Giant maxillary radicular cyst: Case report

The most prevalent cystic lesions in the jaws are radicular cysts. They are made up of epithelial remains that multiply after pulp necrosis due to inflammation. Radicular cysts rarely grow to substantial sizes, while typically being tiny and asymptomatic. When localized, they are treated with root canal treatment; when large, they are treated with surgical procedures. A 28-year-old systemically healthy male patient was admitted to our clinic due to extraoral swelling in the right maxillary posterior region. Radiographic examination revealed a well-defined lesion localized in the right maxillary region, extending into the nasal cavity and orbital floor, including the maxillary sinus. The lesion, in which cyst fluid was observed as a result of aspiration biopsy, was enucleated under general anesthesia and an excisional biopsy was performed. The biopsy confirmed the preliminary diagnosis of the radicular cyst. Radicular cysts seen in the maxillary region may resorb adjacent anatomical structures and develop into the sinus. Such lesions should be diagnosed before they reach large sizes and the causative teeth should be treated.

Radicular or periapical cysts are one of the most commonly occurring odontogenic cysts of the jaws. The present article reported a case of a 12-year-old female with the chief complaint of swelling and pain on the right lower back tooth region. The radiographic examination revealed the presence of a well-defined radiolucency surrounded by a corticated border with respect to the right mandibular 1st, 2nd and 3rd molar. The case was managed by complete enucleation under general extraction with the extraction of right mandibular 1st, 2nd and 3rd molar. The success of the surgery was apparent by the uneventful healing during the follow-up period and evidence of complete healing after 1-month follow-up. Early diagnosis of the lesion would have lead to a less aggressive treatment plan.

Odontogenic cysts of the jaws are one of the most common tumour-like neoplasms of the maxillofacial region in children. Today, there are various methods of treating cysts, but in childhood, the problem of choosing a treatment method is relevant and at the same time difficult, as some authors prefer cystectomy, while others prefer cystotomy. Purpose - to analyze the methods of treatment and management of jaw cysts in children depending on the type of cyst. Materials and methods. A retrospective analysis of 286 case histories of patients with jaw cysts aged 4 to 17 years was performed. The case histories were analysed according to the developed examination chart. Results. A retrospective analysis of 286 case histories of children with jaw cysts was performed. The age of the children ranged from 4 to 18 years. Radicular dentoalveolar cysts were diagnosed in 35% (n=100) of cases, radicular cysts - in 36% (n=104), follicular cysts - in 15% (n=43), suppurative cysts - in 5% (n=14), residual cysts - in 5% (n=14), primary bone cysts - in 3% (n=7), polycystic cysts - in 2% (n=4). Cystotomy was performed in the vast majority of cases - 58% (n=166), cystectomy - in 42% (n=120). Tamponade was performed in 63% (n=181) of cases, and in 37% (n=105) it was not performed. Conclusions. Radicular cysts are the most common odontogenic cysts of the jaws. Cystotomy and cystectomy are variable methods of cyst treatment. The most common method of treating cysts in children is cystotomy. The research was carried out in accordance with the principles of the Helsinki Declaration. The study protocol was approved by the Local Ethics Committee of the participating institution. The informed consent of the patient was obtained for conducting the studies. No conflict of interests was declared by the authors.

Radicular cyst is the second most common odontogenic cyst. Most of these lesions involve the apex of offending tooth and appear as well-defined radiolucencies. When asymptomatic, a cyst can acquire a large size, even extending into anatomic cavities. The present case reports an extensive radicular cyst extending into the maxillary sinus that could have been easily mistaken for just a displaced tooth. Conventional radiographs show two-dimensional images of three-dimensional objects. Cone-beam computed tomography provides undistorted three-dimensional information of hard tissues, gives adequate spatial resolution, and recently seen as an expansion in diagnosis and management of periapical lesions. Prompt diagnosis and treatment of radicular cyst were successful with endodontic treatment and surgical management.

Radicular cysts and Chin implants; An unexpected complication prompting explantation – Case report

The radicular cyst is one of the most common odontogenic cysts in dentistry, with a higher incidence occurring in the anterior portion of the maxilla. According to the literature, its characteristics are described as a pathological cavity containing citrine fluid, lined by odontogenic epithelium, and externally limited by a fibrous capsule. Case Report: A 31-year-old male patient presented at the Oral and Maxillofacial Surgery (OMF) of the University of Passo Fundo (UPF) with edema on the left side of the face. After clinical examination and vitality tests yielded negative results for teeth 21 and 22, and imaging tests revealed the presence of a unilocular radiolucent lesion with irregular borders in the region, the patient was referred for endodontic treatment of these teeth. During the endodontic treatment, citrine fluid leakage was observed, leading to the suspicion of a cystic lesion. For the treatment of this lesion, endodontic treatment of tooth 23 was initially performed as it was also compromised, followed by enucleation and curettage. Additionally, an apicoectomy was necessary for the affected teeth. The specimen was sent for histopathological examination, which confirmed the diagnosis of a radicular cyst. After 7 days, the patient returned showing excellent tissue healing and is currently under long-term follow-up.

A Light and Ultrastructural Analysis in Odontogenic Cysts and Oral Squamous Cell Carcinoma

A radicular or periapical cyst or periradicular cyst is the most common odontogenic cyst mostly seen in mandible. It is mostly symptomless and are found accidently during periapical radiographs involving non-vital pulp which heals spontaneously after endodontic treatment of the aberrant tooth or after extraction. However, recommended treatment is complete enucleation of the cyst to remove all the epithelial remnants. In this article, we report a case of a 18year old female patient with large periapical cyst along with deep bite and attrition.

