Executive Dysfunction and Working Memory in Borderline Personality Disorder and Their Association with Impulsivity and Self-injurious Behavior: A Cross-sectional Study.
Borderline Personality Disorder (BPD) features impulsivity, emotional dysregulation, and self-injurious behaviors (SIB). Cognitive deficits, such as executive dysfunction (ED) and working memory (WM) impairments, may drive these symptoms, yet their roles remain underexplored. This study examined relationships among ED, WM, impulsivity, and SIB in BPD to identify cognitive predictors of these behaviors. This cross-sectional study assessed 30 individuals with BPD per DSM-5 criteria. Participants completed the Barratt Impulsiveness Scale (BIS-11) for impulsivity, Deliberate Self-Harm Inventory (DSHI) for SIB, Wisconsin Card Sorting Test (WCST) for ED, and Letter-Number Sequencing (LNS) task for WM. Spearman's correlations and regression analyses evaluated associations between cognitive deficits, impulsivity, and SIB. Elevated impulsivity was linked to more WCST perseverative errors (ρ = 0.45, p = .02) and lower LNS scores (ρ = -0.38, p = .04). SIB correlated with ED (ρ = 0.41, p = .03) but not WM (ρ = -0.20, p > .05). High-impulsivity participants had greater WCST errors (median = 20 vs. 11, H = 8.12, p < .05) and lower LNS scores (median = 6 vs. 10, H = 7.33, p < .05) than low-impulsivity peers. Regression confirmed BIS-11 scores predicted WCST errors (β = 0.40, p < .01) and LNS performance (β = -0.35, p < .05). The ED and WM impairments were linked to impulsivity, with ED also tied to SIB. Cognitive remediation may reduce these behaviors. Lacking a control group, longitudinal research is needed.

Impact of telemedicine in STEMI care system: A five-year experience from Tamil Nadu, India.
Background & objectives Timely reperfusion reduces mortality in ST-segment elevation myocardial infarction (STEMI). Telemedicine and social media have been shown to improve STEMI care in community settings as reported previously. We established a government-sponsored STEMI care system in the State of Tamil Nadu with a goal of enhancing guideline-directed revascularisation (GDR) in STEMI. This study aimed to improve GDR by networking non-percutaneous coronary intervention (PCI)-capable hospitals with PCI capable hospitals in a hub-and-spoke model and implementing telemedicine through social media groups. Methods Eighteen Government medical college hospitals were provided cardiac catheterisation laboratories and linked to 188 Taluk and non-PCI capable Government hospitals forming 18 STEMI clusters. Telemedicine guidance on STEMI management was provided through WhatsApp groups created for each cluster. Cumulative daily data on the number of STEMIs, the revascularisation provided, and mortality were collected through free online web forms. Annual cumulative data on STEMI volumes and GDR were analysed. Results A total of 71,907 individuals were treated for STEMI from 2019 to 2023. A 67 per cent annual increase in the total individuals treated for STEMI was observed. The proportion of individuals receiving pharmaco-invasive therapy (PIT) increased from 7.9 to 31.7 per cent. There was a 68 per cent increase in the individuals receiving primary PCI. The number of individuals without GDR decreased by 20 per cent. There was a 6.7-fold rise in the individuals referred from spoke to hub hospitals for catheter-based revascularization. Overall mortality showed a marginal decrease from 8.7 to 8.4 per cent. Interpretation & conclusions Findings of this study suggest that a government-sponsored STEMI care system in a hub-and-spoke model, guided by telemedicine through social media groups will improve GDR in STEMI care in developing nations.

ABSTRACTThis study investigates the design and analysis of solid and thin‐walled slender structures made of hyperelastic soft materials using high‐order beam finite elements integrated into the Carrera unified formulation (CUF) framework. The primary aim is to evaluate the load‐bearing capacity and deflection characteristics of these structures through finite element analysis (FEA). Numerical results indicate that the experimental load capacity reached up to 19,000 kN, while FEA simulations reported a capacity of 15,000 kN. Midspan deflection measurements showed experimental values peaking at 5000 mm, compared to 4200 mm in FEA results. The average ratio of experimental to numerical results (Mtest/MFEA) across various specimens was 0.943 with a standard deviation of 0.11, suggesting a strong agreement between experimental and numerical analyses. These findings validate the effectiveness of high‐order beam finite elements for modeling the behavior of slender soft material structures, thus contributing to advancements in structural engineering.
