Anatomic Considerations of the Facial Nerve and Their Relevance to Facial Surgery
Carpal Tunnel Syndrome: Retrospective Analysis of 60 Consecutive Cases of Ambulatory Surgical Treatment With Local Anesthesia.
Background: The purpose of this study was to evaluate the safety and efficacy of ambulatory open surgical treatment for carpal tunnel syndrome with local anesthesia and without tourniquet ischemia, sedation by an anesthesiologist, or hospitalization. Methods: A retrospective analysis of 60 consecutive cases performed during 1 year was conducted, excluding other compressive or noncompressive neuropathies. The criteria for treatment efficacy were improvements in symptoms and recurrence rates, and the safety criterion was the presence of complications. Results: The mean patient age was 53.39 years, with 91.7% of women and 8.3% of men. The right side was more frequently affected (65% of the cases). The anesthetic and surgical procedures were well tolerated by patients. No complications during the procedures or postsurgical recurrences were observed. Conclusions: Consistent with previous data, the results of this study corroborate that the proposed treatment is effective, safe, and of low cost, resulting in great savings and increased productivity for the public health system.

Seroma formation has become the most reported complication after abdominoplasties. In 2000, progressive tension sutures (PTS) were described and reported to be associated with a seroma rate of 0.1%. Surgeons worldwide use PTS to prevent seroma; however, there are discrepancies in the number of PTS commonly used, starting from five up to 35 sutures. The absence of standardization may cause a lack of seroma prevention, increased surgical time, and general hesitation to perform the technique. To determine whether 11 PTS are sufficient to prevent seroma in abdominoplasties. We performed a single-blind, prospective, comparative, randomized clinical trial of 63 healthy patients from January to December 2017. Patients were divided into groups: group 1, classic abdominoplasty without the use of PTS and drains; group 2, abdominoplasty with the use of 11 PTS and drains; and group 3, abdominoplasty with the use of 22 PTS and drains. The duration of the surgical technique, presence of clinical seroma, number of aspirations, and days to drain removal were evaluated. The Cochran-Armitage, Fisher exact, Kruskal-Wallis, and t tests were used in the statistical analyses. PTS reduced the rate of seroma formation to 3.5%. Results showed that there is a benefit in using 11 PTS instead of 22 PTS because using more sutures led to an increased surgical time (30min vs. 13min) and possible complications. Compared to patients with PTS, those without PTS underwent more seroma aspirations and delayed drain removal. Eleven PTS should be used in classic abdominoplasties. intro_obj: La esclerodermia es una enfermedad autoinmune multisistémica caracterizada por una inflamación crónica del tejido conectivo. Intentando encontrar la técnica ideal para obtener el mejor resultado quirúrgico posible con un abordaje menos invasivo, proponemos la utilización de injertos autólogos de grasa para tratar las deformidades producidas por esta patología. Presentamos el caso de una paciente con diagnóstico de esclerodermia, difusa, multicéntrica, con compromiso extenso y severo de la cara y de varios segmentos corporales, con lesiones múltiples en placa (morfea) y lineales (golpe de sable) en cara, mamas y abdomen. Formulamos un plan de tratamiento quirúrgico basado en las posibilidades de remodelación y reestructuración facial y corporal con transferencia de tejido graso en 4 tiempos quirúrgicos consecutivos realizados en un período de 8 años. Consideramos que la transferencia de grasa es útil para la reversión de las alteraciones anatómicas y clínicas de la esclerodermia.
