pressure compared to other sites, however Dahab had the lowest amount of dead corals (4%) due to the rough conditions (strong wind and waves) causing difficulties in accessing that site. Nuweiba and Taba had high percent cover of sands (10% and 11%, respectively) playing a key role in the considerable amount of dead corals in both sites. Lowest % new coral recruits in Taba (0.5%) was associated with the soft sandy bottom, the opposite was true in Sharm El Sheikh due to the rocky bottom. Future needs of the studied sites include decreasing the number of divers to the diver car- rying capacity (DCC), repair of existing buoys and incorporation of 5 more buoys in Ras Umm Seid. Other sites have no future needs in these regards as they are far beyond the DCC. Nuweiba needs first of all to be cleaned, then being pro- tected for giving the chance of recovery. Except for Taba which has efficient patrolling, the other 3 sites need efficient ter- restrial and marine patrolling as well as enforcement of EEAA regulations. However reef access points which were absent in all sites need to be established to minimize reef damage on accessing deeper water. Also, the environmental awareness field signs which were absent in all sites need to be established and managed. Taba is recommended as a protected area as it has the food for threatened species beside being useful as a nesting site. Ras Umm Seid is recommended to have a estab- lished artificial reef to draw the attention of divers and snorklers from the natural reefs and prepare artificial shelters for fishes thus promoting the fish resources.

Ductus diameter and left pulmonary artery end-diastolic velocity at 3 days of age predict the future need for surgical closure of patent ductus arteriosus in preterm infants: A post-hoc analysis of a prospective multicenter study

The objective of this study was to identify biomarkers that predict a future need for anti-VEGF therapy in diabetic retinopathy (DR). Eyes with DR that underwent ultra-widefield angiography (UWFA) and had at least a 1 year follow-up were grouped based on future anti-VEGF treatment requirements: (1) not requiring treatment, (2) immediate treatment (within 3 months of UWFA), and (3) delayed treatment (after 3 months of UWFA). Quantitative UWFA features and clinical factors were evaluated. Random forest models were built to differentiate eyes requiring immediate and delayed treatment from the eyes not requiring treatment. A total of 173 eyes were included. The mean follow-up was 22 (range: 11–43) months. The macular leakage index, panretinal leakage index, presence of DME, and visual acuity were significantly different in eyes requiring immediate (n = 38) and delayed (n = 34) treatment compared to eyes not requiring treatment (n = 101). Random forest model differentiating eyes requiring immediate treatment from eyes not requiring treatment demonstrated an AUC of 0.91 ± 0.07. Quantitative angiographic features have potential as important predictive biomarkers of a future need for anti-VEGF therapy in DR and may serve to guide the frequency of a follow-up.

RESUMENO monitoramento biológico é uma ferramenta fundamental para o manejo ambiental. Ele permite avaliar o estado do ambiente ao longo do tempo, informando sobre a necessidade e sobre as conseqüências das medidas de manejo. Na Amazônia, o monitoramento é uma atividade particularmente difícil devido à variedade das ameaças ambientais, à vastidão da área, e à enorme riqueza de espécies. Para ser eficiente, um programa de monitoramento precisa ser claramente enraizado em uma ameaça ambiental que se pretende resolver – só assim haverá um critério claro para decidir quais variáveis serão medidas e como planejar essa medição. Numa área tão grande quanto a Amazônia, é particularmente importante que os programas de monitoramento definam a priori a sua escala de atuação. Um desenho amostral que serve para a escala local será deficiente na escala regional, e vice‐versa. A idéia de trabalhar simultaneamente em diferentes escalas espaciais é louvável mas altamente arriscada. Finalmente, quando o monitoramento requer identificação de espécies, a riqueza biológica da Amazônia e a escassez de pessoal qualificado limitam o avanço do trabalho. Para garantir a viabilidade futura das atividades de monitoramento é essencial investir com criatividade e determinação no treinamento de observadores. Este comentário sublinha alguns aspetos de progresso recente em monitoramento biológico da Amazônia e identifica três necessidades para o futuro: 1) Garantir a orientação do monitoramento para ameaças claramente identificadas; 2) Trabalhar em uma escala espacial prioritária, definida antes do lançamento do programa; e 3) Investir no treinamento de observadores.El monitoreo biológico es una herramienta fundamental para la gestión ambiental porque permite evaluar el estado del ambiente al largo del tiempo, informando sobre la necesidad e sobre las consecuencias de acciones de gestión. En Amazonia, el monitoreo es una actividad particularmente difícil gracias à la variedad de amenazas, a la extensión de la área, y a la enorme riqueza de especies. Para ser eficiente, un programa de monitoreo necesita ser claramente enraizado en una amenaza ambiental que se pretende resolver – solo así existirá un criterio claro para decidir lo que medir y como medirlo. En una área tan grande como Amazonia, se hace esencial que los programas de monitoreo establezcan a priori su escala de actuación. Un diseño de muestreo que funciona en la escala local será deficiente en la escala regional y vice‐versa. La idea de trabajar simultáneamente en distintas escalas espaciales es muy arriesgada – hay que priorizar. Finalmente, cuando el monitoreo exige identificación de especies, la riqueza de Amazonia y la escasez de personal calificado limitan el progreso del trabajo. Para garantizar la viabilidad del monitoreo, se hace esencial invertir con creatividad y determinación en el entrenamiento de observadores. Este comentario refiere algunos progresos recientes en el monitoreo biológico de Amazonia y identifica tres necesidades para el futuro: 1) Garantizar que cada programa esté claramente orientado para una amenaza ambiental; 2) Trabajar en una escala espacial prioritária; y 3) Invertir en el entrenamiento de observadores.

