Linked color imaging improves polyp miss rates in total colonoscopy in a multicenter randomized back to back trial
Linked color imaging (LCI) was developed to detect gastrointestinal neoplasms. The current study aimed to determine whether the use of LCI, compared with white-light imaging (WLI), can improve the miss rates of colorectal polyp. A multicenter, randomized back-to-back study was conducted in 16 Japanese endoscopy units. Patients were randomized according to examination: tandem colonoscopy with WLI followed by LCI (WLI-LCI group) and with LCI followed by WLI (LCI-WLI group). The detected polyps were evaluated according to location, size, morphology, and histopathological diagnosis. The primary outcome was polyp miss rate per patient (PMR-PP) in total colonoscopy. The secondary outcome was adenoma detection rate (ADR) during the first assessment in each group. The full analysis set comprised 327 participants, and 320 were included in either two groups. The PMR-PPs were 9.3% and 20.6% in the LCI-WLI and WLI-LCI groups, respectively. Regarding location, the PMR-PP of LCI was significantly lower than that of WLI in the transverse and descending colons and rectum. In terms of diminutive adenomas (< 5 mm), the ADR of LCI (38.2%) was significantly higher than that of WLI (29.1%). Oral mucositis (stomatitis) is a painful condition that affects the mouth lining. Kampo medicines (e.g., Hangeshashinto [Chinese name, Ban-Xia-Xie-Xin-Tang], Orento, and Orengedokuto) have been widely used to treat stomatitis, such as gargling with Hangeshashinto. However, the mechanisms by which Kampo medicines work are not widely understood due to their oral administration and the subsequent digestion, absorption, and metabolization of their components. Stomatitis is associated with advanced glycation end-products (AGEs) in patients with lifestyle diseases, and can be induced by both intra- and extracellular AGEs (blood and dietary AGEs). Various natural products inhibit intracellular AGE generation and suppress cytotoxicity, such as inflammation caused by extracellular AGEs. This review summarizes 19 natural products identified in the Hangeshashinto water extract and 16 natural products identified in the crude drug extract. The data show that several natural products, such as glycyrrhizin, baicalin, 6-shogaol, quercetin, epigallocatechin-3-galate, and genistein, inhibit intracellular AGEs and suppress extracellular AGE inflammation. Furthermore, several natural products in the Hangeshashito water extract can suppress cytotoxicity in stomatitis.

北海道に生育するスズランの葉緑体ゲノムpsbA - trnH領域,trnL intron領域,trnL 3’ exon - trnF領域,atpB - rbcL領域,atpI - atpH領域における遺伝変異を調査した。その結果,3か所の塩基置換と1か所の挿入・欠失の遺伝変異が検出できた。また,それらの遺伝変異の組み合わせから,北海道に存在する5種類のハプロタイプが明らかになった。一方,これらハプロタイプの分布割合には地域間差が見られた。これらの結果は北海道におけるスズランの種苗移動について慎重に検討する必要があることを示唆している。

The standardization of medical records through structured templates has gained importance in improving the quality and safety of patient care. The results showed that missing rates ranged from 40.0% (fever reduction) to 43.9% (pain), while redundant entries ranged from 11.7% (mild fever) to 20.0% (fever). Analysis of medication mentions revealed reliance on general classifications, such as "analgesics" (0.43%) and "antipyretics" (0.35%), over specific drugs like acetaminophen (0.44%) or bisacodyl (0.51%). Free-form documentation led to inefficiencies and ambiguities, whereas structured templates improved specificity, particularly for medications critical to patient safety. The findings highlight the need for user-friendly templates to reduce variability and enhance the completeness of nursing records. Structured templates can streamline documentation, reduce redundancy, and improve the quality of care by prompting detailed and accurate entries. Additionally, integrating advanced technologies like AI could further optimize adherence to templates, ensuring consistent and safe clinical practices. A deeper exploration of user experience and organizational factors is warranted in future studies to enhance implementation success. AI-based tools, such as intelligent assistants that prompt missing entries in real time, could further enhance template adherence and improve data completeness. This study analyzed 90,432 anonymized nursing records to evaluate adherence to predefined documentation templates across five key topics: mild fever, pain, fever, bowel control, and fever reduction. Text mining techniques identified missing items, redundant entries, and medication mentions.
