Pressure injuries are a known complication during surgery, especially in patients positioned laterally under general anesthesia. This study investigated the relationship between preoperative transepidermal water loss (TEWL) and postoperative skin injuries in patients undergoing thoracoscopic pneumonectomy. A prospective observational study was conducted in 86 patients aged 20years or older. Preoperative assessments included TEWL; stratum corneum hydration; and skin temperature, moisture, and pH at high-risk sites. Logistic regression and receiver operating characteristic analyses were used to evaluate predictive factors. Preoperative TEWL was significantly associated with postoperative skin injuries (R2=0.545, B=-0.094; 95% confidence interval, -0.161 to -0.037; P<.001). Receiver operating characteristic analysis identified a TEWL cut-off of 19.5 g/m2/h, with sensitivity of 0.500 and specificity of 0.903. These findings suggest that TEWL may indicate patients at risk for postoperative skin injuries, and early preventive strategies may reduce the incidence of these injuries in high-risk patients.

Nurse turnover affects both patient care and health care costs, with turnover intention reflecting employees' desire to leave an organization. This systematic review examines factors that contribute to turnover intention among OR nurses. A search of quantitative and qualitative studies in PubMed, Scopus, Cumulative Index to Nursing and Allied Health Literature, PsycInfo, and Embase identified 11 studies meeting the inclusion criteria. The contributing factors were categorized into individual and work-environment factors, with the latter further divided into job attributes and work-related issues. Key factors such as job satisfaction, age, and work experience were strongly associated with increased turnover intention. Addressing these factors is crucial for retaining OR nurses and improving health care outcomes. Given the complexity of factors involved, future research should adopt more standardized approaches, including consistent definitions and methodologies, to enable accurate comparisons and effective interventions. Then, strategies to improve job satisfaction and working conditions are recommended.

The OR is a time-pressured and high-risk environment in which nurses prioritize patient care tasks. The factors influencing perioperative nurses' ability to prioritize care activities are not well understood. This descriptive qualitative study aimed to describe the contextual factors that influence perioperative nurses' care priorities. Twelve semi-structured perioperative nurse interviews were conducted via an online platform. Thematic analysis yielded three themes including enhancing patient focus: prioritizing care based on patient characteristics; efficient collaboration: working harmoniously to achieve goals; and strategic preparation and decision-making: minute-by-minute planning. The priorities of surgeons and nurses are different and surgeons' priorities may prevent nurses from undertaking routine patient care activities that affect safety, such as the surgical count. This may lead to missed nursing care and potentially threaten patient safety. Strategies that promote a culture of safety, such as improving communication among team members may help nurses prioritize care activities.
