A Comparison of the Effectiveness of Antibiotic Therapy and Surgical Treatment in Patients with Acute Appendicitis
Acute appendicitis is one of the most common causes of abdominal pain requiring emergency surgical intervention. While appendectomy has long been the standard treatment, recent evidence has explored the use of antibiotics as a non-operative alternative, particularly for uncomplicated cases. In this study, we want to compare the effectiveness, safety, and long-term outcomes of antibiotic therapy versus surgical appendectomy in the management of acute appendicitis. A comprehensive review of recent randomized controlled trials (RCTs), including the APPAC and CODA trials, as well as meta-analyses, was conducted to evaluate treatment success rates, recurrence, complications, costs, and patient preferences. Results: Antibiotic therapy successfully avoided surgery in most of the cases (approximately 73%) of patients within one year,. However, recurrence rates reached up to 39%. Surgical treatment was associated with lower recurrence (<5%) but higher direct costs and longer recovery. Antibiotic therapy resulted in shorter hospital stays and reduced overall treatment costs. Patient-centered decision-making was found to play a crucial role in selecting the appropriate therapy. Conclusions: Antibiotic therapy represents a safe and cost-effective alternative to appendectomy for selected patients with uncomplicated acute appendicitis. However, shared decision-making and proper patient selection are essential to optimize outcomes and minimize risks. Further research is needed to refine the criteria for non-operative management and ensure long-term safety. During muscle work irisin is produced, which plays an important role in the formation of neurons and exhibits an anti-inflammatory effects. This review also looks at the aspect of maintaining proper composition of the gut microbiota, whose imbalance can promote the process of neuronal inflammation and cognitive dysfunction. Attention is also given to issues of training intensity and the effect of of exercise on blood flow in the brain. All of these elements play an important role in preventing and slowing down neurodegeneration.

The development of chronic tubulointerstitiаl nephritis in the course of gout leаds to ischemiа of the renаl pаrenchymа, аctivаtion of the renin-аngiotensin-аldosterone system (RааS), sodium аnd wаter retention in the body, which leаds to the development of hypertension. The kidneys, on the other hаnd, аre responsible for the metаbolism аnd excretion of uric аcid from the body, so their dаmаge аccordingly leаds to the deteriorаtion of this process. In pаtients with gout, the level of uric аcid determines the severity of gouty аrthritis, dаmаge to blood vessels аnd joints. Therefore, it is extremely importаnt to know the specifics of uric аcid metаbolism in pаtients with gout, since the effectiveness of the prescribed hypouricemic therаpy depends on it. Aim of the studyThe аim of the study wаs to аnаlyze the metаbolism of uric аcid in pаtients with gout аnd concomitаnt аrteriаl hypertension, аnd to determine the impаct of concomitаnt metаbolic disorders on the excretory function of the kidneys. ConclusionThe results of the conducted reseаrch prove thаt: 1. Pаtients with gout with concomitаnt hypertension hаve more pronounced metаbolic disorders thаn pаtients with gout without hypertension. This is mаnifested by impаired cаrbohydrаte tolerаnce, аn increаse in totаl cholesterol, triglycerides аnd а decreаse in HDL levels.2. Pаtients with gout аnd hypertension hаve deeper renаl dysfunction, which is mаnifested by а tendency to decreаse GFR аnd increаse creаtinine levels compаred to pаtients with gout without hypertensive syndrome.3. Violаtion of uric аcid metаbolism in pаtients with gout аnd аrteriаl hypertension is mаnifested by аn increаse in the reаbsorption process аnd а decreаse in the filtrаtion process of uric аcid, which results in higher rаtes of uricemiа.

