A rare case of giant hemangioma combined with actinomycosis infection.
Hemangiomas are primarily associated with neovascularization and the abnormal proliferation of blood vessels. No cases of hemangioma combined with actinomycosis have been reported in the literature. We present a case of a 54-year-old man with a rapidly enlarging left upper limb mass after two traumatic injuries. Pathological examination performed after surgical resection of the mass at our center revealed a hemangioma complicated by actinomycosis. This case indicates that actinomycetes may colonize hemangiomas and contribute to their continuous enlargement, with excellent prognosis after surgery.

BackgroundPreservation of normal-quality infrapatellar fat pads (IPFP) during total knee arthroplasty (TKA) has been proposed, and IPFP of abnormal quality are recommended for resection.MethodsIn this trial, patients with knee osteoarthritis (OA) undergoing TKA were stratified by IPFP status on magnetic resonance imaging (Group 1 with normal IPFP [signal intensity score ≤1; ranging from 0 to 3]; Group 2 with abnormal IPFP [signal intensity score ≥2]) and randomly assigned in a 1:1 ratio to IPFP preservation or IPFP resection. The primary outcome was 12-month change in the mean of five Knee Injury and Osteoarthritis Outcome Score (KOOS) subscales (KOOS5), which range from 0 (worst) to 100 (best).ResultsIn Group 1, 179 participants were randomly allocated to IPFP preservation (n=90) and IPFP resection (n=89), and 198 participants were randomly assigned to IPFP preservation (n=101) and IPFP resection (n=97) in Group 2. The increases in KOOS5 score were 38.1 and 36.1 in Group 1 and were 39.2 and 37.5 in Group 2 in IPFP preservation and IPFP resection groups, respectively, resulting in between-group differences of 1.9 (95% confidence interval [CI], −1.7 to 5.6) in Group 1 and 1.6 (95% CI, −1.3 to 4.6) in Group 2. Adverse events were predominantly nonserious musculoskeletal or skin-related events (three vs. two in Group 1 and eight vs. two in Group 2).ConclusionsResection of abnormal IPFP and preservation of normal IPFP did not improve postoperative outcomes 12 months after TKA. Read moreImmune escape mechanisms revealed by multifunctional nanoprobes targeting PD‐L1 in hepatic cancer progression
Abstract Hepatocellular carcinoma (HCC) often arises from liver cirrhosis, yet real‐time, noninvasive monitoring of immune escape during this transition remains limited. We developed a multifunctional biomimetic nanoprobe (ICGB@Apt) by conjugating a programmed death‐ligand 1 (PD‐L1)–specific DNA aptamer (selected via Cell–SELEX) onto an indocyanine green/BMS‐684 polymer, enabling in vivo imaging and immune modulation. In CCl 4 ‐induced mouse models progressing from cirrhosis to HCC and in complementary in vitro 3D co‐culture systems, ICGB@Apt preferentially accumulated in tumor tissue, dynamically visualized PD‐L1, and mitigated disease progression. The probe enhanced CD8 + T‐cell activity (IFN‐γ and granzyme B), reduced fibrosis and tumor burden, and outperformed non‐aptamer controls (Apt+ICG or ICGB alone). Knockdown experiments and RNA‐seq highlighted the centrality of the programmed death‐1 (PD‐1)/PD‐L1 axis; immune deconvolution implicated CD8 + T cells, follicular helper T cells, and M0 macrophages in the cirrhosis‐to‐cancer shift. The nanoprobes showed favorable biocompatibility and enabled longitudinal tracking of primary and metastatic lesions. Together, these findings establish ICGB@Apt as a PD‐L1–targeted platform that simultaneously monitors and counteracts immune escape, offering translational potential for early detection and immunotherapy guidance in HCC.

Bioactive components and antimicrobial mechanisms of Momordica charantia L. against Cutibacterium acnes
Momordica charantia L. ( M. charantia L. ) is a widely cultivated crop with significant potential for non-food industrial applications. This study aimed to develop a plant-based antibacterial ingredient from its fruit targeting Cutibacterium acnes , a key pathogen in acne vulgaris. Bioassay-guided fractionation via liquid-liquid extraction and column chromatography yielded a bioactive subfraction, EAEMS (Fr 7.4), which showed potent anti- C. acnes activity with a minimum inhibitory concentration (MIC) of 0.25 mg/mL. UPLC-Q/TOF-MS and molecular networking analysis revealed a complex chemical profile rich in triterpenoids, steroids, and glycosides. In vitro and in silico studies on antibacterial mechanisms demonstrated that EAEMS alters bacterial growth kinetics, inhibits biofilm formation, disrupts cell wall and membrane integrity, and interferes with metabolic activity. Cytotoxicity assays confirmed that EAEMS is non-toxic to mammalian cells at 4 × MIC. Among 125 putative compounds, 15 were selected based on drug-likeness and toxicity prediction, with three exhibiting strong binding affinities to key bacterial targets in molecular docking. The absence of cytotoxicity combined with its multi-mechanism anti- C. acnes efficacy positions EAEMS as a promising candidate for developing plant-based anti-acne ingredients, supporting the high-value, non-food utilization of this industrial crop. • EAEMS fraction from Momordica charantia L. fruit exhibits potent anti-C. acnes activity. • Chemical characterization reveals triterpenoids and steroids as main constituents. • Multi-mechanism action disrupts membrane integrity, biofilm formation, and energy metabolism. • Three promising antibacterial candidates identified through in silico screening.
