Die Gluteale Tendinopathie – wenn die Hüfte seitlich schmerzt
ZusammenfassungMit der Glutealen Tendinopathie sind Schmerzen über dem Trochanter major, die lateral ins Bein ausstrahlen können, verbunden. Sie betrifft sowohl Sportler*innen als auch Nicht-Sportler*innen. Momentan wird von einer nicht-inflammatorischen Insertionstendinopathie des Gluteus medius und minimus als Ursache ausgegangen. V.a. während funktioneller Aktivitäten, die eine Stabilisierung des Beckens in der Frontalebene erfordern, sind diese Sehnen einer Kombination aus hohen mechanischen Kompressions- und Zugkräften ausgesetzt. Es ist wichtig, die Gluteale Tendinopathie gegenüber der Hüftarthrose und somatisch ausstrahlenden LWS-Schmerzen abzugrenzen. Eine Kombination an Tests, bestehend aus Palpation um den Trochanter major, dem Single Leg Stance Test sowie einer Hüftabduktion gegen Widerstand, zeigt eine positive Wahrscheinlichkeit von 99% und eine negative Likelihood-Ratio von 95,5%. Mittels Fragebögen können die subjektiven Einschränkungen der Patient*innen erfasst werden. Eine angemessene und patient*innenzentrierte konservative Therapie zeigt eine hohe Erfolgsrate. Diese umfasst edukative Maßnahmen, einen langsamen Trainingsaufbau und ggf. Stoßwellentherapie. Bei 90% der Patient*innen zeigen diese konservativen Ansätze positive Effekte. Edukation spielt eine essenzielle Rolle, um die Selbstwirksamkeit und somit die Alltagsbewältigung zu steigern. Das Training sollte in Positionen mit geringer Belastung begonnen und progressiv gesteigert werden. Ein besonderes Augenmerk liegt auf der Stabilität des Beckens in der Frontalebene bei funktionellen Aktivitäten.

Early-onset SCN2A developmental and epileptic encephalopathy is caused by SCN2A gain-of-function variants. Here we describe the clinical experience with intrathecally administered elsunersen, a gapmer antisense oligonucleotide targeting SCN2A, in a female preterm infant with early-onset SCN2A developmental and epileptic encephalopathy, in an expanded access program. Before elsunersen treatement, the patient was in status epilepticus for 7 weeks with a seizure frequency of 20-25 per hour. Voltage-clamp experiments confirmed impaired channel inactivation and increased persistent current consistent with a gain-of-function mechanism. Elsunersen treatment demonstrated a favorable safety profile with no severe or serious adverse events reported after 19 intrathecal administrations over 20 months. After administration in combination with sodium channel blockers, status epilepticus was interrupted intermittently and ultimately ceased after continued dosing. A >60% reduction in seizure frequency corresponding to five to seven seizures per hour was observed, which has been sustained during follow-up until the age of 22 months. These data provide preliminary insights on the safety and efficacy of elsunersen in a preterm infant. Additional investigation on the benefits of elsunersen in clinical trials is warranted.

Zusammenfassung1/2023 wurde das dritte Update der seit 2005 bei der AWMF (Arbeitsgemeinschaft wissenschaftlich-medizinischer Fachgesellschaften) gelisteten und fortlaufend aktualisierten Leitlinie „Harninkontinenz bei geriatrischen Patienten – Diagnostik und Therapie“ durch die interdisziplinäre Arbeitsgruppe „Harninkontinenz“ der Deutschen Gesellschaft für Geriatrie (DGG) publiziert. Aus dieser Leitlinie, die als offizielle Leitlinie DGG akkreditiert ist, stellt der vorliegende Artikel die Kapitel „Verhaltensintervention, Toilettentraining, physiotherapeutische Maßnahmen“ dar.In einem strukturierten Bewertungsprozess identifizierte eine Literaturrecherche zunächst die vorhandene Literatur im Kontext des „geriatrischen Patienten“, wie er als zumeist über 70jährig und multimorbid oder über 80jährig durch die Fachgesellschaften definiert ist. Primäre Berücksichtigung fanden randomisierte, doppelblinde, plazebokontrollierte Studien sowie bereits vorhandene Leitlinien zum Thema. Wo keine solchen Untersuchungen vorlagen oder aus methodischen Gründen prinzipiell nicht durchführbar sind, wurden auch Publikationen anderer Designs (nicht randomisierte Untersuchungen, Fallkontrollstudien) zur Leitlinienerstellung herangezogen. Die daraus resultierenden Leitlinienempfehlungen wurden einem strukturierten Abstimmungsprozess unterzogen und abschließend Deligierten relevanter Fachgesellschaften vorgelegt.Vor dem Hintergrund der Polypharmazie mit ihren Medikamenteninteraktionen und der besonderen Vulnerabilität des hochbetagten, geriatrischen Patienten sind nicht-pharmakologische und nicht-operative Interventionen besonders wichtig. Diese bedürfen jedoch ein Mindestmaß an Compliance, kognitiven Ressourcen und Restmobilität. Deswegen wurden Verhaltensinterventionen wie Gewichtsabnahme, Flüssigkeitsmanagement, Diuretikaeinsatz, Stuhlregulierung und Toilettentraining sowie aktive und passive physiotherapeutische Maßnahmen im Hinblick auf ihre Wirkung und Anwendbarkeit bei geriatrischen Patienten überprüft und bewertet.Verhaltensinterventionen, Toilettentraining und physiotherapeutische Maßnahmen sind besonders vor dem Hintergrund der mit spezifischen Risiken behafteten medikamentösen oder operativen Therapiealternativen wertvolle Bausteine der geriatrischen Inkontinenzbehandlung und sollten in angepasster Form flankierend oder alleinig bei der Therapie geriatrischer Patienten Anwendung finden. Es gilt, die personelle Ausstattung einer Einrichtung mit der Anzahl der derart behandelten geriatrischen Patienten in ein realistisches, zielführendes Verhältnis zu bringen.

