Laryngopharyngeal reflux in the light of current medical knowledge – a literature review
Introduction and aim of the work: Laryngopharyngeal reflux often occurs in daily otolaryngological clinical practice. Diagnostics and treatment of this disease are complex for clinicians. Proton pump inhibitors cause many adverse effects, so alternative therapeutic methods are required. This study aims to submit current medical knowledge about laryngopharyngeal reflux, including possible treatment options. Materials and methods: The literature review of medical articles published between 2014 and 2024 was performed. These papers were accessed from the PubMed and Springer Nature Link databases. To find relevant articles, the following keywords were used: “laryngopharyngeal reflux”, “reflux laryngitis”, “proton pump inhibitors”, and “baclofen side effects”. State of knowledge: Symptoms of laryngopharyngeal reflux can result in reduced quality of life. Laryngopharyngeal symptoms are not that specific. Patients often search for a reason for their complaints at different specialists. Treatment of this disease is usually based on the trial-and-error method and observing patients’ responses. Summary: The accurate incidence of laryngopharyngeal reflux is not known. Various factors influence the occurrence of this disease. There are disparate opinions about using 24-hour pH monitoring as a gold standard; thus, other methods are sought. Proton pump inhibitors are the most common drugs used in the therapy, but their effectiveness is doubtful. More research should be done on the different methods of laryngopharyngeal reflux treatment. Pepsin inhibitors seem like an auspicious alternative, but there are too few studies on this subject.

Patients with alcohol use disorder (AUD) tend to selectively approach alcohol cues in the environment, demonstrating an alcohol-approach bias. Alcohol-approach-bias modification (Alcohol-ApBM) effectively increases abstinence rates in patients with AUD when added to abstinence-focused treatment, but the evidence for its proposed working mechanism (reduction of the alcohol-approach bias) is limited. Moreover, not all patients benefit from Alcohol-ApBM, and previous research did not identify reliable pretreatment predictors of Alcohol-ApBM effectiveness. Therefore, the current study focused on learning processes during the Alcohol-ApBM training itself. Specifically, we examined whether changes in approach-avoidance tendencies over the course of Alcohol-ApBM would predict abstinence after inpatient treatment. The training data of 543 AUD patients in Germany (70% male, M = 47.96, SD = 9.08), receiving Alcohol-ApBM training during inpatient treatment, were used to examine whether various aspects of learning during training predicted abstinence 1 year after treatment discharge, both separately and in interaction with potential sociodemographic and clinical moderators of Alcohol-ApBM effectiveness. Overall, successful learning across six Alcohol-ApBM training sessions was observed; that is, the approach tendency toward alcoholic stimuli was reduced over time. However, none of the examined learning parameters were predictive of abstinence, neither separately nor in combination with clinical or sociodemographic variables. Previous studies have shown that Alcohol-ApBM is an effective add-on to abstinence-focused treatment for AUD, and this study showed that learning took place during Alcohol-ApBM training. However, specific learning parameters during training did not predict abstinence 1 year after treatment discharge. Therefore, we cannot specify which patients are most likely to benefit from ApBM with regard to abstinence after 1 year.

In an increasing number of countries, patients are given online record access (ORA) to their clinical notes ("open notes"). In many places, psychotherapy notes are exempt, even if patients explicitly wish to read them. Previous research suggests that psychotherapists (PTs) have reservations that are not yet fully understood. To investigate the attitudes and perceived effects of open notes on psychotherapeutic care, patients, and individual psychotherapeutic practice in Germany. Psychological and medical therapists were invited to participate in a national online survey. Sociodemographic characteristics such as gender, age, professional group, and psychotherapeutic school were gathered. Descriptive statistics were used to analyze the 51-item survey. 129 PTs completed the survey. Only a small proportion of respondents (30 out of 129, 23.3%) suspected that open notes would improve the efficiency of psychotherapeutic care. On the one hand, participants assumed that patients gain more control over their treatment (59 out of 129, 45.7%) and are better able to remember therapy goals (55 out of 129, 42.6%), although this was considered unlikely to lead to greater engagement in the therapy process (94 out of 129, 72.9%). On the other hand, PTs expected patients to misunderstand their notes, feel offended (98 out of 129, 76.0%), and approach them with questions (107 out of 129, 82.9%) or requests for changes (94 out of 129, 72.9%). The respondents also anticipated being less honest when writing (95 out of 129, 73.6%) and reported they needed more time for documentation (99 out of 129, 76.7%). A meaningful use of open notes for working with relatives was envisaged (101 out of 129, 78.3%). PTs in Germany tend to have a negative attitude towards patients' ORA on open notes. Further research on clinical efficacy and feasibility is necessary to demonstrate whether open notes add value in the context of psychotherapy.