Orbital Floor Fracture Reconstruction using Conchal Auricular Cartilage Graft

Introduction Hemangiomas are benign vascular tumors frequently occurring in the head and neck region; however, involvement of the paranasal sinuses is rare. Among these, cavernous hemangiomas of the maxillary sinus are particularly uncommon and may mimic sinonasal malignancies clinically and radiologically due to their aggressive features. Common symptoms include unilateral nasal obstruction, epistaxis, and facial swelling, necessitating a comprehensive diagnostic and surgical approach. Case Report We report a case of a 68-year-old male with a two-year history of persistent right-sided nasal obstruction and blood-tinged discharge, accompanied by facial asymmetry and weight loss. Examination revealed a mass occupying the right nasal cavity, displacing the inferior turbinate. Imaging via contrast-enhanced CT showed a 6.2×5.7×3.7 cm enhancing soft tissue mass in the right maxillary sinus with bony erosion and extension into the nasal cavity, sphenoid sinus, and orbit, raising suspicion for malignancy. Pre-operative angiography confirmed vascular supply from the internal maxillary artery, and embolization was performed. The patient underwent a combined open and endoscopic right partial maxillectomy. Histopathology confirmed a diagnosis of cavernous hemangioma. Discussion Despite their benign nature, sinonasal hemangiomas—especially cavernous types—can cause aggressive bony destruction, mimicking malignancy. Imaging may reveal enhancing masses with areas of necrosis and hemorrhage, contributing to diagnostic challenges. Pre-operative embolization is a valuable adjunct to surgical resection to minimize intraoperative bleeding. Histologically, cavernous hemangiomas consist of dilated blood-filled vascular spaces with no cytological atypia, aiding in the differentiation from malignant lesions. Conclusion Maxillary sinus cavernous hemangiomas are rare and can present as a diagnostic dilemma due to their malignant-like features. A multidisciplinary approach involving endoscopy, imaging, embolization, and histopathological evaluation is critical for accurate diagnosis and effective management. Complete surgical excision remains the mainstay of treatment with favorable outcomes.

Dentoalveolar injuries can lead to complications like pulpal necrosis, apical periodontitis, and usually radicular cysts, which are common inflammatory odontogenic cysts often triggered by trauma or infection. Radicular cysts are typically asymptomatic but may cause slow, noticeable swelling. This case report details the surgical management of the same in the periapical region of a maxillary incisor in a 12-year-old female patient. The approach included root canal therapy followed by periapical surgery. The cystic lesion was surgically enucleated, and a combination of platelet-rich fibrin (PRF) and demineralized bone matrix (DMBM) xenograft was applied to enhance healing, alongside a type I collagen resorbable membrane. Follow-up evaluations showed the patient was asymptomatic, with significant periapical healing and almost complete bone regeneration observed on radiographs.

Among the odontogenic cysts, there are radicular cysts or also called periapical cysts, are the most common odontogenic cyst. They develop at the apex of a tooth with pulp necrosis, which may be the consequence of dental caries or trauma. they are asymptomatic, grow slowly, and are discovered on routine radiographs. With the proliferation of the ERM and its activation, there is an increase in osmotic pressure, causing the cyst to expand, normally this ERM is not activated, so only granulation tissue develops at the apex of the affected tooth. Granulation tissue is called periapical granuloma and, as such, histologically lacks an epithelial lining. In the present study, a case of a 65-year-old female patient with an inflammatory periapical cyst in the region of the lower incisors is reported.

Surgical precision in the reconstruction of the orbital floor is crucial to functional visual and aesthetic outcomes. Increasingly, computer-aided design is being utilized to aid in precise preoperative planning by using the mirror images of the unaffected side. The authors aim to use 3-dimensional (3D) quantitative analysis to establish whether the native orbital floor topography is sufficiently symmetric to support this practice. Ten high resolution head and neck computed tomography scans of patients without periorbital pathology were obtained. These were imported into a 3D medical image processing software and segmented to isolate bilateral orbital floors. Each native orbital floor was compared to the mirror image of the contralateral side by conformance map computation. Data collection included measures of 25% and 75% quartile, median, mean, standard deviation, and root-mean-square (RMS). The topographic analysis demonstrated a high degree of topographic conformance with a mean RMS of 0.58 ± 0.37 mm. Further volumetric analysis comparing the total orbital volume between each side also demonstrates a high degree of volumetric symmetry with a mean difference of 0.55 mL (P = 0.30). Comparison of the native orbital floor and the mirror image of the contralateral side by conformance map computation and volumetric analysis demonstrated a high degree of morphologic similarity. The native orbital floor topography provides optimal symmetry to support mirror imaging techniques used in orbital floor reconstruction.

Radicular cysts rarely impact the primary dentition. Apical infections in primary molar teeth accounted for the majority of cases. However, there is relatively modest data about radicular cysts related to primary incisors and its association with compound odontoma. Case Report: An 11-year-old boy presented with a history of trauma to the left upper anterior teeth region. Clinical and radiographic examinations revealed the presence of a compound odontoma associated with a radicular cyst. The cystic lesion appeared to be linked to a previously traumatized tooth. Surgical management involved the removal of the compound odontoma along with enucleation of the radicular cyst under local anaesthesia. Conclusion: Since the majority of radicular cysts of primary teeth go undetected and potentially harm the growing succedaneous tooth bud, it is critical that paediatric patients receive the proper diagnosis and treatment for better outcome of succedaneous tooth.