Adversarial attacks on Natural Language Processing (NLP) models expose vulnerabilities by introducing subtle perturbations to input text, often leading to misclassification while maintaining human readability. Existing methods typically focus on word-level or local text segment alterations, overlooking the broader context, which results in detectable or semantically inconsistent perturbations. We propose a novel adversarial text attack scheme named Dynamic Contextual Perturbation (DCP). DCP dynamically generates context-aware perturbations across sentences, paragraphs, and documents, ensuring semantic fidelity and fluency. Leveraging the capabilities of pre-trained language models, DCP iteratively refines perturbations through an adversarial objective function that balances the dual objectives of inducing model misclassification and preserving the naturalness of the text. This comprehensive approach allows DCP to produce more sophisticated and effective adversarial examples that better mimic natural language patterns. Our experimental results, conducted on various NLP models and datasets, demonstrate the efficacy of DCP in challenging the robustness of state-of-the-art NLP systems. By integrating dynamic contextual analysis, DCP significantly enhances the subtlety and impact of adversarial attacks. This study highlights the critical role of context in adversarial attacks and lays the groundwork for creating more robust NLP systems capable of withstanding sophisticated adversarial strategies.

In the recent past, there were several works on the prediction of stock prices using different methods. Sentiment analysis of news and tweets and relating them to the movement of stock prices have already been explored. But, when we talk about the news, there can be several topics such as politics, markets, sports etc. It was observed that most of the prior analyses dealt with news or comments associated with particular stock prices only or the researchers dealt with the overall sentiment scores only. However, it is quite possible that different topics have different levels of impact on the movement of the stock price or an index. The current study focused on bridging this gap by analysing the movement of Nifty 50 index with respect to the sentiments associated with news items related to various different topic such as sports, politics, markets etc. The study established that sentiment scores of news items of different other topics also have a significant impact on the movement of the index.

Adversarial attacks pose a significant threat to the reliability of pre-trained language models (PLMs) such as GPT, BERT, RoBERTa, and T5. This paper presents Adversarial Robustness through Dynamic Ensemble Learning (ARDEL), a novel scheme designed to enhance the robustness of PLMs against such attacks. ARDEL leverages the diversity of multiple PLMs and dynamically adjusts the ensemble configuration based on input characteristics and detected adversarial patterns. Key components of ARDEL include a meta-model for dynamic weighting, an adversarial pattern detection module, and adversarial training with regularization techniques. Comprehensive evaluations using standardized datasets and various adversarial attack scenarios demonstrate that ARDEL significantly improves robustness compared to existing methods. By dynamically reconfiguring the ensemble to prioritize the most robust models for each input, ARDEL effectively reduces attack success rates and maintains higher accuracy under adversarial conditions. This work contributes to the broader goal of developing more secure and trustworthy AI systems for real-world NLP applications, offering a practical and scalable solution to enhance adversarial resilience in PLMs.

An Investigation on the Efficiency of Some Text Attack Algorithms
Machine learning models trained on human language, also known as Natural Language Processing (NLP) models, are susceptible to manipulation. These attacks, called NLP attacks, work by subtly altering the input text. This can involve swapping words, adding new ones, or removing existing ones. While these changes might seem insignificant to humans, they can fool the model into making incorrect predictions. The robustness of NLP models against adversarial attacks has become a critical area of research. This work delves into the effectiveness of three well-known text attack methods: PWWS, TextBugger, and TextFooler. The performance of these schemes is evaluated on two popular pre-trained transformers, DistilBERT and RoBERTa (both based on BERT). The experiments involve three text classification datasets: IMDB (movie review sentiment), AG News (topic classification), and SST2 (sentiment analysis). From each dataset, 100 examples are used to assess how effectively each attack method deceives the models and analyze the unique strengths and weaknesses of each approach. PWWS employs word saliency and probability weighting to generate adversarial examples, whereas TextBugger and TextFooler utilize semantic and syntactic perturbations to fool well-trained models. The results reveal the efficiencies of these attacks in different conditions. This analysis offers crucial insights into the susceptibilities of BERT-based models to prominent attack strategies, highlighting the need for enhanced defense mechanisms. By meticulously examining the attack vectors and their impacts, this research contributes to the broader understanding of adversarial attacks in NLP. It lays the groundwork for future advancements in model robustness.
