Long-term forecasting of blood donations using time series of donation activity: Findings from seven blood establishments.
To meet the long-term demand for blood products while preserving donor health in the long run, blood establishments must recruit a sufficient number of new donors annually. To determine what will suffice, it is essential to be able to forecast future blood donation volumes as a function of new donors using estimates based on historical donation activity. Donor (n = 11,629,873) and donation data (n = 64,510,294) extracted from operational information systems of seven blood establishments were transformed into anonymous donation activity data (grouped by blood establishment, sex and blood group) in the form of time series of mean number of donations per donor, indexed by years since first donation. Linear models were fitted to the time series using ordinary least squares. Out of the various estimated models, the best fit (mean R2 over blood establishments 99.95%) for past mean donation activity was achieved when regressing the logarithm of cumulative donation activity on the logarithm of years since first donation and an indicator variable for the year of first donation. In addition to the predictions, comparison of the estimated parameters revealed that there are significant differences between blood establishments, translating to differences in the expected number of donations accumulated over the lifetime of a donor. Average donation activity can be modelled using a few variables and simple models, with high precision. The estimates can be used to create forecasts of future donation volumes, which in turn can be useful in long-term blood donation management.

Endothelial dysfunction is central to COVID-19 pathophysiology, contributing to vascular complications and disease progression. However, the mechanisms driving disease evolution and response to endothelial-targeted therapies remain unclear. This study characterizes endothelial activation throughout the course of acute COVID-19 and evaluates the response to potential therapeutic agents.Serum samples from patients admitted due to moderate to severe COVID-19 pneumonia were prospectively collected on three study time points (+1 day, +4 days and +10 days). Human microvascular endothelial cells were cultured in medium supplemented with pooled serum within the same disease stage, with or without defibrotide, apixaban, or tocilizumab. Endothelial activation was assessed by immunofluorescence and quantitative mRNA expression of adhesion molecules, extracellular matrix (ECM) proteins, and innate immunity receptors. ECM reactivity was evaluated using a platelet adhesion assay. Intracellular signaling pathways were analyzed by immunoblotting.One-hundred and two patients were included. Compared to healthy donor plasma, patient plasma induced upregulation of Vascular cell adhesion molecule-1, Toll-like receptor 4, and von Willebrand factor in endothelial cells. This effect decreased over admission days and in response to drugs. Similarly, ECM reactivity was highest at admission and declined as the disease progressed. Vascular-endothelial cadherin was mildly downregulated, but its expression was unaffected by drug treatment in vitro. Defibrotide mitigated COVID-19 serum induced p38MAPK and Erk activation but enhanced Akt phosphorylation.Serum from severe COVID-19 patients induces a proinflammatory and prothrombotic endothelial phenotype, which can be modulated by endothelial-targeted therapies. These findings support the potential clinical value of endothelial-directed treatments.

At the Hospital Universitari Arnau de Vilanova Lleida, Spain, nursing staff utilize different transfusion safety systems (TSSs) depending on their clinical unit. These range from a basic model involving wristband-based patient identification to a more advanced system incorporating a Personal Digital Assistant (PDA), location beacon, physical barriers, specific wristbands and personal identification. This study aimed to compare nursing staff satisfaction across the various TSSs used. In March 2025, nursing staff were invited to voluntarily and anonymously complete an online, self-designed questionnaire assessing their perceptions of the systems in use. A total of 120 nurses participated, representing 15.6% of the total population. While overall satisfaction did not differ significantly by system type, 40.6% of respondents reported a negative perception. Users of the basic system felt less safe (65.4%) and considered it less effective at preventing errors (64.1%) than those using the complex system (87.2% and 92.8%, respectively). However, the complex system was associated with more frequent technical or usability issues (66.7%) compared with the basic system (34.6%), which contributed notably to dissatisfaction. Although specific factors influencing satisfaction were identified, no clear preference emerged regarding overall system satisfaction. The variability in responses may reflect the subjective nature of satisfaction. An optimal TSS would combine adaptability to staff needs, perceived safety, ease of use, intuitive visual cues, time efficiency and high reliability with minimal technical complications.

Calculating the collection efficiency (CE) of CD34+ cells in leukocytapheresis (LVL) is critical to understanding the performance of cell collection. Traditional CE values are typically based on both pre- and post-apheresis calculations of the pooled cells (CE1), or they may be based only on the pre-procedure pool calculation (CE2) and total blood volume (TBV) processed. However, determining the actual CD34+ concentration in peripheral blood (PB) during LVL is more problematic. New crude CE (CruCE) takes into account the recruitment and extraction of CD34+ cells during LVL. This study aims to retrospectively compare traditional (CE2, CE1) and CruCE and to assess the variables that influence new CruCE-including donor characteristics, procedure characteristics, and device setting parameters-in both large-volume LVL and non-LVL. Data from adult donors who underwent autologous and allogeneic collections of PB stem cells between January 2020 and April 2023 were retrospectively included. Our series comprises 123 autologous and 37 allogeneic LVL procedures. The median of CD34+ CE2, CE1, and CruCE was 51%, 65%, and 74% in autologous donors and 58%, 69%, and 76% in allogeneic donors. In multivariate analysis, TBV processed and prior CD34+ cell count in PB showed a positive and a negative correlation with CruCE in autologous and allogeneic settings, respectively. In conclusion, CruCE may be closer to actual CE in comparison to CE2 and CE1. LVL and low CD34+/μL prior to the procedure may enhance CD34+ cell CE.
