Application of family-centered transition management scheme in Pediatric Intensive Care Unit

Objective To explore the effects of family-centered transition management scheme in pediatric intensive care unit. Methods A transition management group was founded. According to Pediatric Critical Illness Score (PCIS) , PICU discharge criteria was unified. Then, two sets of transition management scheme were developed according to the scoring results. A SBAR communication record form was designed for the structural shift by the transitional care nurses. At last, family-centered of diversified education were applied. After the implementation of the scheme from November 2016 to April 2017, a total of 75 children and 75 parents were recruited as experimental group, while from May to October 2016, before the implementation of the scheme, 68 children and 68 parents were recruited as control group. The two groups were compared with the incidence of re-admission to PICU, hospitalization time, nursing adverse events, family uncertainty and satisfaction rate on nursing service. Results The rate of re-admission to PICU, total hospitalization time, the rate of adverse events and the scores of parent's perception uncertainty scale-family in the experimental group were significant lower than those in the control group, and the differences were statistically significant (P<0.05) . The satisfaction rate on nursing service in the experimental group was significant higher than that in the control group (P<0.05) . Conclusions Family-centered transition management scheme is conducive to the improvement nursing quality for PICU children and their family and the promotion of patient safety.
