Obesity is a multifactorial condition arising from complex interactions between genetic susceptibility, environmental exposures, and behavioural factors. Among the genetic contributors identified through genome-wide association studies, variants within the fat mass and obesity-associated (FTO) gene represent some of the most consistently replicated loci associated with body mass index, adiposity, and appetite regulation across populations. Experimental and observational evidence suggests that FTO variants may influence energy intake, food preference, and metabolic pathways through effects on hypothalamic signalling, adipocyte biology, and epigenetic regulation. These findings have stimulated interest in precision nutrition approaches that aim to tailor dietary strategies according to individual genetic profiles. This narrative review critically examines the role of FTO gene variants in polygenic obesity and evaluates the current evidence supporting gene-diet interactions relevant to personalized weight management. We synthesized data from mechanistic studies, observational cohorts, randomized controlled trials, and meta-analyses to assess whether dietary interventions, including macronutrient composition, mediterranean-style dietary patterns, and discretionary food intake, can meaningfully modify obesity risk in individuals carrying FTO risk alleles. While mechanistic plausibility and observational associations are well established, evidence from intervention studies indicates that genotype-specific responses are generally modest and context-dependent. Overall, current findings support the potential of precision nutrition as a complementary framework rather than a deterministic approach to obesity management. Further large-scale, long-term, and ethnically diverse intervention studies are required to clarify clinical utility and inform evidence-based implementation.

Sarcopenia is a progressive skeletal muscle disorder characterized by the loss of muscle mass and strength. The concept of pro-anabolic modulators (including vitamin D, leucine, omega-3 fatty acids, and probiotics) as nutritional agents to counteract sarcopenia has been introduced as a promising strategy to restore anabolic balance in aging muscle. This systematic review aimed to synthesize recent evidence on the effectiveness of these compounds on muscle mass and physical performance. A total of 53 randomized controlled trials were included: 30 evaluated vitamin D, 8 leucine, 9 omega-3, and 6 probiotics. Across studies, although results for vitamin D were heterogeneous, daily supplementation suggested a more consistent potential for beneficial effects compared to bolus regimens, particularly when co-administered with other agents or physical exercise. Leucine demonstrated greater efficacy when combined with resistance training or other pro-anabolic agents. Most studies on omega-3 fatty acids reported improvements in muscle strength and functional outcomes, especially in long-duration interventions. Probiotics also showed promising results, with almost all studies reporting positive effects on muscle mass and strength, despite variability in strains and protocols. Given the low to very low certainty of evidence for most outcomes (except for physical performance, which reached moderate certainty), these results should be interpreted with caution, despite a general trend toward favorable outcomes. These findings suggest that combining pro-anabolic modulators (or pairing them with exercise or additional nutrients) may enhance their efficacy on muscle-related outcomes. Further research is warranted to define optimal protocols and to clarify the mechanisms underlying their potential synergistic effects.

Despite increasing efforts to promote and support breastfeeding, the United States continues to have some of the lowest exclusive and sustained breastfeeding rates globally. Foreign-born immigrants and refugees specifically have been reported to have high initiation but low exclusive breastfeeding (EBF) rates. This scoping review aims to explore what is known about strategies to support breastfeeding among foreign-born mothers in the United States using the Arksey & O’Malley framework for scoping reviews and PRISMA-ScR (Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews). Six databases were searched using a comprehensive search strategy and 2103 articles were identified, of which 31 met eligibility criteria and discussed 33 specific breastfeeding interventions. The articles describe a range of interventions, including breastfeeding promotion and education (n = 30), hands-on breastfeeding support (n = 9), material support like giving breastfeeding supplies and food (n = 16), social support (n = 18) and social marketing campaigns (n = 1) to promote community support for breastfeeding. Common strategies for implementing these interventions include individual counselling (n = 21), group breastfeeding education (n = 17), informational materials (n = 12) and family support promotion (n = 11). In total, 87·3% of the immigrant mothers targeted by these breastfeeding support interventions were Hispanic, and 4·5% and 7·0% were mothers of African and Asian descent, respectively. This study reveals limited data and key gaps in efforts to preserve the culture of breastfeeding and promote EBF among multicultural immigrant and refugee families, particularly non-Hispanic groups. Addressing these gaps will improve optimal infant feeding practices among foreign-born mothers in the United States and, consequently, maternal and infant health outcomes.

