Update der ESC-Vorhofflimmerleitlinien 2024
Turnusmäßig sind die aktualisierten Leitlinien der European Society of Cardiology (ESC) zum Management von Vorhofflimmern 2024 erschienen. Trotz bedeutender Fortschritte in der Prävention, Diagnostik und Therapie ist das Vorhofflimmern die häufigste anhaltende Rhythmusstörung und hat einen großen Einfluss auf die Betroffenen, ihr Umfeld und das Gesundheitswesen. Die prägnanten Leitlinien fokussieren auf evidenzgestützte Empfehlungen und praktische Umsetzbarkeit einschließlich klarer Schaubilder und Behandlungspfade. Die ganzheitliche Betreuung des Patienten und seiner Komorbiditäten wird in einem umfassenden AF-CARE-Konzept in den Vordergrund gestellt. Es beinhaltet „C“ Komorbiditäten und Risikofaktorenmanagement, „A“ Vermeidung von Schlaganfall und Thrombembolien, „R“ Reduktion von Symptomen durch Rhythmus- oder Frequenzkontrolle und dynamische „E“ Re-Evaluation. Bei paroxysmalem Vorhofflimmern ist die Ablation jetzt Erstlinientherapie. Aktuelle Daten für Device-detektiertes Vorhofflimmern und Trigger-induziertes Vorhofflimmern wurden aufgenommen. Eine maximale Involvierung von Patienten u. a. durch parallel publizierte Patientenleitlinien wird zur gemeinsamen, informierten Entscheidungsfindung befürwortet.

BackgroundApproximately 30,000 people are affected by severe injuries in Germany each year. Continuous progress in prehospital and hospital care has significantly reduced the mortality of polytrauma patients. With increasing survival rates, the functional outcome, health-related quality (hrQoL) of life and ability to work are now gaining importance. Aim of the study is, the presentation of the response behavior of seriously injured patients on the one hand and the examination of the factors influencing the quality of life and ability to work 12 months after major trauma on the other hand. Building on these initial results, a standard outcome tool shall be integrated in the established TraumaRegister DGU® in the future.MethodsIn 2018, patients [Injury Severity Score (ISS) ≥ 16; age:18–75 years] underwent multicenter one-year posttraumatic follow-up in six study hospitals. In addition to assessing hrQoL by using the Short-Form Health Survey (SF-12), five additional questions (treatment satisfaction; ability to work; trauma-related medical treatment; relevant physical disability, hrQoL as compared with the prior to injury status) were applied.ResultsOf the 1,162 patients contacted, 594 responded and were included in the analysis. The post-injury hrQoL does not show statistically significant differences between the sexes. Regarding age, however, the younger the patient at injury, the better the SF-12 physical sum score. Furthermore, the physically perceived quality of life decreases statistically significantly in relation to the severity of the trauma as measured by the ISS, whereas the mentally perceived quality of life shows no differences in terms of injury severity. A large proportion of severely injured patients were very satisfied (42.2%) or satisfied (39.9%) with the treatment outcome. It should be emphasized that patients with a high injury severity (ISS > 50) were on average more often very satisfied with the treatment outcome (46.7%). A total of 429 patients provided information on their ability to work 12 months post-injury. Here, 194 (45.2%) patients had a full employment, and 58 (13.5%) patients were had a restricted employment.ConclusionThe present results show the importance of a structured assessment of the postinjury hrQoL and the ability to work after polytrauma. Further studies on the detection of influenceable risk factors on hrQoL and ability to work in the intersectoral course of treatment should follow to enable the best possible outcome of polytrauma survivors.

Cutaneous oxalosis is a rare manifestation of systemic oxalosis, typically associated with primary or secondary hyperoxaluria. We present a rare case of a 23-year-old female diagnosed with primary hyperoxaluria and end-stage renal disease, who presented with papules on the palms without any vascular complications. The skin can be affected by oxalate deposition, resulting in various manifestations such as vascular complications or calcified nodules. In our case, the patient had primary hyperoxaluria and end-stage renal disease but exhibited atypical features of cutaneous oxalosis. Histopathology confirmed the presence of oxalate crystals in the dermis, subcutis, and medium-sized arteries. The mechanism of oxalate deposition in this case remains unclear. This case underscores the importance of considering cutaneous oxalosis in the differential diagnosis of patients with renal failure and skin lesions, and highlights the variability of clinical presentations in primary hyperoxaluria.

