In recent years, there has been a surge in the development of new, blood-based, single- and multi-cancer detection tests (SCD and MCD), which can detect cancer signals prior to the onset of symptoms or clinical diagnosis of cancer. Recognizing the need for consensus definitions and standardized evidence development frameworks for these new types of blood tests, the Early Detection and Screening Working Group of the Blood Profiling Atlas in Cancer Consortium, a collaborative initiative dedicated to advancing standards and best practices, developed and published a lexicon for liquid biopsy–based SCD and MCD tests. During the preparation of the lexicon, the group recognized challenges with regard to the definitions of key terms and concepts describing absolute and RR assessment of intended use populations for cancer screening tests. This article captures the working group’s discussions on (i) risk assessment including considerations for adapting historical SCD risk terminology like “average risk” and “elevated risk” to MCD tests, (ii) the implications of this terminology for describing intended use populations, and (iii) the existing gaps in evidence for determination of absolute risks.

Diabetes is associated with emotional distress and poor mental health, especially for individuals with low income, hindering patients' ability to manage their condition. The health care system's workforce constraints limit its capacity to holistically support patients. To assess the effectiveness of layperson-delivered empathetic engagement over the telephone in helping improve glycemic management for patients with diabetes. This parallel-arm randomized clinical trial with blinded outcome assessment was conducted from February 12, 2022, to April 15, 2023, with final measurements on November 18, 2023, among 260 patients with uncontrolled diabetes from a federally qualified health center in Austin, Texas, engaging telephonically from home. Patients assigned to the intervention group received empathy-oriented telephone calls by community-hired laypeople for 6 months, while those assigned to the control group received usual care. Patients were stratified by baseline score (≥5 vs <5) on the depressive symptom scale of the 9-item Patient Health Questionnaire (PHQ-9). The primary outcome was hemoglobin A1c level at baseline, 3 months, and 6 months, assessed for interaction between time and trial arm. Secondary outcomes were self-perceptions of managing diabetes, diabetes-related behaviors and distress, and mental health symptoms (measured via surveys). Analysis was performed on an intention-to-treat basis. Of 260 participants (mean [SD] age, 49.5 [10.1] years; 163 of 259 women [62.9%]; 176 of 203 [86.7%] with annual income <$40 000) enrolled, 6 withdrew. At 6 months, 204 of 254 (80.3%; intervention, 109 of 127 [85.8%] and control, 95 of 127 [74.8%]) returned for measurements. Participants in the intervention group had statistically significant mean (SD) decreases in hemoglobin A1c level at 6 months (from 10.0% [1.9%] to 9.3% [2.0%]) compared with those in the control group (from 9.8% [1.6%] to 9.7% [2.3%]) (P = .004). The within-person change in hemoglobin A1c level was -0.7% (95% CI, -1.0% to -0.4%) for the intervention group and 0.02% (95% CI, -0.4% to 0.4%) for the control group. For the subgroup with a PHQ-9 score of 5 or more at baseline (38.1% [99 of 260]), the within-person change in hemoglobin A1c was -1.1% (95% CI, -1.8% to -0.5%) for the intervention group and 0.1% (95% CI, -0.7% to 0.8%; P = .004) for the control group. For the subgroup with a PHQ-9 score less than 5, the within-person change in hemoglobin A1c was -0.4% (95% CI, -0.8% to -0.1%) for the intervention group and -0.02% (95% CI, -0.5% to 0.5%; P = .21) for the control group. At 6 months, 91.7% of the participants (99 of 108) responded that the program was very or extremely beneficial. In this randomized clinical trial of telephone-based layperson-delivered empathetic engagement, patients with diabetes and low income achieved clinically meaningful improvements in glycemic control. With workforce constraints, layperson-delivered programs for diabetes show promise. ClinicalTrials.gov Identifier: NCT05173675.