This systematic review examined the associations of dietary factors such as nutrients, food intake, dietary patterns and dietary biomarkers with structural and functional brain MRI biomarkers, focusing on macrostructural, microstructural, lesion and perfusion measures, and functional activity/connectivity. Articles published in English were systematically searched in PubMed, Embase and PsycInfo up to 19 July 2024. A total of thirty-eight prospective cohort studies (twenty-three cross-sectional and fifteen longitudinal analyses) and thirteen intervention studies were included. Cross-sectional analyses revealed heterogenous associations: baked fish correlated with larger hippocampal volumes (β = 0·21), while oily fish, dairy products and tofu adversely related to ventricle grade. Pro-inflammatory dietary patterns were positively associated with silent infarct risk (DII Q4 v. Q1, OR = 1·77), whereas anti-inflammatory patterns tended to favour brain preservation. Longitudinal studies demonstrated more consistent protective associations: green tea consumption (+100 mL/d) reduced hippocampal atrophy by 0·024%/year, prudent dietary patterns preserved +203 mm3 left hippocampal volume over 4 years and higher plasma carotenoids decreased medial temporal lobe loss by 0·02 cm3/year. However, null findings were common across multiple dietary factors. Interventions showed limited structural benefits (effective in only two of six studies), while polyphenol-rich supplements more consistently improved cerebral perfusion and functional connectivity. Longitudinal and intervention studies demonstrated more consistent patterns than cross-sectional analyses; however, current evidence remains limited for clinical translation. Findings from cross-sectional analyses, despite being from prospective cohorts, require careful interpretation. Further replication across diverse populations and standardised long-term studies are needed before translating these associations into clinical practice.

Obesity pathophysiological conditions and obesogenic diet compounds may influence brain function and structure and, ultimately, cognitive processes. Animal models of diet-induced obesity suggest that long-term dietary high fat and/or high sugar may compromise cognitive performance through concomitant peripheral and central disturbances. Some indicated mechanisms underlying this relationship are discussed here: adiposity, dyslipidaemia, inflammatory and oxidative status, insulin resistance, hormonal imbalance, altered gut microbiota and integrity, blood–brain barrier dysfunction, apoptosis/autophagy dysregulation, mitochondrial dysfunction, vascular disturbances, cerebral protein aggregates, impaired neuroplasticity, abnormal neuronal network activity and neuronal loss. Mechanistic insights are vital for identifying potential preventive and therapeutic targets. In this sense, flavonoids have gained attention due to their abundant presence in vegetable and other natural sources, their comparatively negligible adverse effects and their capacity to cross the blood–brain barrier promptly. In recent years, interventions with flavonoid sources have proven to be efficient in restoring cognitive impairment related to obesity. Its modulatory effects occur directly and indirectly into the brain, and three fronts of action are highlighted here: (1) restoring physiological processes altered in obesity; (2) promoting additional neuroprotection to the endogenous system; and (3) improving neuroplasticity mechanisms that improve cognitive performance itself. Therefore, flavonoid consumption is a promising alternative tool for managing brain health and obesity-related cognitive impairment.

The health benefits of the long-chain omega-3 PUFA, eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) have been known for over 50 years and underpin the UK population recommendation to consume > 450 mg EPA+DHA per day. These recommendations, last revised in 2004, are based mainly on epidemiological evidence. Much research has been conducted in the interim. Most RCTs use doses of EPA+DHA of 840 mg per day or more. For anti-inflammatory, triglyceride lowering and anti-hypertensive effects, > 1.5 g EPA+DHA per day is needed. Cognitive benefits are also likely to require these higher intakes. Farmed salmon now contains considerably less EPA+DHA relative to wild-fish, and relative to farmed fish of 20 years ago, meaning one portion per week will no longer provide the equivalent of 450 mg EPA+DHA per day. Oily fish alone can only provide a fraction of the EPA+DHA required to meet global needs. Furthermore, there is low global oily fish consumption, with typical intakes of < 200 mg EPA+DHA per day, and limited intakes in vegans and vegetarians. Therefore, there is an urgent need for affordable, acceptable, alternative EPA+DHA sources, including vegan/vegetarian friendly options, such as bio-enriched poultry, red meat and milk products; fortified foods; enriched oilseeds e.g. genetically modified Camelina sativa; algae and algal oils; and approaches which enhance endogenous EPA/DHA synthesis. In this narrative review we suggest that current EPA+DHA intake recommendations are too low, consider EPA/DHA from a holistic health-sustainability perspective, and identify research, policy and knowledge mobilisation areas which need attention.

