117 Spread, adoption, implementation and maintenance of the falls management exercise (FaME) programme; learning from a mixed methods implementation study in demographically diverse regions of the UK
BackgroundThe Falls Management Exercise (FaME) Programme is a group-based, face-to-face and home-based, six-month multi-component exercise intervention for older adults. It reduces falls and fear of falling, increases physical activity and improves confidence and balance. FaME is also cost effective. Despite this evidence, it is not widely available in the UK.ObjectiveTo study the spread, implementation, adherence, ‘real world’ outcomes and quality management of FaME in 14 organisations across three demographically diverse regions of the UK.Programme DescriptionUsing the Consolidated Framework for Implementation Research to map themes, we undertook a qualitative analysis of 40 Semi-structured interviews with FaME commissioners, providers and participants, 25 class observations and content analysis of management documents. To assess participant outcomes we analysed quantitative functional and self-reported fall outcomes from 1601 participants.Outcomes and LearningsClass availability tripled in one region, whilst delivery remained consistent in others. Factors influencing spread included FaME’s evidence base, perceived fit with organisational values and the personal experiences and beliefs of decision-makers.Univariate pre/post analyses of routinely-collected outcomes showed significant improvements in confidence and balance (Confbal p<0.001), falls (in the previous 3 months, p<0.001) and Timed-Up-and-Go (p<0.001).Participants valued social opportunities as well as improvements in their physical abilities and reported wellbeing benefits beyond fall prevention.Newer programmes or those with quality assurance systems in place showed higher fidelity and quality. Programmes varied in duration and aspects of delivery but a national community of practice forum provided improvement opportunities and mechanisms.ImplicationsComplex and interacting factors influence FaME’s availability, delivery, uptake, participant adherence and outcomes. Findings could be extrapolated to other exercise interventions and have informed the production of an updated implementation toolkit to assist its high-quality implementation.ConclusionsFaME is an evidence-based fall prevention programme but delivery is often inconsistent and there are areas without any provision in the UK. Awareness of its evidence-base is necessary but not sufficient for spread and measures to ensure continued fidelity, quality, outcomes and adherence need to be designed into contracted delivery.

Protección de los derechos fundamentales en la pandemia del coronavirus
¿Ha cambiado la situación de los derechos fundamentales en tiempos de la pandemia provocada por el coronavirus? Lo cierto es que la protección de los derechos fundamentales no es una vía de un solo sentido que comienza con el individuo, sino una promesa de la Ley Fundamental que debe ser implementada por todos los poderes estatales. Todo el ordenamiento jurídico en sí debe ser libre, no solo el estatus legal del individuo. En los últimos meses, incluso la jurisprudencia de los tribunales inferiores y del Tribunal Constitucional Federal ha demostrado ser bastante débil. En opinión del autor, el mérito de haber provocado un cambio fundamental en la cultura de ejecución de toma de decisiones dentro del poder judicial corresponde al Tribunal Supremo Administrativo (en adelante OVG) de Münster, que en su «decisión Gütersloh» (OVG Münster, decisión de 29.06.2020-13 B 940/20.NE-Lockdown Gütersloh) volvió a la aplicación correcta de la prueba de proporcionalidad.Has the situation of fundamental rights changed in times of the corona pandemic? One thing is certain: the protection of basic rights is not a one-way street that begins with the individual, but a promise of the Basic Law that must be implemented by all state powers. The legal system as such, not just the legal status of the individual, should be a free one. In the last few months, even the case law of the lower courts and the Federal Constitutional Court has proven to be rather pale. The merit of having brought about a fundamental change in the executive decision-making culture within the judiciary belongs in the perception of the author to the Supreme Administrative Court (OVG) Münster, which in its «Gütersloh decision» (OVG Münster, decision of 29.06.2020 - 13 B 940 / 20.NE - Lockdown Gütersloh) showed the way back to a normal proportionality test.

This study identified the future ideal functions of PYP and developed a conceptual framework for the program at King Saud University. Four research questions were answered in this study including: 1) Using the Concept Mapping/Pattern Matching methodology, how do Saudi male and female postsecondary stakeholders conceptualize the future ideal functions of Preparatory Year Deanship to help students successfully transition into their first year of college/university?; 2) Does this Concept Mapping/Pattern Matching conceptualization compare and contrast with the current working model of the Preparatory Year Program at King Saud University?; 3) Does this Concept Mapping/Pattern Matching conceptualization compare and contrast with King Saud University's explicit strategic plan?; and 4) When conducting a Concept Mapping/Pattern Matching conceptualization, how do the results for males compare and contrast with the results for females?;The Concept Mapping/Pattern Matching methodology advanced by (Trochim 1989a, 1989b; Kane & Trochim, 2007) utilized for data collection and analyzing. Fourteen male including 2-PYP students, 1-Medicine College Faculty, 1-Business Administration College Faculty, 1-Engineering College Faculty, 4-PYP Faculty, 1-Academic Advising Faculty at PYP, 1-Students Affairs Faculty at KSU, 1- Students Affairs at PYP, and 2-Educational Company and nine female including 3-PYP' students, 1- Pharmacy College Faculty, 2-PYP Faculty, 2-Parents, and 1-Educational Company participated in two separate brainstorming sessions. The male participants generated 48 statements and the female group generated 36 statements. The participants in each group sorted the generating statements individually into groups and rated them based on a five-point scale for relative importance and institutional efficacy.;For each group male/female the Multidimensional Scaling and Hierarchical Cluster Analysis were conducted to create various concept maps, the final one being a seven-cluster and six-cluster concept map for male and female, respectively, representing the stakeholders' conceptualization of the field being studied. Following the creation of the concept maps for each group, the resulting priorities and their conceptualization schema were both compared and contrasted to the current working model of PYP and the explicit strategic plan of KSU (KSU2030) for similarities and differences in a qualitative document analysis. The male and female perceptions were also compared and contrasted.;The study revealed three main dimensions for developing PYP at KSU including Organizational design and structure, Pedagogical functionality, and Student services. Social-political and social-economic transformation of the Slovak Republic
The way of the Slovak Republic (SK) towards democracy and market economy appeared to be very hard. Starting position for implementation of reforms objectively was less favorable in comparison with the neighboring countries. The fundamental law of the SK guaranteed political pluralism, civil rights and liberties for citizens. In 2004 the state became a full member of the EU and NATO. In 2005 main macroeconomic indices of the state reached a threshold level, established by the Maastricht Treaties, and as to the rate of GDP increase and direct foreign investment Slovakia joined the EU leaders. Along with the achievements in the political life of the country there are also serious continuous problems. Among them are: irredentist attitudes among the Hungarian minority which fuel conflict potential in the Slovak-Hungarian relationship; hardships of social integration of the Roma population. An acute problem is corruption, the level of which is higher than in other Visegrad countries. Aiming at formation of conditions for stable continuous growth Slovakia carries out a “reset” of the economic policy. Large-scale reforms of the social system and social policy in SK have been implemented.
