Facteurs associés à la prise en charge en centre de référence ou en centre de compétence pour les maladies rares : une étude nationale observationnelle
Les maladies rares sont définies comme des maladies avec une prévalence inférieure ou égale à 1/2000. Plusieurs types d’établissement peuvent prendre en charge ces conditions, les plus spécialisés étant les établissements de référence. L'objectif de cette étude a été d’étudier les facteurs liés aux patients et aux territoires qui pourraient influer sur la probabilité d'une prise en charge en centre spécialisé (de référence ou de compétence) et d’évaluer la variabilité dans l'accès aux soins. Il s'agit d'une étude observationnelle nationale conduite entre le 1 janvier 2015 et le 31 décembre 2019. La probabilité d'un séjour en centre de référence ou de compétence a été étudiée par un modèle de régression logistique, avec ajustement sur l’âge, le sexe, la saison, la distance au centre, l'indice de défavorisation sociale FDep, et le score de Charlson. Les variations d'accès aux différents types de centres ont été quantifiées par la méthode du Systematic Component of Variation (SCV). Les patients hospitalisés en centre de référence ou de compétence étaient plus jeunes (Odds Ratio pour 5 ans supplémentaires 0.87, IC95% 0.87 - 0.88), plus souvent de sexe masculin (OR 1.08, IC95% 1.01-1.16), habitaient plus loin de l'hôpital concerné (OR pour 10km supplémentaires 1.12, IC95% 1.11-1.12), étaient moins souvent résidents de zones socialement défavorisées (OR 0.90, IC95% 0.88-0.92), avec un effet saisonnier (hospitalisations plus fréquentes en été). Le SCV était plus élevé pour les centres de compétence (17.61) que pour les centres de référence (7.35). L'accès aux centres de référence n'est pas également réparti sur l'ensemble du territoire français, et les patients plus aisés avaient un meilleur accès aux soins les plus spécialisés. La communication sur l'existence de moyens de transports adaptés et remboursés pour ces prises en charges à distance du domicile doit être renforcée.

Factors associated with deferral or non-performance of an organized breast cancer screening program during the COVID-19 pandemic in France.
Delays in detection and treatment of breast cancer can lead to increased mortality. To date, participation in organized breast cancer screenings (OBCS) has been suboptimal worldwide. The objective of this study was to investigate the factors associated with deferral or non-performance of mammography during the COVID-19 pandemic for women who had previously participated in OBCS. A retrospective observational study was conducted on a cohort of 6282 women from the Aube Department of France, who were invited to an OBCS in 2020. Participants were divided into women who promptly underwent screening after receiving an invitation (between 22 and 25 months elapsed since the last mammogram), women who underwent late screening (≥26 months since the last mammogram), and those who were never screened. Data were collected from a self-reported questionnaire. Comparative and multivariable analyses modeling the probability of each type of attendance were performed using these data. In total, 2301 women (aged 50-74 years) returned a valid questionnaire. Compared to women who promptly underwent mammography, non- and late-screening participants were younger, had less frequent gynecological follow-up and a less frequent history of colorectal cancer screening. Women with higher education status and those residing in socially disadvantaged areas were more likely to attend late. The absence of regular gynecological follow-up and the absence of colorectal cancer screening were significant factors associated with deferral of or non-attendance at OBCS.

Mediastinal tumors are exceedingly rare during fetal development, presenting significant diagnostic challenges and potentially leading to severe outcomes such as stillbirth or metastatic disease if not promptly identified and managed. Pleuropulmonary blastomas are primitive mesenchymal tumors often linked to mutations in the DICER1 gene, indicating a hereditary pattern associated with other common adult neoplasms with dominant inheritance. This report describes a case involving a 20-year-old Caucasian woman whose pregnancy was complicated by a stillbirth in the second trimester. Initial suspicions of a mediastinal tumor arose from blood tests and ultrasound examinations during pregnancy surveillance. However, the definitive diagnosis of a type II pleuropulmonary blastoma was established through a pathological examination at autopsy. This case underscores the complexities of diagnosing fetal mediastinal tumors and contributes to the sparse literature on neonatal pleuropulmonary blastomas. Our comprehensive review of the differential diagnoses and literature emphasizes the unique characteristics of pleuropulmonary blastoma and its similarities to other soft tissue sarcomas, enhancing understanding of their clinical and genetic profiles.
