A multi-focus oral panoramic x-ray image dataset based on pixel-level annotations.
The dataset is a comprehensive, large-scale collection of panoramic X-ray images developed to advance research in dental artificial intelligence. It comprises 8,655 de-identified images and over 30,186 pixel-level annotations of lesion regions. These images were obtained from Changsha Stomatological Hospital with informed patient consent, and all personally identifiable information was removed. Annotation was performed manually by a team of 20 experienced dental imaging specialists using LabelMe software. The experts delineated individual tooth contours through polygonal annotations, targeting various common oral conditions. This rigorous process ensured high annotation precision and clinical reliability.The dataset supports multiple research applications, including tooth segmentation, lesion detection, and computer-aided diagnosis. Its effectiveness has been validated using several widely adopted deep learning models, demonstrating strong generalization capabilities and broad applicability.

Macrophage-delivered Nifuroxazide reprograms immunosuppressive microenvironment and synergizes with oxaliplatin for enhanced anti-hepatocellular carcinoma therapy
Nifuroxazide (NFX), an antibacterial agent, also exhibits notable antitumor effects by inhibiting STAT3 signaling, which is often aberrantly activated and linked to chemoresistance in tumors such as hepatocellular carcinoma (HCC). Combining NFX with chemotherapy may enhance therapeutic efficacy, but its poor solubility limits oral bioavailability. To address this, we developed a biomimetic delivery system by loading NFX into murine macrophage-like RAW264.7 cells (Mφ-NFX). This strategy aims to improve drug delivery, enhance antitumor effects, with the potential to circumvent or overcome drug resistance. We evaluated the efficacy of Mφ-NFX alone and in combination with Oxaliplatin in vitro and in preclinical HCC models. Macrophages effectively carried and delivered NFX to tumor cells and tissues without significant toxicity to the carriers. Additionally, NFX promoted macrophage polarization toward the M1 phenotype within the tumor microenvironment. Mφ-NFX significantly inhibited tumor growth and increased the M1/M2 macrophage ratio. Co-treatment with Mφ-NFX and Oxaliplatin demonstrated enhanced tumor suppression and modulation of the tumor microenvironment. Mechanistically, NFX and Oxaliplatin acted synergistically via inhibition of the AKT/β-catenin pathway. In conclusion, this macrophage-based NFX delivery platform offers improved antitumor activity and sensitization to chemotherapy, presenting a promising strategy for HCC treatment.Graphical abstractSupplementary InformationThe online version contains supplementary material available at 10.1186/s12951-026-04196-6.

The aim of this study was to evaluate the clinical effectiveness and reliability of an improved maxillary sinus floor elevation technique combined with immediate implant placement in patients with insufficient residual bone height in the posterior maxilla. A total of 64 edentulous patients with inadequate posterior maxillary bone height were enrolled. Among these patients, 34 underwent the improved maxillary sinus floor elevation technique with simultaneous implant placement, whereas 30 underwent the conventional bone chisel technique under the same surgical protocol. Radiological and clinical assessments were performed to comprehensively evaluate the effectiveness of the improved technique and to compare its osteogenic outcomes with those of the conventional method. The mean immediate increase in vertical bone height was significantly greater in the improved technique group (6.016 ± 1.312 mm) than in the bone chisel group (2.797 ± 0.898 mm; P < 0.001). Significant differences were also observed between the two groups in terms of apical bone coverage along the implant axis immediately postoperatively and at the 6–10 month follow-up (P < 0.001). Similarly, peri-implant bone coverage significantly increased in the improved technique group at both time points (P < 0.001). No significant differences were found in the short-term implant success or survival rates between the two groups. Notably, no intraoperative maxillary sinus membrane perforations occurred in the improved technique group, and postoperative complications were limited to mild swelling and pain at the surgical site. The improved maxillary sinus floor elevation technique yields favourable clinical outcomes in patients with severely resorbed posterior maxillae, particularly when the residual bone height is smaller than 5 mm. This technique constitutes a viable alternative to lateral window sinus augmentation for such cases.