This study investigates the need for incorporating sustainability into corporate governance structures. This research identifies lessons and practice through the consideration of global sustainability challenges, business imperatives, as well as good practices provided by companies successfully leveraging effective governance. Both challenges and limitations such as implementation barriers being stood point of measurements are emphasized with necessity for an enduring governance transformation. Implications for theory and practice highlight the significance of leadership commitment, stakeholder inclusion in engagement, transparent report shows how important to incorporate sustainable business practices. The sustainable governance transformation is a call to action for policy makers, organizations and key stakeholders that collaboratively implement constructive pathways towards environmental sustainability.

One of the major problems that has plagued Dutch society since the Second World War is the persistent housing shortage. At the end of the 1980s, this shortage was thought to have been solved. Therefore it came as a shock when the Ministerie voor Volkshuisvesting, Ruimtelijke Ordening en Milieubeheer (Ministerie VROM: Ministry of Housing, Physical Planning and Environment) published its 'Vierde Nota over de Ruimtelijke Ordening' (Fourth Report on Physical Planning) announcing an additional housing need of approximately one million dwellings in the Randstad between 1988 and 2015 (VROM, 1988). This figure was based on population forecasts and prognoses of future individual housing need. The Stichting Natuur en Milieu (Nature and Environment Foundation), which advocates the preservation of the 'Green Heart' area that lies in the middle of the Randstad, commissioned us to 'test' the reliability of the figures on future housing need presented in the Fourth Report. We calculated future housing need on the basis of the same population forecasts but other assumptions concerning individual housing needs (headship rates) (De Gans and Oskamp, 1990a). Whereas the national planning agency assumes continuing trends of individuation, we explore the possibilities of a shift in these processes. This article identifies signs that such a shift could indeed occur. We do not assert that the shift will definitely take place; in the short run, the odds are against this. But we posit that such a shift is more likely than is generally thought. This article does not set out to prove the forecasters of the national planning agency wrong. Rather, we intend to show that the future may be different than they lead us to expect.

Chapter 13 - Current Challenges and Future Needs in Biomaterials and Devices for Bone Disorders

The study revealed a thriving Nigerian food industry and policymakers toward understanding food traceability systems in three ways. Firstly, a realistic strategy toward supply chain visibility. Secondly, verify the concept relating to the implementation of documentation and technological innovations. Thirdly, emphasized the significance of food safety and food recall taking into account current status and future needs and how a traceability system would be realized in the poultry sector by giving a clear roadmap for technology. Also the work explained the current state of the two most practiced method of poultry production; backyard and commercial scale was also investigated.

Dengue fever, dengue haemorrhagic fever, and dengue shock syndrome (DF/DHF/DSS) are tropical diseases that cause significant humanitarian and economic hardship. It is estimated that more than 2.5 billion people are at risk of infection and more than 100 countries have endemic dengue virus transmission. Laboratory tests are essential to provide an accurate diagnosis of dengue virus infection so that appropriate treatment and patient management may be administered. In many dengue endemic settings, laboratory diagnostic resources are limited and simple rapid diagnostic tests (RDTs) provide opportunities for point-of-care diagnosis. This paper addresses current issues relating to the application of commercial dengue RDTs for the diagnosis of acute dengue virus infection, recent diagnostic evaluations, and identifies future needs.