Patients with potential or proven cardiovascular diseases represent arelevant proportion of the total spectrum in the emergency department. Their monitoring for cardiovascular surveillance until the diagnostics and acute treatment are initiated, often poses an interdisciplinary and interprofessional challenge, because resources are limited, nevertheless ahigh level of patient safety has to be ensured and the correct procedure has a major prognostic significance. This consensus paper provides an overview of the practical implementation, the modalities of monitoring and the application in aselection of cardiovascular diagnoses. The article provides specific comments on the clinical presentations of acute coronary syndrome, acute heart failure, cardiogenic shock, hypertensive emergency events, syncope, acute pulmonary embolism and cardiac arrhythmia. The level of evidence is generally low as no randomized trials are available on this topic. The recommendations are intended to supplement or establish local standards and to assist all physicians, nursing personnel and the patients to be treated in making decisions about monitoring in the emergency department.

Kardiologische Rehabilitation (CR) hat das Ziel, die individuell bestmögliche physische und psychische Gesundheit der Patienten mit Herzinsuffizienz durch die Therapie in einem multiprofessionellen Team wiederherzustellen, nachhaltig zu stabilisieren und die berufliche und soziale Re-Integration zu ermöglichen. Basis der CR sind körperliche Aktivität und Bewegungstherapie zur Verbesserung der Belastbarkeit im Alltag. Daneben spielt die psychosoziale Unterstützung zur Förderung der Krankheitsbewältigung, zur Reduktion von Ängstlichkeit und Depressivität mit dem Ziel einer Verbesserung der Lebensqualität eine große Rolle. Schulungen zur Steigerung der Selbstmanagementfähigkeiten im Umgang mit der Erkrankung, zur Erhöhung der Adhärenz für die Medikation und für einen gesunden Lebensstils sind ebenfalls wichtige Aufgaben des interdisziplinären Teams. Zu den ärztlichen Aufgaben gehören ganz wesentlich die Implementierung, Optimierung und Dosistitration der evidenzbasierten Herzinsuffizienzmedikation. Durch engmaschige klinische Kontrollen ist es in aller Regel sehr gut möglich, die 4 evidenzbasierten Medikamente für Herzinsuffizienz mit reduzierter Ejektionsfraktion (± Diuretikum) während einer durchschnittlichen Verweildauer von 3 bis 4 Wochen in der CR zu vervollständigen und ggf. die Dosierung dieser Medikamente zu steigern („big 4 in 4 weeks“). Die Indikation zur CR ist bei Patienten mit Herzinsuffizienz in allen nationalen und internationalen Leitlinien (ACC/AHA, ESC, AWMF, NVL) fest etabliert. Eine Verringerung der Gesamtmortalität konnte bisher nicht sicher belegt werden, und die Ergebnisse zur Reduktion der stationären Wiederaufnahme sind heterogen. Die körperliche Belastbarkeit und die krankheitsbezogene Lebensqualität werden durch die Teilnahme an einer CR signifikant verbessert. Ein frühzeitiger Beginn der CR in der unmittelbar poststationären Phase ist wichtig, damit kurzfristige Dekompensationen mit erneuter Krankenhausaufnahme (Drehtüreffekt) vermieden werden. Der Erfolg der CR wird maßgeblich durch die Auswahl geeigneter Patienten und die Adhärenz zu dieser nichtpharmakologischen Therapie bestimmt. Dabei sollten Alter, Gebrechlichkeit oder der Erwerbsstatus keine Ausschlusskriterien für eine trainingsbasierte CR sein. Für den langfristigen Erfolg bietet die Fortführung der multiprofessionellen Therapie am Wohnort in den Herzinsuffizienzgruppen neue Möglichkeiten. Der weitere Ausbau der Herzinsuffizienznetzwerke bietet die Chance, die CR als festen Bestandteil in die Therapie der Patienten mit Herzinsuffizienz zu integrieren.