Abemaciclib combined with adjuvant endocrine therapy in patients with high risk early breast cancer who received neoadjuvant chemotherapy (NAC).
517 Background: monarchE, a phase 3, open-label, randomized study evaluating abemaciclib combined with adjuvant endocrine therapy (ET) compared to ET alone in patients with HR+, HER2-, high risk early breast cancer (EBC), resulted in a statistically significant improvement in invasive disease-free survival (IDFS) (HR = 0.713; 95% CI: 0.583, 0.871). NAC is used in patients with HR+, HER2- EBC at higher risk of recurrence despite often limited response, suggesting a need for enhanced adjuvant ET. Methods: Patients with ≥4 positive notes (LNS), or 1-3 LNS and either Grade 3 disease, tumor size ≥5 cm, or central Ki-67 ≥20% were eligible. Prior chemotherapy (NAC, adjuvant, none) was one of the stratifications factors. Prior therapy and tumor characteristics prior to study entry were collected. Here, we present the results of the prespecified subgroup of patients who received NAC. Results: Out of 5,637 randomized patients, 2056 (36.5%) received NAC. For 84.8%, the chosen regimen included anthracycline + cyclophosphamide + taxane, for 4.4%, it included anthracycline + taxane. A total of 1044 (50.8%) patients who received NAC had a radiologic tumor size between 2-5 cm and 599 (29.1%) had tumors ≥5 cm at diagnosis. 6.2%, 49.4% and 36.7% of patients had tumors with histologic Grade 1, 2, 3 respectively. 55.2% of patients had LNS ≥4+ and 44.4% had 1-3+ LNS. Central Ki-67 prior to NAC was ≥20% in 64.8% of patients with available Ki-67 results (664 (32.3%) patients had missing Ki-67 results). Evaluation of clinical and pathological measures of response will be presented. A multivariate cox regression analysis of IDFS in the intent-to-treat (ITT) population, identified prior chemotherapy as prognostic, suggesting patients who received NAC were at risk of a worse outcome. Primary outcome efficacy data for patients who received NAC are shown in the table below. Abemaciclib + ET demonstrated treatment benefit in terms of IDFS vs ET alone (HR: 0.614 95% CI: 0.473, 0.797) with 2-year IDFS rates of 87.2% vs 80.6%, respectively. The addition of abemaciclib to ET resulted in an improvement in distant relapse-free survival (DRFS) (HR: 0.609, 95% CI: 0.459, 0.809), with 2-year DRFS rates of 89.5% and 82.8%, respectively. Safety profile was similar to the overall safety population. Conclusions: Patients with HR+, HER2- EBC who received NAC were noted to be at a higher risk of recurrence. In this subgroup, abemaciclib combined with ET demonstrated a clinically meaningful treatment benefit in IDFS and DRFS, which was numerically greater than in the ITT population. The regional policy pursued today is based on actions aimed at integrated development of local government units, including more aspects of functional and spatial connections. It leads to a change in the approach to the assessment of local capital available in rural areas in a broad sense. One of the specific examples of rural areas where the potential of creation depends on endogenous and exogenous factors are the suburban areas which, according to the new paradigm, are called cities functional areas. The article attempts to assess the developmental disparities resulting from the potential of the resources available in the areas of interaction of the two urban centres: Wrocław and Kielce. Regions selected for the purpose of a comparison are characterised by different demographic and economic potential conditions, as well as by the different conditions and historical location. What they have in common is the administrative functionality, which results from the rank of province capital. The article aimed at establishing the main determinants of rural development within the range of the strongest influence of the urban center. For the study adopted 18 municipalities in the zone of influence of the central city (after 9 for each center). Time range assumed for 2002 and 2014. Diagnostic variables were divided into 3 categories of local capital: social resources; economic resources; and spatial resources. The research process showed slight disproportions in terms of baseline year (2002) between the units constituting each suburban area. Municipalities centered around Kielce and Wroclaw were characterised by a similar level (assessment based on synthetic index) mainly forspatial resources. In the case of economic and social resources disproportions were higher but they do not have a significant influence on the final assessment of its potential. Data analysis in 2014 showed an increase in disparities of development in almost all of the analysed areas. This in turn contributed to the final assessment of the actual level of socio-economic changes in the suburban areas. As noticed in the article, the transformations in the areas bordering with Wrocław and Kielce were generated mainly by changes in the area of social and economic resources. The dynamics and scope of this transformation was faster in the influence zone of Wrocław. Therefore, it has been indicated that the level of socio-economic changes is to a larger extent a result of the size and range of central city and, thus, the relations between the city and its immediate surroundings.

Granulomatosis with polyangiitis (GPA) is an idiopathic vasculitis of medium and small arteries, characterized by necrotizing granulomatous inflammation. GPA typically affects upper and lower respiratory tract with coexisting glomerulonephritis. This disease is generally characterized by antineutrophil cytoplasm antibodies (ANCA), nevertheless, there are rare cases with negative ANCA. GPA affects people at any age, with predominance of the sixth and seventh decade of life. In 80%–95% of the patients the first symptoms of GPA are otorhinolaryngological manifestations of head and neck including nose/sinuses, ears, eyes, larynx/trachea, oral cavity, and salivary glands. Diagnosis of GPA is based on Criteria of the American College of Rheumatology. In clinical practice diagnosis, the presence of distinctive ANCA antibodies and biopsy of affected organ are crucial. GPA must be differentiated from neoplastic, infectious or inflammatory ulcerative lesions of the head and neck. The standard treatment procedure is divided into two essential phases, induction and maintenance. The induction phase is based on combination of systemic corticosteroid and immunosuppressant therapy, whereas the maintenance phase comprises corticosteroids and azathioprine/methotrexate supplementation. Surgical treatment ought to be considered for patients who are not responding to pharmacotherapy.
