Entre las dificultades alimentarias que pueden manifestarse durante la infancia, el picky eating ha adquirido relevancia en la literatura cientfica debido a su alta prevalencia y a las posibles repercusiones sobre el estado nutricional, el desarrollo psicosocial y la dinmica familiar.Se ha observado que los nios picky eaters consumen menos cantidad de ciertos grupos de alimentos y pueden tener una ingesta menor de algunos macro-y micronutrientes.Dada la relevancia del tema, surge esta revisin narrativa que tiene el objetivo de abordar la definicin del picky eater en pediatra, criterios de evaluacin, factores de riesgo, impacto nutricional y psicosocial, as como las principales estrategias de intervencin basadas en la evidencia.Un abordaje temprano, integral e interdisciplinario, centrado en la familia es fundamental para manejar el picky eating, promover hbitos alimentarios saludables

Methylmalonic acidemia is characterized by the inability to convert L-methylmalonyl-CoA to succinylCoA, resulting in the accumulation of methylmalonic acid in the blood and/or urine. In its chronic form, it can cause cardiomyopathy with impaired cardiac function. Its management is based on several pillars; the most important is intramuscular hydroxycobalamin supplementation, which serves as a cofactor for methylmalonyl-CoA mutase. We present the case of a 6-month-old girl who was admitted to the emergency department with signs of heart failure. A multidisciplinary evaluation was performed to determine the etiology, and methylmalonic acidemia was diagnosed. We also report a novel route of administration for hydroxycobalamin in this condition (subcutaneous). We present the diagnostic approach and therapeutic challenges in this patient, given that the usual route of administration of hydroxycobalamin was contraindicated due to her comorbidities.

Avoidant/restrictive food intake disorder (ARFID), defined by the DSM-5, affects children's growth and development and is common in those with autism spectrum disorder (ASD). This update aims to describe the causes, consequences, detection strategies, and therapeutic approaches to ARFID in children diagnosed with ASD, based on published scientific articles. Original articles, reviews, meta-analyses, and clinical trials published between January 2016 and February 2025 in peer-reviewed journals were included in this review. The search was conducted in PubMed, Cochrane Library, and Google Scholar using the descriptors "ARFID AND AUTISM." Only fulltext studies in English or Spanish focusing on the pediatric population were considered; we found 23 relevant publications. This study explores how ASD characteristics can influence ARFID symptoms. Considering the findings, recommendations, and treatments based on clinical trials and meta-analyses are analyzed.

Introduction. Pediatric cancer exposes families to numerous chronic stressors, which require resources that are often lacking in vulnerable settings. Objectives. To assess the psychosocial risk among pediatric cancer patients and to explore caregivers' perceptions of the usefulness of the psychosocial support services offered by hospitals. Population and methods. This was an observational, prospective, descriptive, cross-sectional study. The study included caregivers of patients with a recent cancer diagnosis from five Argentine hospitals. Participants were administered the Psychosocial Assessment Tool (PAT) questionnaire and a hospital psychosocial intervention inventory designed for this study. Results. The sample consisted of 103 subjects. Public and private hospitals obtained similar risk scores. Average risk: moderate. The sample was distributed as follows: low risk (43%), moderate risk (41%), and high risk (16%). The areas of highest risk were family structure and resources, stress response, and parental problems. All institutional measures aimed at strengthening financial resources were reported as highly helpful. The choice of other types of interventions varied, with particular emphasis on the usefulness of those directed at patients and at strengthening social support. Interventions aimed at treating caregiver psychopathology were considered less useful, despite this type of problem being one of the most prevalent among parents at high and moderate psychosocial risk. Conclusion. This study assessed the psychosocial risk among pediatric cancer patients in Argentina, identified the psychosocial interventions offered by hospitals, and examined caregivers' perceptions of the usefulness of these interventions during the first month following diagnosis.

The use of extracorporeal membrane oxygenation (ECMO) in pediatrics poses a significant clinical and ethical challenges due to its high costs and associated morbidity and mortality. We present the case of a 2-year-old boy with severe bone marrow aplasia and septic shock who required ECMO as a bridge to hematopoietic stem cell transplantation. High-flow venoarterial support was initiated, initially without anticoagulation, due to marked thrombocytopenia and a high risk of bleeding. A multidisciplinary discussion was held involving the treating medical team and the institutional Ethics Committee. The patient was successfully weaned from support, showed favorable clinical progress until receiving the transplant, and achieved effective hematopoietic recovery. This case illustrates the complexity of decision-making regarding ECMO use in oncohematological patients, highlighting the importance of individualized assessment, family involvement, and the need to avoid futile therapeutic intervention.

Introduction. Patient information transfers between professionals on different shifts are a particularly sensitive area for errors and omissions. Our objective was to evaluate the impact of implementing a structured handover (I-PASS) on data omission in patient information transfers between pediatric residents. Population and methods. The study was conducted in a public hospital in the province of Salta, Argentina, from November 14, 2020, to December 14, 2020 (pre-intervention) and from March 15, 2021, to April 15, 2021 (post-intervention). The design was quasi-experimental, before-and-after, uncontrolled. Pre- and post-intervention shift handovers were evaluated. The intervention included training resident physicians in a structured handover using the mnemonic rule I-PASS, teamwork training, and digitized written handover. The quality of the handover was evaluated through direct observation. Results. There were 233 pre-intervention and 245 post-intervention evaluations. The pre- and postintervention comparison showed a significant improvement in most key handover data. Distractions were reduced from 40.8% to 24.1% (p = 0.001); communication of severity increased from 36.8% to 63.2% (p =0.001). Conclusion. The introduction of the I-PASS program reduced the omission of sensitive data and interruptions, without prolonging handover duration.