Firefighters face significantly elevated cancer risks due to chronic exposure to carcinogenic fire effluents and occupational stressors. In 2022, the World Health Organization classified firefighting as a carcinogenic occupation, linking it to increased incidences of cancers, including mesothelioma, bladder, prostate, colon and melanoma. Drawing on UK-specific data where possible, this narrative review explores how dietary strategies, particularly the Mediterranean diet, may complement existing protective measures in mitigating these risks. It investigates specific food-based nutrients that show promise in addressing risks associated with fire effluent contaminants, examining nutrient-mediated mechanisms and their relevance to firefighter health. The review also highlights the distinct combination of challenges firefighters face in adopting healthier dietary patterns, including disrupted routines, group eating cultures and gaps in nutritional education. While the evidence for firefighter-specific dietary interventions is still emerging, this review highlights the potential of sustainable dietary strategies to significantly reduce cancer risks and improve long-term health outcomes. Finally, it calls for targeted research and interventions to refine these strategies and deliver tangible health benefits for firefighters worldwide.

To achieve infectivity, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the virus responsible for COVID-19, must first traverse the upper respiratory tract mucosal barrier. Once infection is established, the cascading complexities of the pathophysiology of COVID-19 makes intervention extremely difficult. Thus, enhancing the defensive properties of the mucosal linings of the upper respiratory tract may reduce infection by SARS-CoV-2 and indeed by other viruses such as influenza, which have been responsible for the two major pandemics of the last century. In this review we summarise potential opportunities for foods and nutrients to promote an adequate mucosal immune preparedness with an aim to assist protection against infection by SARS-CoV-2, to maximise the mucosal vaccination (IgA inducing) response to existing systemic vaccines, and to play a role as adjuvants to intranasal vaccines. We identify opportunities for vitamins A and D, zinc, probiotics, bovine colostrum and resistant starch to promote mucosal immunity and enhance the mucosal response to systemic vaccines, and for vitamin A to also improve the mucosal response to intranasal vaccination. It is possible that an entirely different virus may in the future, by way of convergent evolution, utilise a similar upper respiratory tract infection pathway. A greater research focus on mucosal lymphoid immune protection in partnership with nutrition would result in greater preparedness for such an event.

Entomophagy (eating edible insects) could potentially address human deficiencies of iron, zinc and vitamin B12. This article aims to summarise available evidence about the iron, zinc and vitamin B12 content of raw and processed edible insects and compare these with the nutritional needs of different human life stages. A systematic literature search using specific keywords (edible insects, iron content, zinc content, vitamin B12 content and nutritional composition) in Web of Science and Scopus databases was performed. Forty-six studies were reviewed. To ensure standardised comparisons, articles with nutrient-enriched edible insects were excluded. The quality of records was assessed using standardised protocols. Results indicate that edible insects are generally either 'sources of' or 'rich in' iron, zinc and vitamin B12 required for optimal nutrition and health of different human life stages. Moreover, iron, zinc and vitamin B12 contents of edible insect species were generally either comparable to or higher than that of (lean) beef, (lean) pork, poultry and kidney beans. Most insect species were oven processed with little/no species-specific data for other processing methods. Variations in micronutrient content existed between processing methods and among oven-processed edible insects. Data inaccuracies, poor data quality control and lack of insect-specific official analytical methods contributed to fairly high variations and made comparisons difficult. Based on available data, edible insects can potentially address human deficiencies of iron, zinc and vitamin B12 despite the observed variations, data gaps and lack of edible insect matrix-specific official methods, in addition to limited human bioavailability and efficacy studies.

At present, the treatment of Alzheimer's disease involves only symptomatic medications which have continually demonstrated little efficacy, primarily due to the presence of biological barriers. Despite efforts, researchers have yet to discover a therapeutic treatment that delays neurodegenerative progression or restores associated Alzheimer neuropathological processes. For centuries, Hericium erinaceus (HE) has been used predominantly in Asian countries for its culinary and medicinal purposes; however, this mushroom has not yet been utilised in western pharmacology. This review systematically investigates evidence pertaining to the use of HE as a potential future therapeutic treatment for the prevention and delayed progression of Alzheimer's disease, by highlighting any fundamental neurotrophic and neuroprotective properties. In total, three human clinical trials and thirteen animal-model studies were included in review. The use of HE demonstrated positive significant differences in results obtained from behavioural, histological and biochemical assessments from both human clinical trials and animal model studies accentuating its utility for the improvement of cognitive function. In addition, erinacine-A-enriched HE appears to demonstrate the highest bioactive potency of all HE extracted compounds, providing the greatest effects while also showing transportability ease across biological barriers. In conclusion, evidence suggests that intake of HE may be an appropriate and relevant future therapeutic treatment for the prevention and delayed progression of Alzheimer's disease; however, continued research is necessary to provide further significant evidence of this relationship, through an increased quantity of human clinical trials.
