Undervalued, misled, & isolated: foster parents’ experience of (mis)attunement within our compliance-centered system
Rest, shade, hydration and hygiene for the prevention of kidney injuries and inflammation in a Nicaraguan sugarcane worker cohort.
To study the effect of a progressively enhanced rest-shade-hydration-hygiene (RSHH) intervention on kidney injury and inflammation biomarkers, and rates of clinical acute kidney injury (AKI) in Nicaraguan sugarcane workers with a very high rate of chronic kidney disease of non-traditional origin (CKDnt). We analysed serum creatinine and C-reactive protein (CRP) and leukocyturia from samples obtained before and at the end of four harvest seasons (H1-4). An increase in creatinine≥0.30 mg/dL over the harvest was considered incident kidney injury (IKI). Rates of clinically diagnosed AKI were obtained from medical records. Each season the RSHH intervention included progressively longer and more frequent rest periods with improved access to shade and hydration, implementation monitoring, qualitative interviews and health outcome assessments. 1044 workers were followed for 1938 person-harvests. Among burned cane cutters, the job group with the highest workload and worst outcomes initially, there were decreasing rates of IKI (21% in H1 to 1% in H4, p<0.01), AKI (20/1000 worker-months to 8/1000 worker-months, p<0.01) and end-harvest leukocyturia (26% to 1%, p<0.01), and less rise in cross-harvest CRP (median 1.75-fold increase in H1 to 1.00 in H4, p<0.01). Kidney outcomes among outdoor heat-stressed workers at high risk of CKDnt improved as a structured RSHH intervention was implemented and committed to by workplace management. The findings support a causal relationship between occupational heat stress, kidney injury and CKDnt and point to possibilities for prevention.
Read moreNaturalistic patterns of sustained attention across early childhood: Developmental profiles and longitudinal associations with executive functions.
The capacity to sustain attention over time develops rapidly over early childhood and is associated with socioemotional and cognitive outcomes. However, sustained attention has largely been studied in either shared or independent contexts, leading to gaps in our understanding of how trade-offs between sustained attention to shared versus individual targets may predict later outcomes. We examined this question in a longitudinal sample of 1,290 children (49% female, 43% Black), living in predominately rural, low-income regions, using a naturalistic shared picture book reading task. Children's sustained attention to individual relative to shared targets during the book reading task was measured at 24 and 35 months. Using latent profile analysis, we identified four developmental profiles of children's attentional trajectories: three of the profiles differed in the extent that children' attention shifted toward more socially directed relative to individually directed attention; a fourth profile showed atypical decreases in both socially directed and individually directed attention across development. Importantly, heterogeneity in children's attentional profiles were associated with differences in executive functions at 48 months of age. Specifically, children who showed greater relative increases in socially directed attention had higher executive functions performance, whereas children with atypical decreases in attention showed substantial deficits in this domain. These findings reveal distinct longitudinal patterns of sustained attention in naturalistic contexts and show that heterogeneity in these patterns are robust predictors of subsequent executive functions. This person-centered approach provides novel insights into how quantitative and qualitative changes in attention may impact executive functions development and may help identify children at risk for nonnormative trajectories.

Read moreDemocratic Education in the Twenty-First Century (Part 4)
“Reconstructing Democratic Education” was an international conference held virtually on June 22, 2024. This article uses the words of the speakers and the participants to present some of the stories and reflections that stimulated us that day. Speakers touched on a wide variety of challenges that democracy in general and schools in particular are facing. We heard about an effort to reconstruct the teaching profession in Cambodia, a collaboration between third-graders and a college class for preservice teachers, a yearlong sequence of courses for teachers that evolves from the students’ life experiences, and a film arts program for educationally challenged students in British Columbia. Two periods of participant questions and comments produced lively conversations about the opportunities and dangers that mark our efforts to reconstruct democratic schools for the twenty-first century.

ABSTRACT This qualitative, narrative, retrospective study of adults (N = 21; mean age 27.8 years) with child welfare involvement in young adolescence explored the participants’ challenges in childhood and young adulthood. The sample had positive psychosocial functioning at age 18 years. Analysis identified three areas of challenges: Individual Challenges including mental health problems and shame/isolation; Family Challenges including child maltreatment by family members, lack of family support, and parentification; and System Challenges including housing instability, child welfare/case worker issues, and lack of a safety net. Even though these individuals had positive psychosocial functioning entering adulthood, their challenges were significant and required supportive services.

Group Interpersonal Psychotherapy for minoritized Head Start mothers with depressive symptoms: A mixed method study.
This study explores minoritized mothers' experiences in group interpersonal psychotherapy (IPT-G) and relates their experiences to treatment outcomes. Quantitative and qualitative data were gathered from 26 Latinx and Black mothers who participated in IPT-G. Mothers were divided into three groups: (1) not depressed at follow-up, (2) depressed at follow-up, and (3) those with subclinical symptoms throughout the intervention, and similarities and differences across groups were examined. Results showed that mothers not depressed at follow-up reported high levels of emotional safety in IPT-G, facilitating emotional processing. Mothers depressed at follow-up referenced the impact of stigma and had greater difficulty sharing their feelings and also reported lower socioeconomic status and higher levels of trauma. It appears that high levels of environmental stressors and difficulty developing trusting therapeutic relationships were related to experiencing depression at the conclusion of treatment. Alternatively, for many mothers, IPT-G provided within head start was an effective therapeutic option.

Photography is a medium that has historical roots in psychiatry. It is an art form that shares some concepts with psychodynamic psychotherapy and can also provide psychotherapeutic value. In addition, it can provide a means of stress reduction for the burned-out clinician. Photographs can also be utilised for educational activities to stimulate reflection and discussion.