Abstract Background: Alcohol use disorder (AUD) belongs to the most burdensome clinical disorders worldwide. Current treatment approaches yield unsatisfactory long-term effects with relapse rates up to 85%. Craving for alcohol is a major predictor for relapse and can be intentionally induced via cue exposure in real life as well as in Virtual Reality (VR). The induction and habituation of craving via conditioned cues is used in Cue Exposure Therapy (CET), a long-known but rarely used strategy in Cognitive Behavioral Therapy (CBT) of AUD. VR scenarios with alcohol related cues offer several advantages over real life scenarios and are within the focus of current efforts to develop new treatment options. As a first step, we aim to analyze if the VR scenarios elicit a transient change in craving levels and if this is measurable via subjective and psychophysiological parameters. Methods: A single-arm clinical study will be conducted including n=60 patients with AUD. Data on severity of AUD and craving, comorbidities, demographics, side effects and the feeling of presence in VR will be assessed. Patients will use a head-mounted display (HMD) to immerse themselves into three different scenarios (neutral vs. target situations) while heart rate, heart rate variability, pupillometry and electrodermal activity will be measured continuously. Subjective craving levels will be assessed before, during and after the VR session. Discussion: Results of this study will yield insight into the induction of alcohol craving in VR cue exposure paradigms and its measurement via subjective and psychophysiological parameters. This might be an important step in the development of innovative therapeutic approaches in the treatment of patients with AUD. Trial registration: This study was approved by the Charité - Universitätsmedizin Berlin Institutional Review Board (EA1/190/22, 23.05.2023). It was registered on ClinicalTrials.gov (NCT05861843).

Zusammenfassung: Zielsetzung: Um die Möglichkeiten smartphonebasierter Interventionen für die stationäre Entwöhnungstherapie in einer Rehabilitationsklinik zu nutzen, wurde die MeinSalus-App entwickelt. Ziel der Studie ist es, (1) die Akzeptanz (Nutzungsabsicht) der App bei Patienten sowie beim therapeutischen Personal zu untersuchen, (2) Prädiktoren der Akzeptanz auf der Grundlage eines erweiterten UTAUT (Unified Theory of Acceptance and Use of Technology-)Modells zu bestimmen und (3) Aufschlüsse über das Nutzungsverhalten zu bekommen. Methodik: 193 von 282 stationären Patienten (68,4 %) sowie 18 von 34 Therapeuten (52,9 %) beantworteten Fragebögen zur Akzeptanz (Nutzungsabsicht) der App und deren einzelnen Funktionen, zu UTAUT- und mHealth-bezogenen Variablen sowie zu ihrem Nutzungsverhalten. Ergebnisse: Die mittlere Nutzungsabsicht (Range 1–5) für die App insgesamt lag für Patienten bei 4.65 ( SD = 0.83) und für Therapeuten bei 4.07 ( SD = 0.62). Alter, mobile Internetnutzung, erwarteter Nutzen sowie Vorbehalte gegenüber der App erwiesen sich als signifikante Prädiktoren der Akzeptanz. Beim Nutzungsverhalten ergab sich ein differenziertes Bild für die unterschiedlichen Funktionen. Schlussfolgerungen: Es zeigte sich sowohl in der Patientenstichprobe als auch beim teilnehmenden therapeutischen Personal eine hohe Akzeptanz der App, was auf eine akzeptable Grundlage für eine Implementierung in der stationären Suchtbehandlung hindeutet. Studien zu Effekten der App sollten sich anschließen.