This research aims to describe and analyze the role of digital media as a supporter of Church mission services. Digital media is utilized as one of the facilities for church services and digital media is also a challenge for every church. An optimistic attitude is a real attitude that needs to be instilled and carried out by every church as an effort to improve and achieve service goals and quality. Descriptive qualitative method is a research method through literature research used by the author in conducting research. Literature studies are used by the author as a target to provide a sense of awarness and optimism for the progress of media in the world and the church in seeing and making an opportunity not a threat. The results of the discussion and research provide input to a pastor and every congregational leader in a church regarding what things must be improved and improved optimally to utilize digital media that is and continues to develop in supporting church services and spreading the gospel throughout the world.

ObjectivesSimulation learning is an effective experiential learning tool1; despite this, there have been very few studies around using simulation in sensitive topics such as discrimination. There is concern that simulating such scenarios could cause strong emotional responses and negatively impact learning. It is thought that this should not deter us from using an excellent learning modality to enhance learning around ED&I topics. We attempt to create a session that delivers both psychological safety and learning points. The simulation package aims to empower simulation providers to provide sessions on ED&I themes.MethodsA simulation package was developed consisting of 1- A pre-brief format catered to ED&I simulation to lay the foundation for psychological safety.2 32- Simulations Scenarios: Micro-aggression, unconscious bias, and active bystander. To ensure psychological safety, we have made videos of the scenarios to be used in the sessions instead of live simulations. This also allows for online sessions if appropriate. Each scenario includes recommended reading and debriefing points. The onus is on the facilitator to read before the session.3- A debrief for ED&I flowchart, although debriefing for ED&I sounds intimidating, draws mainly on simulator providers’ existing skill in facilitating group discussions, leading towards the debrief points and navigating difficult learners.44- Post-session support: Links to available mental and emotional support.ResultsA questionnaire was distributed pre- and post-session. 80% of attendees had not attended previous ED&I learning. 40% of attendees were feeling not confident before starting the session. 100% of attendees felt very good after the session. 83% felt the pre-brief prepared them for the session. 100% felt the debrief allowed them to explore their feelings and ideas safely and that the session was delivered in a way that strived to maintain psychological safety. Self-reported confidence increased as per table 1. Attendees expressed feeling optimistic, empowered, and determined, as per image 1. 6270 Table 1 Pre-session Post-session Understanding microaggression 1.5 2.5 Understanding active bystander 1 2 6270 Image 1ConclusionThis package is a step towards making ED&I simulation more accessible and deliverable. This was the pilot course, but more work is going into spreading the course in the Southwest region in collaboration with HEE and then nationally. More pilots are planned around the region, and data collection will continue.ReferencesKolb D, Kolb, DA (1984): Experiential learning: experience as the.Rudolph JW, Raemer DB, Simon R. ABSTRACT Counselors make important contributions to the counseling relationship, and their attachment organizations can influence the strength, quality, and effectiveness of the working alliance (Bucci et al. 2015). This study explored first year counselor education doctoral students’ (n = 7) experiences of being interviewed with the Adult Attachment Interview (AAI; George et al. 1996) as a means of supporting self-awareness and fostering effective counselor-client working alliances. Four superordinate themes emerged: (a) reactions to the AAI interview process, (b) process with AAI feedback, (c) AAI and intrapersonal process, and (d) AAI and interpersonal process. Findings suggest that the AAI is an effective tool in supporting counselor self-awareness regarding attachment strategies and can provide multiple personal and professional benefits, such as increased awareness of conflict and stress management strategies. Implications for clinical practice, counselor education, and future research are discussed.

Abstract The papers in this forum offer an interdisciplinary assessment of the state of the field of Anglican Studies and perspectives on future trajectories. The first three papers, on liturgy, history, and world Anglicanism, offer an assessment of the respective state of these areas of Anglican Studies. The second set, on theology, sociology of religion, and biblical studies, stake out positions on how these disciplines inform the work of Anglican Studies. A concluding essay offers a synthesis of these papers, focusing on the themes of local contexts for Anglicanism, a further complexification of decolonizing processes in Anglicanism, and the critical role of conversation in Anglican Studies regarding disciplines, languages, and power dynamics.
