Abstract

Case description: A 26-year-old female patient presented with left knee pain and swelling secondary to a fall. Radiographs showed no fracture; however, swelling persisted. Arthrocentesis was performed, including cytological and cytochemical analysis of synovial fluid, yielding results suggestive of pigmented villonodular synovitis (PVNS). Magnetic resonance imaging was performed, confirming the diagnosis. A literature review was conducted using PubMed. Pigmented villonodular synovitis refers to a subtype of giant cell tumors of the tendon sheath that diffusely affect the soft tissue lining of joints and tendons. The estimated incidence rate of PVNS is 1.8 per million people, with a prevalence typically between the third and fourth decades of life. Results, treatment, and follow-up: Diagnostic and therapeutic arthroscopy (synovectomy) was performed. A synovial biopsy confirmed the histopathological diagnosis of pigmented villonodular synovitis. This was complemented with radiotherapy sessions and follow-up in the outpatient clinic. The patient showed decreased knee swelling, reduced pain, and full range of motion. Conclusion: Pigmented villonodular synovitis is a rare benign tumor pathology that may lead to complications if not diagnosed promptly. A series of studies is recommended to establish the diagnosis. Arthroscopic synovectomy combined with radiotherapy is considered an effective treatment, with cure rates approaching 100%.