Zusammenfassung: Hintergrund: Obwohl die Kauf-Shopping-Störung bereits vor über 100 Jahren von Kraepelin (1909) als Oniomanie beschrieben wurde und von einer geschätzten Prävalenz von 4,9 % in der Bevölkerung ( Maraz, Griffiths & Demetrovics, 2016 ) ausgegangen wird, ist sie weiterhin keine eigenständige Erkrankung in den Klassifikationssystemen. Patienten und Patientinnen mit einer Kauf-Shopping-Störung berichten von unterschiedlichen Konsequenzen die von Konflikten in der Partnerschaft und Schulden bis hin zu delinquentem Verhalten (z. B. Strafverfahren wegen Bestellungen auf falschem Namen) reichen. Weiterhin liegen oft komorbide Störungen vor, v. a. Depressionen, soziale Ängste, zwanghaftes Horten, Binge-Eating-Störung, Glücksspielsucht und Substanzkonsumstörungen ( Müller, Mitchell, Crosby et al., 2010 ; Granero et al., 2016 ). Hinsichtlich der Behandlung der Kauf-Shopping-Störung, ist nach aktuellem Stand keine ausreichende Wirksamkeit von psychopharmakologischen Behandlungen gezeigt worden (e. g. Nicoli de Mattos et al., 2020 ; Koran, Aboujaoude, Solvason, Gamel & Smith, 2007 ). In wenigen randomisiert kontrollierten Studien ( Müller, Arikian, de Zwaan & Mitchell, 2013 ; Müller, Müller et al. 2008 ; Mitchell, Burgard, Faber, Crosby & de Zwaan, 2006 ; Benson, Eisenach, Abrams & van Stolk-Cooke, 2014 ), die durchgeführt worden sind, zeigte sich die kognitive Verhaltenstherapie als wirksam. Ob die Kauf-Shopping-Störung eher als eine Impulskontrollstörung, eine Zwangsstörung oder eine Verhaltenssucht eingeordnet werden kann, wird aktuell noch debattiert. Die Diagnose wird im aktuellen ICD-10 ( Dilling & Freyberger, 2016 ) von Klinikern als eine sonstige (F63.8) oder nicht näher bezeichnete Impulskontrollstörung (F63.9) eingeordnet. Im ICD-11 ( WHO, 2019 ) wird sie ohne Nennung diagnostischer Kriterien als eine andere spezifische Impulskontrollstörung (6C7Y) zugeordnet. Führende Experten sehen jedoch eine bessere Zuordnung der Kauf-Shopping-Störung unter den Störungen durch abhängiges Verhalten ( Müller, Brand et al. 2019 ). Ziel: Ziel dieses Fallberichtes ist es, das Bewusstsein über die Existenz der Kauf-Shopping-Störung zu schaffen, Beispiele zur diagnostischen Einordnung aufzuzeigen, Behandlungsbausteine Therapeuten und Therapeutinnen näher zu bringen und die Notwendigkeit für weitere Forschung zu zeigen.

SummaryThe present study aimed at investigating the impact of the pandemic on sleep and mental health in healthy individuals (n = 78) as well as in psychiatric outpatients (n = 30) during the first and the second lockdown in Germany, in March and November 2020, respectively. Sleep quality and anxiety were worse in patients compared with controls during both lockdowns. Further, patients but not controls exhibited higher levels of depression and overall psychiatric symptomatology during the second lockdown. No differences were found in the perceived threat evoked by the pandemic. The data suggest that healthy individuals adapt flexibly to the difficult situation over the time course of the pandemic, whereas psychiatric patients seem to get worse, indicating difficulties in adapting to stressful circumstances.

Psychische Komorbidität bei Kaufsucht im Vergleich zu anderen psychischen Störungen
Kaufsucht geht mit einer hohen psychischen Komorbiditat einher. Bislang haben nur wenige Studien untersucht, ob sich Patient*innen mit Kaufsucht hinsichtlich der komorbiden Psychopathologie von Patient*innen mit anderen psychischen Storungen unterscheiden. Die Arbeit untersuchte die Haufigkeit von Angst‑, Depressions- und somatoformen Symptomen sowie traumatischer Kindheitserfahrungen bei Patient*innen mit Kaufsucht und Patient*innen mit anderen psychischen Storungen. Es haben 128 ambulante Patient*innen mit Kaufsucht und 116 stationare Patient*innen mit anderen psychischen Storungen folgende Fragebogen beantwortet: Pathological Buying Screener, die Module fur depressive Erkrankungen (PHQ-9) und somatoforme Storungen (PHQ-15) des Gesundheitsfragebogens fur Patient*innen (PHQ-D), den Generalized Anxiety Disorder Screener (GAD-7) sowie den Adverse Childhood Experiences Questionnaire. Es wurden keine Gruppenunterschiede bezogen auf Alter, Geschlecht, Bildung und Partnerschaftsstatus gefunden. Auch ergaben sich keine Gruppenunterschiede hinsichtlich Depressions- und somatoformer Symptome oder traumatischer Kindheitserfahrungen. In der Gruppe mit Kaufsucht war der Anteil der Patient*innen, die im GAD‑7 uber dem Schwellenwert fur eine Angststorung scorten, marginal hoher als in der Gruppe der stationaren Psychotherapiepatient*innen. Die hohe psychische Komorbiditat sollte bei der Behandlung von Patient*innen mit Kaufsucht Berucksichtigung finden.
