Missed Nursing Care in Hospitalized Patients

This study aims to explore the effects of human and material resources on mathematical literacy. For this purpose, mathematical literacy test scores and questionnaire responses of 304,444 fifteen-year-olds in 45 countries participated in the 2012 cycle of Programme for International Student Assessment (PISA) Project, were analysed through two-level and three-level hierarchical linear models (HLM). Selected indices and scales representing material and human resources’ effects on students’ mathematical literacy were investigated. The results revealed that 23% of the total variance in the literacy scores is attributable to between-countries, 34% of the variance is attributable to between-schools and the remaining 43% to individual student characteristics. Only two school factors, the quality of school educational resources and teacher morale, were found to have effects on students’ performance after accounting for the gender, the index of economic, social and cultural status, and the cumulative expenditure on education. The results of the study have potential to help policy makers determine their priorities in education and provide hints for future studies. This study aimed to examine the influence of missed nursing care on satisfaction with nursing care and trust in nurses among patients with cancer. A descriptive and correlational study. The study involved 271 patients with cancer hospitalised in the Medical Oncology units of a university-affiliated Oncology Hospital in Türkiye. Data were collected using the Patient Information Form, MISSCARE Survey-Patient (MCSP), Newcastle Satisfaction with Nursing Care Scale (NSNCS) and Trust in Nurses Scale (TNS) between December 26, 2022, and August 30, 2023. Descriptive and inferential statistics were conducted using SPSS 26. The STROBE checklist guided reporting. Mean scores were 1.71 (SD = 0.77; possible range = 1-5) for the communication subscale of the MCSP, 3.86 (SD = 1.19; possible range = 1-5) for the basic care subscale of the MCSP, 92.93 (SD = 13.80; possible range = 0-100) for NSNCS and 27.64 (SD = 3.21; possible range = 5-30) for TNS. Missed communication was statistically significantly and negatively correlated with both satisfaction and trust, whereas missed basic care was not significantly correlated with either. Regression analyses confirmed that missed communication significantly and negatively predicted satisfaction and trust, whereas missed basic care did not. Missed communication was associated with lower satisfaction with nursing care and reduced trust in nurses among patients with cancer, whereas missed basic care, despite higher levels, did not predict either outcome. These findings highlight the central role of nurse-patient communication in shaping patients' perceptions of nursing care quality in oncology settings. Within oncology care contexts, healthcare institutions and nurse managers should prioritise strategies to reduce missed nursing care, particularly missed communication. Short-term actions include strengthening nurse-patient communication through structured communication training and manageable workloads, while long-term strategies should embed communication and basic care standards into institutional quality frameworks to enhance patient satisfaction and trust. In the United States, the incidence rate of new or recurrent stroke is approximately 795 000 per year, and stroke prevalence for individuals over the age of 20 years is estimated at 6.5 million.1 Mortality rates in the first 30 days after stroke have decreased because of advances in emergency medicine and acute stroke care. In addition, there is strong evidence that organized postacute, inpatient stroke care delivered within the first 4 weeks by an interdisciplinary healthcare team results in an absolute reduction in the number of deaths.2,3 Despite these positive achievements, stroke continues to represent the leading cause of long-term disability in Americans: An estimated 50 million stroke survivors worldwide currently cope with significant physical, cognitive, and emotional deficits, and 25% to 74% of these survivors require some assistance or are fully dependent on caregivers for activities of daily living (ADLs).4,5 Notwithstanding the substantial progress in acute stroke care over the past 15 years, the focus of stroke medical advances and healthcare resources has been on acute and subacute recovery phases, which has resulted in substantial health disparities in later phases of stroke care. Additionally, healthcare providers (HCPs) are often unaware of not only patients’ potential for improvement during more chronic recovery phases but also common issues that stroke survivors and their caregivers experience. Furthermore, even with evidence that documents neuroplasticity potential regardless of age and time after stroke,6 the mean lifetime cost of ischemic stroke (which accounts for 87% of all strokes) in the United States is an estimated $140 000 (for inpatient, rehabilitation, and follow-up costs), with 70% of first-year stroke costs attributed to acute inpatient hospital care1; therefore, fewer financial resources appear to be dedicated to providing optimal care during the later phases of stroke recovery. The integration of Artificial Intelligence (AI) and digital connectivity can revolutionize material and human resources management in secondary schools. The study focused on human and material resources management in education in Nigeria and its impact on students’ academic performance in secondary schools, in Ebonyi State, Nigeria. Subjects for the study were two hundred (200) students and hundred (100) teachers randomly selected from five secondary schools in Afikpo, Ohozara, and Ezza South Local Government Areas of Ebonyi State. Questionnaire was used to collect relevant data for the study. Pearson Product Moment Correlation was used to analyses the formulated hypotheses. The findings of the study revealed that there was significant positive relationship between human resources management and students’ academic performance (r = 0.814; p < 0.05). Also, significant relationship exists between material resources management and students’ academic performance (r = 0.629; p < 0.05). The implication is that effective material resource management may bring about improvement in student’s academic performance. While it was recommended that effective practice of both human and material resources is to be well implemented for effective performance of students at any level of educational system in Nigeria.