Read moreAdvanced CT angiography in detecting rare arterio-venous malformations: Addressing diagnostic challenges in obscure gastrointestinal bleeding.
Gastrointestinal bleeding is a common clinical issue, but obscure gastrointestinal bleeding (OGIB) presents significant diagnostic challenges, especially when caused by arteriovenous malformations (AVMs). This study explored the role of advanced CT angiography (CTA) in diagnosing and managing AVM-related OGIB in 100 patients. CTA identified AVMs in 12% of cases where standard endoscopy failed (p < 0.001) and guided successful surgical control of bleeding in 95% of these cases. The sensitivity of CTA (92%) was significantly higher compared to other imaging modalities (68%; p = 0.002), particularly for small or inaccessible lesions. These findings underscore CTA's crucial role in improving diagnostic accuracy and enabling targeted therapeutic interventions for challenging cases of OGIB.
Read moreAn observational study on global longitudinal strain in acute coronary syndrome and its correlation with coronary angiogram
Background: Coronary angiography (CA) is crucial for diagnosing acute coronary syndrome (ACS) and assessing coronary artery disease (CAD), enabling healthcare providers to determine treatment options and assess the risk of cardiovascular events. Global longitudinal strain (GLS) is a sensitive marker of cardiac function. This study aimed to determine the correlation between GLS in ACS and CA findings. Methods: This prospective observational study was conducted at the cardiology department of the Salem government medical college. The study involved transthoracic echocardiography (TTE) to assess LV volumes, ejection fraction (EF), and LVGLS and strain analysis using 2D echocardiography to measure regional longitudinal peak systolic strain in different LV segments. CA was performed to visualise the coronary vessels and detect significant CAD. Results: The study population consisted predominantly of male patients (n=93, 62%), with notable smoking (n=48, 32%) and alcohol consumption (n=36, 24%). A significant majority of the patients (n=111, 74%) exhibited ECG abnormalities. Obstruction was present in 82% of the patients (n=123), with those showing impaired myocardial function compared to those without obstruction. GLS values below 13.95 were more common in patients with obstruction (n=8 out of 11), indicating reduced myocardial deformation. The GLS demonstrated excellent diagnostic performance for detecting significant coronary artery obstruction, with a high sensitivity of 88.89%, specificity of 90.63%, and overall accuracy of 90.24%. Conclusions: Our study revealed a strong correlation between reduced GLS and CAD, highlighting its importance in CAD assessment and risk stratification in ACS. GLS demonstrated excellent diagnostic performance for detecting significant CAD, showing high sensitivity and specificity.
Read moreAN OBSERVATIONAL STUDY ON AORTIC MEASUREMENTS IN SOUTH INDIAN POPULATION DONE AT TERTIARY CARE CENTRE
Background: Aortic dimensions vary between populations due to factors like age, sex, BSA, and ethnicity. Global reference ranges may not be applicable for the Indian population. The present study is aimed at defining the standard aortic dimensions in the South Indian population using 2D echocardiography. Methods: An observational study was conducted on patients admitted to Chengalpattu Medical College, Tamil Nadu, in the period from October 2023 to March 2024. Atotal of 800 patients aged 10 years to 80 years, in both sexes and with different sizes of BSA, had their aortic dimensions measured on 2D echocardiography. Descriptive statistics and Pearson correlation were analyzed. Results: The study involved 280 men (35%) and 520 women (65%). The average age was at 38.33 ± 16.02 years. Average aortic diameter was AA1.82 ± 0.42 cm, SOV 2.91 ± 0.52 cm, STJ 2.37 ± 0.47 cm and ascending aorta 2.64 ± 0.48 cm. The dimension diameters were greater in men by all measurements as compared with women by all the other parameters (AA, SOV, STJ). Apositive correlation was found between BSAand aortic dimensions. Age significantly influenced aortic size, with older groups showing larger values. Comparisons with the WASE guidelines showed that aortic dimensions in the Indian population were slightly smaller, particularly in females. Conclusion: Population-specific data on aortic dimensions of South Indians with the help of this study and, thereby, the need for region-specific reference ranges would be the take-home message. Thereby, sex differences were significant, and a good correlation was found with age and BSA, which would definitely improve clinical assessment and intervention.
