Implementation of Risk Management in Hospitals

Background: TPPRJ as a leading unit that provides services to patients needs to minimize the risks and hazard that can be experienced by officers and patients. From a preliminary study at one of the local government hospitals in the Central Java region, it was found that at the outpatient registration point there was no Standar Procedur Operational (SPO) regarding risk management, so there was no standard set for risk control for officers and patients.Objective: The purpose of this study is to identify the application of risk management in the outpatient registration site (TPPRJ) of the hospital.Method: The type of research used in this study is qualitative descriptive with a case study design. The subjects of the study were 4 informants determined by purposive sampling techniques. Data collection through interviews and observationsResults: There are several risk factors in TPPRJ, namely from physical aspects (air temperature, lighting, radiation), biological aspects and ergonomic aspects. From the results of interviews and observations, it is known that the implementation of risk management in hospitals has been running, but it is still not optimal because it has not yet reached a comprehensive evaluation from the hospital's quality management.Conclusion: Risk management has been running but has not been maximized, because there is no SPO that specifically regulates risk management at TPPRJ. The risk management process that has been running is to identify risks, risk analysis and risk control.Keywords: Risk Management, RMIK Work Unit, TPPRJ

Moreover, there is an increasing amount of literature covering specific geotechnical risk analysis and management topics. Examples of these are the difference between unsafe geotechnical certainty and safe geotechnical uncertainty (Barends, 2005), the role of the human factor in achieving geotechnical reliability (Bea, 2006), objective and subjective ways of geotechnical risk classification (Altabba et al., 2004), and contractual allocation of geotechnical risk (Essex, 2007).

The implementation of a rapidly developing library service system can create threatening risks caused by the failure of library managers to assess the source of risk threats. Risks can take the form of events and conditions that result in disruption of information services and the decision-making process stops. The aim of this research is to describe the implementation of library service risk management at the Palembang Aviation Polytechnic Library using the ISO 31000 approach to determine the factors that influence the implementation of library risk management, especially in library material management activities. Library risk management at the Palembang Aviation Polytechnic Library has been implemented to anticipate various sources of risk threats by carrying out risk assessment, risk mitigation and risk evaluation activities through the Government Internal Control System (SPIP) in general.

This research discusses the risk management process applied at Islamic State Universities with a case study at UIN Walisongo Semarang. The research seeks to describe and analyze the risk management process by using interviews and documentation techniques. The analyzed data used was an interactive model, including data collection, data reduction, data presentation, and conclusions. The result showed that risk management has been well running at UIN Walisongo Semarangan, even though there is no risk management committee, and it is still attached to the Internal Supervisory Unit. The risk management process used the AS/NZS 31000:2009 standard which consists of the process of establishing context, risk identification, risk analysis, risk evaluation, risk handling, monitoring and review, and communication and consultation. If it is assosiated to the reputation of higher education, UIN Walisongo risk management has prepared anticipatory steps related to the reputation of the institution. These steps are damage control, reputational risk assessment, and management academic community control.

In modern conditions, regardless of what type of economic activity an organization faces various risks. For manufacturing enterprises, the area of risks is one of the most extensive, since it is necessary to take into account uncertainties in both financial and production activities. Creation of an effective risk management system determines ways and opportunities to ensure sustainability of organizations and their ability to withstand adverse situations. Risk reduction is possible through risk management, which involves their identification and assessment, as well as the use of procedures and management me-thods that would reduce possible risks. For the most effective implementation of risk management, a systematic approach to organizing this process is required. Risk management will be of particular importance for energy companies due to the changes that have been observed in this area over the past few years. The paper proposes a risk management model for energy enterprises, which includes the goal, object, subject, functions, principles and processes of risk management, taking into account the characteristics of energy enterprises. An algorithm for implementing this model is presented, consisting of five main stages. The use of this algorithm will allow us to obtain intermediate results from the implementation of the risk management system and, if necessary, adjust this process. The paper presents an analysis of existing concepts on the use of risk points. Attention is also paid to the issue of using this method at energy enterprises in other countries. The methodology for determining critical control points, which lies in at the heart of HACCP (Hazard Analysis Critical Control Point) system used in the food industry, is discussed in detail. The possibility of using risk points to determine those responsible for risks at energy enterprises is substantiated. An analysis of the operating thermal power plant was carried out in order to identify points of production and technical risk. The result of the analysis was the identification of those responsible for this type of risk.