Communicating Health Information

Maintaining a high standard of nursing care is imperative for ensuring patient safety. Several factors significantly impact the provision of nursing care, including work environment resources, personnel coordination, work systems, and head nurse leadership. In addition, each nurse's clinical and academic career also plays a role in shaping the quality of care provided to patients. This article reports results of a cross-sectional study aimed to identify the different types of missed nursing care and the factors that contribute to them, as perceived by nurses, and second, to investigate how nurses' characteristics may relate to the occurrence of missed nursing care. Data for this study were obtained through a self-administered questionnaire that was distributed to participants working in an intensive care unit. The study included a final sample size of 176 participants, all of whom worked in intensive care unit hospitals located in the north region of Palestine. The study found that handwashing, setting up meals for patients who feed themselves, discharge planning, and response to a call light were the most frequently missed nursing care activities. The primary factors identified as reasons for missed nursing care were inadequate availability of labor and material resources, along with communication issues. Efforts to address these identified issues can potentially lead to improved quality of nursing care in intensive care units.

Quality of nursing care across hospitals is variable, and this variation can result in poor patient outcomes. One aspect of quality nursing care is the amount of necessary care that is omitted. This article reports on the extent and type of nursing care missed and the reasons for missed care. The MISSCARE Survey was administered to nursing staff (n = 4086) who provide direct patient care in 10 acute care hospitals. Missed nursing care patterns as well as reasons for missing care (labor resources, material resources, and communication) were common across all hospitals. Job title (ie, registered nurse vs nursing assistant), shift worked, absenteeism, perceived staffing adequacy, and patient work loads were significantly associated with missed care. The data from this study can inform quality improvement efforts to reduce missed nursing care and promote favorable patient outcomes.

Introduction:Tracking patients through health systems is fundamental to coordinated care provision. However, it is an inconsistent element of emergency preparedness. This presentation presents findings of a study undertaken after the 2011 Christchurch Earthquake, and the resultant nationally implemented changes.Aim:The intent was to investigate options to improve patient tracking in a mass casualty event. By looking at one scenario involving a mass casualty presentation with the central responder disabled by electricity loss, standards of practice were outlined and made scalable to meet the needs of various events.Methods:Clinical and clerical staff involved in the event’s patient tracking were interviewed. Data were analyzed using thematic analysis and reported using the structure, process, and outcomes framework.1Results:Structures were material and human resources. Material resources were identification number systems, technological requirements, disaster-specific documents, minimum data for entry, digital/paper/hybrid registration systems, and digital-paper integration. Human resources were role allocation, and familiarity of plans, roles, processes, tools, and facilities. Process identified the activities to manage unidentified patients, triage, registration, and ongoing tracking processes. Outcomes were management of patient flow, patient-care provision, and patient-family reunification.Initial implementation was local. Structures and processes were agreed upon, with varying response levels according to the incident scope, while staying as close to business as usual for familiarity. National implementation followed via a Ministry of Health working group involving different district health boards. The group developed a consensus on the minimum data to be entered and the process to merge patient identities of initially unidentified patients. Written tools were shared for standardization.Discussion:With inter-agency and inter-organization emergency response, standardized processes and information are required. Collaboration prior to events can mitigate issues when an event occurs.

Defense planning is a particularly important topic, which implies an analysis of the international situation, a realistic assessment of its own national security system, whichinvolves medium- and long-term decisions. The allocation of resources should follow the defense policy established following a strategic analysis process, after the strategic objectivesand level of ambition have been established. The construction of military capabilities is complex, is carried out over time and requires the allocation of financial resources and apredictable and coherent human resource policy. Discontinuous reforms, or the alteration of decisions with opportunistic, conjuncture solutions, adversely affect future programs andproduce waste of financial and material resources, as well as social convulsions in human resources. The structure of forces on which the state's defense capacity is built must beincreasingly professional to have a timely and effective responsiveness, and the resilience capacity for national defense should be based on the mental and armed resilience of thepopulation in the case of temporary occupation of the national territory. Membership of NATO and the European Union increases resilience through the commitments of the two organizationsto support the restoration of Romania's territorial integrity in the event of military aggression. The increasingly technologically evolved human society requires a rethinking of thenational mobilization system, both in terms of human resources and in terms of the involvement of the resources of the national economy. Maintaining the functioning of critical infrastructure,operationalizing industrial facilities and maintaining the capacity to provide the material and financial resources needed for the war effort are particularly important, and the workforce inthese areas cannot be made available to complement military structures. Law enforcement and civil emergency forces will be engaged in the specific missions of each of them but may formbases of resistance in the case of national occupation. Participation in NATO and EU force structures must be in line with the force structure designed for national needs, renegotiated,realistically, in the medium and long term, with sustainable promises corresponding to common standards. Allocating a 2% defense budget is not the single and sufficient solution to achievinga credible and sustainable defense capability. Variations in nursing care quality across hospitals. The aim of the study was to describe Registered Nurses' reports of unmet nursing care needs and examine the variation of nursing care quality across hospitals. Large proportions of Registered Nurses have reported leaving necessary care activities undone because they lacked the time to complete the activities. Nursing care left undone can be expected to adversely affect the quality of care. However, little is known about the degree of variation in the quality of nursing care across hospitals. In 2008, a secondary analysis of a 1999 survey of Registered Nurses (n = 10,184) was conducted using descriptive and comparative statistics. Data were derived from inpatient staff nurses working in acute care hospital settings (n = 168). A hospital-level measure (i.e. unmet nursing care needs) of the quality of nursing care was developed from care needs left undone among all nurses. Across hospitals there was a wide range in the proportion of Registered Nurses who reported leaving each nursing care need undone. They reported leaving two of seven necessary nursing care activities undone during their last shift. After controlling for nurses' demographic information, we found statistically significant variations in the quality of nursing care across hospitals. Differences in nursing care quality across hospitals appear to be closely associated with variations in the quality of care environments. Understanding the determinants of unmet nursing care needs can support policy decisions on systems and human resources management to enhance nurses' awareness of their care practices and the care environment.