Developing countries, despite being the major suppliers of maritime manpower, lag in their future-proof competence development. This study explores whether the current maritime education and training (MET) practices for marine engineering education in a developing nation are perceived as sufficient by the early-stage marine engineers. The current competence requirements set by the Standards of Training, Certification and Watchkeeping for Seafarers (STCW’74 as amended) are compared and contrasted against the current MET practices as well as the future needs for remotely controlled autonomous ship operations. Bangladesh, a maritime nation in the Southeast Asia region was selected as a developing nation for this study. An online survey was conducted among early-stage professional marine engineers. The analysis of 62 valid responses revealed that current MET practices are significantly perceived to be satisfactory for acquiring non-technical competencies, such as being proficient in the English language, but are less satisfactory for instilling technical competencies such as of electrical and electronics, knowledge regarding seaworthiness of ships, as well as medical first aid. In addition, the findings highlight the perceived gaps in existing marine engineering education as well as the need for future studies focused on re-training the future maritime workforce that could potentially improve MET strategies and practices in the developing nations.

Over the last decades, there has been a movement towards higher degree of recognition and involvement of Indigenous knowledge and perspectives worldwide. An integral and defining part of this development is the work of scholars (and activists) from all over the Indigenous worlds claiming the necessity of decolonization/Indigenization and the recognition of Indigenous rights. Today, this struggle is not over. But change has happened. Indigenous research is a growing field. The ethics and formalities of publication and data management are addressed in ways that aim to answer to the needs of Indigenous communities. This panel takes the current situation and diversity as a starting point and looks to the future needs and challenges, and asks how can we continue working for research practices that are FAIR and include CARE? How can we learn across different Indigenous contexts and areas? The panelists represent research communities from different parts of the global north with its variety of Indigenous cultures.

In order to address serious concerns over public health, water scarcity and groundwater pollution in Jordan, the expansion of decentralized wastewater treatment and reuse (DWWT&R) systems to small communities is one of the goals defined by the Jordan government in the "Water Strategy 2009-2022". This paper evaluates the general potential of decentralized wastewater system solutions to be applied in a selected area of the Lower Jordan Rift Valley in Jordan. For the study area, the connection degree to sewer systems was calculated as 67% (5% in the rural sector and 75% in the urban sector). The annual wastewater production available for DWWT&R in the rural sector of the investigation area was calculated to be nearly 3.8 million m(3) at the end of 2007. The future need of wastewater treatment and reuse facilities of the rural sector was estimated to be increasing by 0.11 million m(3) year(-1), with an overall potential of new treatment capacity of nearly 15,500 population equivalents (pe) year(-1). The overall potential for implementing DWWT&R systems in the urban sector was estimated as nearly 25 million m(3) of wastewater in 2007. The future need of wastewater treatment and reuse facilities required for the urban sector was estimated to be increasing at a rate of 0.12 million pe year(-1). Together with the decision makers and the stakeholders, a potential map with three regions has been defined: Region 1 with existing central wastewater infrastructure, Region 2 with already planned central infrastructure and Region 3 with the highest potential for implementing DWWT&R systems.

This study describes what training programs in pediatric dentistry and dental anesthesiology are doing to meet future needs for deep sedation/general anesthesia services required for pediatric dentistry. Residency directors from 10 dental anesthesiology training programs in North America and 79 directors from pediatric dentistry training programs in North America were asked to answer an 18-item and 22-item online survey, respectively, through an online survey tool. The response rate for the 10 anesthesiology training program directors was 9 of 10 or 90%. The response rate for the 79 pediatric dentistry training program directors was 46 of 79 or 58%. Thirty-seven percent of pediatric dentistry programs use clinic-based deep sedation/general anesthesia for dental treatment in addition to hospital-based deep sedation/general anesthesia. Eighty-eight percent of those programs use dentist anesthesiologists for administration of deep sedation/general anesthesia in a clinic-based setting. Pediatric dentistry residency directors perceive a future change in the need for deep sedation/general anesthesia services provided by dentist anesthesiologists to pediatric dentists: 64% anticipate an increase in need for dentist anesthesiologist services, while 36% anticipate no change. Dental anesthesiology directors compared to 2, 5, and 10 years ago have seen an increase in the requests for dentist anesthesiologist services by pediatric dentists reported by 56% of respondents (past 2 years), 63% of respondents (past 5 years), and 88% of respondents (past 10 years), respectively. Predicting the future need of dentist anesthesiologists is an uncertain task, but these results show pediatric dentistry directors and dental anesthesiology directors are considering the need, and they recognize a trend of increased need for dentist anesthesiologist services over the past decade.