Latamoxef has been used to treat various bacterial infections. To provide data supporting the rational clinical use of latamoxef, bacterial resistance over 10 years was analyzed at multiple centers throughout China. The results were used to develop tentative epidemiological cut-off values (TECOFFs) for latamoxef. A total of 46,716 strains of common Enterobacteriaceae were collected from patients with bloodstream infections at 72hospitals in 21 provinces of China between 2014 and 2023. The in vitro antimicrobial activities of latamoxef were compared with those of other commonly used cephalosporin and carbapenem. The distribution of minimum inhibitory concentrations was subjected to cumulative log-normal fitting to obtain the TECOFFs of latamoxef for common Enterobacteriaceae. The sensitivities of Escherichia coli (E. coli), Salmonella species, Serratia marcescens (S. marcescens) and Proteus mirabilis (P. mirabilis) to latamoxef ranged from 94.37% to 98.08%. Klebsiella oxytoca (K. oxytoca) and Enterobacter aerogenes (E. aerogenes) had sensitivities of 89.91% and 87.21%, respectively, whereas Klebsiella pneumoniae (K. pneumoniae) and Enterobacter cloacae (E. cloacae) had lower sensitivities. The in vitro activity of latamoxef against these bacteria, especially extended-spectrum β-lactamase (ESBL)-producing Enterobacteriaceae, was similar to the activities of ertapenem and meropenem and significantly higher than the activities of ceftriaxone, ceftazidime, and cefepime. The sensitivities of ESBL (+) E. coli and ESBL (+) P. mirabilis to latamoxef were similar to their sensitivities to ertapenem and meropenem. ESBL (+) K. pneumoniae and ESBL (+) K. oxytoca exhibited comparable but lower rates of sensitization to latamoxef, ertapenem, and meropenem. Latamoxef had TECOFFs of 2µg/mL for E. coli, K. pneumoniae, K. oxytoca, P. mirabilis, E. aerogenes, Salmonella, and S. marcescens, and 4µg/mL for E. cloacae. Latamoxef has good and stable in vitro antimicrobial activity against Enterobacteriaceae, including ESBL-producing Enterobacteriaceae. The calculated TECOFFs of latamoxef provide an important reference for subsequent use of this antibiotic against Enterobacteriaceae.

Patent foramen ovale (PFO) is a common congenital cardiac anomaly. Transcatheter closure is effective, but guidewire passage failure remains a challenge. We aimed to identify risk factors and develop a predictive model for procedural failure. This retrospective study included 83 PFO patients (October 2021–May 2024) undergoing interventional closure, divided into success (n = 52) and failure (n = 31) groups. Multivariate logistic regression analyzed risk factors, and a nomogram was developed. Model calibration was assessed using the Hosmer–Lemeshow test and calibration curves, while discriminative ability was evaluated via receiver operating characteristic analysis. Predictive accuracy was validated through 5-fold cross-validation, and clinical utility was examined using decision curve analysis and clinical impact curve. Left atrial aperture ≤1.8 mm was a significant risk factor for failure (odds ratio [OR] = 10.691, P < .05), while contrast transthoracic echocardiography-determined right-to-left shunt grade II (OR = 0.147) and III (OR = 0.056), and surgical team experience >3 years (OR = 0.077) were protective (all P < .05). The model was well calibrated (χ² = 4.002, P = .857), and the area under receiver operating characteristic curve was 0.913, indicating medium to strong predictive ability. The mean area under the curve for 5-fold cross-validation was 0.878, and decision curve analysis confirmed clinical utility at threshold probabilities of 0.01 to 0.85. The clinical impact curve demonstrated high consistency between predicted and actual outcomes when the threshold probability exceeded 60%. The model has good calibration, accuracy, distinguishing power, and clinical applicability, and provides a valuable tool for preoperative risk assessment of PFO occlusion.