Introduction: Managing risks means, in the case of risks with negative effects, having them under control to mitigate or eliminate them, if possible, or in the case of risks with positive effects, turning them into opportunities. The Adolfo Lutz Institute (IAL) establishes documents and implements and maintains a management system in accordance with option A of ISO/IEC 17025: 2017. Thus, risk management is one of the requirements to be met. In order to fulfill this requirement, the IAL started the implementation of the risk management process using the Failure Mode, Effect and Criticality Analysis (FMECA) tool. As the methodology was considered complex by the collaborators, this tool was abandoned and only the brainstorming was used for the identification step, and the probability and impact analysis at the risk analysis for the assessment stage. Objective: To assess the level of implementation of the risk management process in the IAL and identify the main difficulties involved in this process. Method: 74 risk matrix forms filled out by the various sectors of the institution were analyzed. Results: There was a 76.3% adherence to the implementation of risk management and the main difficulties encountered in the risk management process were: identification of the risks, including identification of risks with positive effects, selection of an indicator associated with risk, and proposal of actions to treat risks. Conclusions: To implement the risk management process, the use of simpler tools should be recommended when the team maturity level is still low or intermediate. Another point to be considered for the successful implementation of this process is strengthening of the understanding of risks by everyone in the organization. In IAL, the risk management process is in the risk monitoring phase; the next step is to review the risk mapping initially done.

National Nuclear Safety Administration issuance “The Technical Policy of Nuclear Power Plant Configuration Risk Management” in December 2019 to support the risk management of multiple system/equipment failures and the flexible operation of nuclear power plant (NPP). Subsequently, Suzhou Nuclear Power Research Institute PSA teams and Daya Bay NPP nuclear safety advisory/Operation operator/planning Engineer established a joint project team to study the implementation of configuration risk management. The configuration risk management system framework and operation process have been determined through the in-depth discussion of multiple disciplines and departments, and the trial operation of risk management system has been started in January 2021 to optimize iterative software and implementation processes. This paper introduces the composition of Daya Bay configuration risk management system, the responsibilities of each discipline in the implementation of configuration risk management, the establishment method and results of risk threshold and risk management matrix, the functions of operation and maintenance risk assessment tools, the characteristics of risk calculation engine, the implementation process of operation configuration risk management and maintenance configuration risk management, the problems found during the trial operation, the management measures for subsequent upgrades, so as to provide reference for other NPPs to carry out the configuration risk management.

Risk management has been used by health organizations as an investigative, scientific and technical process to achieve service quality, as well as to guarantee patient and professional safety. This research aimed at identifying the perceptions of nurses regarding hospital risk management and analyse facilitators and barriers to the process. This was a qualitative descriptive and exploratory research. A total of 29 clinical and management nurses at two large private hospitals in south of Minas Gerais participated in the research. The semi structured interviews were recorded and their data submitted to content analysis that resulted in five categories: deficiency in the conceptualization of risk management; risk management qualifying care; need for culture change for the operationalization of risk management; risk management perceived as reporting; and influences of human resources on risk management processes. Results revealed that nurses consider risk management as an instrument to qualify patient safety. Another conclusion was that events were underreported due to lack of time to fill forms, work overload, fear of retaliation and punishment. Actions to raise staff's awareness, permanent education programmes and management support were strategies to develop work processes.

As the most critical aspect of risk culture, risk awareness is increasingly seen as informal driver in a firm’s risk management system influencing results and contributing to a firm’s strategic advantage. This study examines the mediation effect of risk awareness on the relationship between formal enterprise risk management (ERM) implementation and company performance. The investigation is based on survey data from 121 non-financial companies in Austria. Hence, I contribute to strategic ERM research by addressing the role of risk awareness in a formal ERM system and explore its benefits. Results support a positive association of formal ERM implementation and company performance and a mediation effect of risk awareness. Furthermore, respondents occupying a controller position but being responsible for the company’s overall risk management process have a more conservative perception of a firm’s performance.

Risk management plays a crucial role in enhancing organizational effectiveness by proactively identifying, assessing, and mitigating uncertainties that may hinder strategic objectives. This study aims to analyze key success factors and barriers in risk management implementation across various organizational contexts in Indonesia. Using a qualitative case study approach, data were collected through semi-structured interviews, direct observation, and document analysis involving six informants with direct involvement in risk management processes. Thematic analysis revealed that effective risk management contributes to improved strategic decision-making, resource optimization, financial performance, and organizational resilience. However, implementation challenges persist, including cybersecurity threats, regulatory uncertainties, limited human resources, and resistance to change. The findings highlight the importance of cultivating a risk-aware organizational culture, leveraging advanced technologies, and aligning risk strategies with dynamic external environments. This research offers a comprehensive conceptual framework for integrating risk management into strategic and operational decision-making, supporting organizational sustainability and competitiveness. The study also provides practical insights for policymakers and managers seeking to strengthen risk governance. Due to its limited sample scope, further research is recommended in diverse sectors and with mixed-method approaches to deepen empirical understanding.