This study aims to assess the performance of machine learning (ML) techniques in optimising nurse staffing and evaluating the appropriateness of nursing care delivery models in hospital wards. The primary outcome measures include the adequacy of nurse staffing and the appropriateness of the nursing care delivery system. Historical and current healthcare challenges, such as nurse shortages and increasing patient acuity, necessitate innovative approaches to nursing care delivery. For instance, the COVID-19 pandemic highlighted the need for flexible and scalable staffing models to manage surges in patient volume and acuity. A descriptive study was conducted in 39 inpatient wards across a university hospital and three state hospitals, involving 117 ward-level observations. Data were collected using the Rush Medicus Patient Classification Scale and analysed using k-Nearest Neighbour, Support Vector Machine, Random Forest, and Logistic Regression algorithms. Effectiveness was measured by the accuracy of machine learning predictions regarding nurse staffing adequacy, while suitability was determined by the congruence between observed nursing care models and patient needs. STROBE checklist. The Random Forest algorithm demonstrated the highest accuracy in predicting both nurse staffing adequacy and the appropriateness of nursing care delivery systems. The study found that 68.4% of wards had sufficient nurse staffing and 26.5% of wards used appropriate care delivery models, with functional nursing and total patient care models being the most commonly used. The study highlights functional nursing and total patient care models, emphasising the need to consider nurse qualifications and patient needs in selecting care systems. Machine learning, particularly the Random Forest algorithm, proved effective in aligning staffing with patient requirements. This study sets a precedent for using data-driven approaches to improve nurse staffing and care delivery, potentially enhancing global clinical outcomes and operational efficiencies. The global clinical community can learn from this study the value of employing machine learning techniques to make informed, evidence-based decisions in healthcare management. While the study lacked direct patient involvement, its goal was to enhance patient care and healthcare efficiency. Future research will aim to incorporate patient and public insights more directly.

Clinical trials on pediatric oncology use therapeutic techniques with the overwhelming majority of children's cancer patients obtaining therapy via clinical investigation procedures. Medical treatment is scheduled according to a specific protocol for enrolled patients. These protocols often do not refer to nursing care. Nursing care, on the other hand, must complement the medical care specified in the medical research protocol. Safe treatment administration, assessment of treatment responses, patients' and families' education, and communication with the whole medical team are just a few of the critical nursing tasks that should be properly managed. Nursing care standards have been developed in this study to strike a good balance between the procedure for clinical research and the nursing care connected with it. These recommendations outline the nursing activities and considerations that must be made while caring for pediatric cancer patients who are engaged in a specific clinical investigation procedure. The objective of this study is to outline the procedure through which nursing care guidelines could be developed and evaluated. The goal of this study was to find out the involvement of nurses in the process of health education for osteosarcoma and family patients.

Missed nursing care in hospital wards is associated with patient safety risks and reduced quality of care. Emergency departments are high-risk settings; therefore, missed nursing care in these settings must be explored. Studies on missed nursing care in emergency departments show, for instance, that patient education, nutrition, and the maintenance of patient privacy are often missed. These findings are similar to those of previous studies conducted in hospital wards. However, crowding, boarding, and patients with acuity issues in emergency departments are examples of contextual differences with hospital wards. How emergency nurses perceive their nursing practices in relation to missed nursing care and their ability to maintain patient safety and quality of care is not well-known. This study aimed to explore emergency nurses' perceptions of nursing care in the emergency departments. This study used a qualitative descriptive design and a purposive sampling strategy. Semi-structured interviews were conducted with 15 emergency nurses. A phenomenographic approach was used for the analyses. The findings showed variations in the perceptions of emergency nurses regarding the quality of nursing care and missed nursing care. Subordinate to acute medical needs, fundamental care needs were addressed if there was sufficient time, which negatively affected older adult patients, particularly frail patients. Emergency nurses strived to maintain adequate patient flow throughout the emergency department and were attentive to all patients in their care; however, they were aware that missed nursing care was a recurring phenomenon. Emergency nurses had various perceptions of nursing care. Demanding conditions, prioritization of medical needs, and maintenance of adequate patient flow challenged them in their efforts to meet patients' fundamental needs and fulfill their responsibilities.
