Recommendation: Using participatory impact chains for assessing regional climate risks and local particularities: A case study in Patagonia drylands — R1/PR7
Climate hazards impact pastoral communities due to their dependency on nature for their primary livelihoods. This study analyzes climate risk in ten pastoral livestock farming communities in Patagonia drylands of Argentina. A participatory impact chains (PICs) approach was used as a qualitative and participatory bottom-up methodology allowing for the identification and contextualization of climate hazards, exposure, intermediate impacts and vulnerability dimensions through knowledge co-production with local stakeholders. Results show that, although drought is the predominant climate hazard across the region, its impacts are heterogeneous and mediated by local socio-environmental conditions. The analysis underlines that vulnerability is not evenly distributed but is shaped by specific historical, political and environmental pathways. These findings challenge standardized top-down risk assessments, and highlight the need for adaptation strategies that are context-sensitive, territorially differentiated and that integrate local knowledge. The study also contributes to advancing qualitative participatory methodologies for climate risk assessment in pastoral systems in arid areas of Latin America, showing the regional heterogeneity and social inequalities.

Assessing the Efficacy and Safety of the Allurion® Gastric Balloon in Latin American Patients: A Multicenter Case Series.
Introduction: The Allurion® gastric balloon (AGB) is an innovative option for treating overweight individuals and those with grade I obesity, or as a bridging treatment for bariatric surgery, or for patients who do not desire surgical intervention. This study aimed to evaluate the efficacy and safety of the AGB in a multicentric cohort. Materials and Methods: A retrospective analysis of consecutive cases treated with AGB (≥18 years old with body mass index [BMI] ≥27) was performed in eight centers in three countries in Latin America (Argentina, Chile, and Perú), between September 2021 and September 2022, with a 12-month follow-up. Results: In total, 402 patients were included (median BMI of 32.81 kg/m2). Mean total weight loss percentage was 8.3%, 11.6%, and 14.9% at 3, 6, and 12 months, respectively, with a follow-up of 93.03%, 75.37%, and 40.54%. The adverse event rate was 1.24% (n = 5). During the first 7 days, 34.58% (n = 139) experienced concomitant symptoms, and 1.99% (n = 8) asked for endoscopic balloon extraction because of intolerance. The complication rate was 6.46% (n = 26). Readmission index was 4.97% (n = 20), and reintervention was 2.23% (n = 9). Balloon elimination was experienced by 5.7% of patients in the first 14 days, 29.10% (n = 117) before the third month, and 62.68% (n = 252) after the 16th week. Early deflation rate was 2.73% (n = 11). Digestive track elimination was perceived by 26.61%. Conclusions: AGB is a safe and effective option for the treatment of overweight and obesity in Latin America. Lifestyle changes and continuous support with a multidisciplinary team are essential to achieving good mid- to long-term outcomes.

La luxación posterior de hombro traumática es una lesión poco frecuente que puede provocar defectos óseos en la cara anterior del húmero (lesión de Hill-Sachs invertida) y lesión del labrum (lesión de Bankart invertida) por la traslación posterior brusca de la cabeza humeral que suele involucrar un enganche entre la cabeza humeral y la glena. Si bien el tratamiento conservador sueleser favorable, en el 65-80% de los pacientes, pueden persistir los episodios de luxaciones, si no se someten a cirugía. Se han descrito tratamientos quirúrgicos, tanto abierto como artroscópicos, para las luxaciones persistentes. Objetivo: Describir la técnica artroscópica, una variante de la técnica abierta de McLaughlin, y compararla con otras publicadas, y comunicar el seguimiento de 3 casos tratados. Materiales y Métodos: Se incluyó a 3 pacientes operados por el mismo cirujano. El seguimiento promedio fue de 7 meses. Se utilizaron la escala analógica visual para dolor y el cuestionario WOSI. Resultados: Se constató la estabilidad del hombro en un tiempo variable de 3 meses. No hubo reluxaciones, sensación de inestabilidad ni infecciones. Conclusiones: Se obtuvo la estabilidad y la movilidad completa, evitando grandes abordajes y complicaciones asociadas. Esta variante de técnica artroscópica se puede utilizar como opción para evitar técnicas de reparación a cielo abierto.