This study aims to evaluate the implementation of the risk management process in the Zero Rupiah Housing Fund Program intended for Low-Income Communities with DKI Jakarta Provincial Governor Regulation Number 122 of 2020 and the ISO 31000: 2018 conceptual framework as a reference concept for best practice in the Housing Fund Management Unit that implements the Regional Public Service Agency Financial Management Pattern. The research method used is qualitative with a case study approach. This research uses a triangulation method that combines various research instruments simultaneously to strengthen the results obtained from all types of data, including document analysis and interviews, to be analysed using the thematic analysis method. In this study, the risk management process in implementing the Zero Rupiah DP Program is evaluated, starting from the program planning process to the monitoring and evaluation process of program implementation. The results of this study show that UPDP has not established a risk appetite, and the risk management process in the Zero Rupiah DP Program is still at the risk evaluation stage. This study provides recommendations regarding implementing risk management that refer to the reference standards as best practice in the risk management process.

Many cases of fraud in the form of corruption and bribery are findings of external examinations. It shows internal supervision is not sufficient. The internal supervision of companies and local governments have not been able to detect fraud. In this study, we suspect the internal auditors' workload, which makes their ability to detect fraud poor. If the company or local government has implemented risk management, the workload will not be as heavy as if it has not implemented risk management. The company and local government can apply a Risk-based internal audit if they have implemented risk management. Based on this explanation, conclude that finding auditor expertise of the fraud is influenced by workload, the implementation of risk management, and risk-based internal controls. The population of this research is the internal auditors of the provincial government of North Sumatra. This study uses primary data obtained from distributing questionnaires to the research sample. Data were analyzed using multiple regression analysis with the help of PLS applications. The findings showed that risk management application and risk-based supervision audit has a significant effect on detecting fraud and the workload is not affect to the ability to detect of fraud finding. The interviews with several informants concluded that if they have implemented risk management and risk-based internal audits, the internal auditors will not have a high workload. Because since the beginning, it has been identified which activities have an increased risk.

This research is focuses on the practice of risk management (RM) in Client’s organization and aims to identify the level of awareness among construction professionals towards risk management and to examine the policy undertaken when dealing with risks in a construction project. Apart from that, it also aims to identify the problems and challenges for the implementation of risk management in Malaysia. Questionnaire survey and interviews were carried out in order to obtain a better view on the implementation of risk management in the Malaysian construction industry. It was found that Malaysian clients are slowly accepting risk management as a management tool that will help in managing a construction project effectively and successfully. Based on the findings of the questionnaires and interviews there are a number of clients who know about risk management and who have attended training and some of them have even practice risk management in their organizations. At least, it has been proven that there are organizations that have implemented risk management in their operations although this is only on a small scale. It can be concluded that risk management still has a long way to go in order to be accepted and recognized in the Malaysian construction industry.

Risk control and management are becoming increasingly important in many organisational operations. It goes beyond the business sector to include security organisations like police departments. The fact that police officers engage in a widerange of daily activities that expose them to a variety of potentially hazardous situations Police risk management includes both physical and financial aspects. Risk identification is the first step in the risk management process, followed by risk analysis, risk evaluation, and risk treatment. According to the literature, the Dubai Police Department has no framework in place to fully manage risks and evaluate the effectiveness of police operations. Early warning systems are critical for detecting potential severe RM because police departments must rely on them to manage risks and keep track of any potential new risks. As a result, the UAE police department's risk management strategy was investigated in this paper. It classified the data collection process using exploratory factor analysis, which wasa quantitative approach. A self-administered survey yielded 381 valid questionnaires from Dubai Police Department officers. The study's findings revealed that risk identification, risk analysis, risk evaluation, and risk response are the four fundamental steps of effective risk management. The Crobach Alpha test was used to assess the consistency and reliability of the data, and the result was 0.814, indicating that the data is suitable for use. Only one of the twenty indicators, which were divided into four groups for risk management, was weak according to outliers and communalities tests. The risk management process was accurately described by another 20 factors. Total variance extraction yielded a rate of 57.130 percent, which was higher than the required 50%. As a result, the risk management procedure investigated in this study may be useful for effectively managing risks in the UAE police force.

Risk management in translation service companies is an effective, integrative, proactive way to handle risk. Such companies must integrate defined long-term risk management into their strategies, decision-making, and daily processes; top-management must fully support the risk management system; risk factors, management benefits, and processes require articulation amongst employees; job roles, responsibilities, performance evaluation, and motivation systems must incorporate risk management practices. Risk management in project-based companies might also account for changing clients’ requirements, tight deadlines and budgets, different participants, and high IT use. However, this requires identifying the critical success factors. The research method builds upon the analysis and systemization of the scientific literature – from risk management, emphasizing the underlying strategic approach and effective management, to deriving the focal-points of effective risk management of translation projects. This work unifies the gathered knowledge and results in a conceptual model that integrates the specific assessment criteria for project risk management in translation companies – process definiteness and versatility, responsibility definiteness, top-management involvement, and risk management communication. The operationalisation of the model may lead to the companies’ critical success performance. To the best of our knowledge, this work is one of the first contributions addressing risk management assessment criteria in this industry.
